Consent for Treatment Radiographs: Arizona State Law mandates that radiographs are the property of the doctor, clinic, hospital, or facility where they are taken. They are kept on file as part of the patient record. It is understood and agreed that the payments made to Power Road Chiropractic are for the processing, examination, and interpretation of the radiographs. The radiographic negatives remain the property of this office. The patient has access to their radiographs at any time. Appointments must be made with the doctor to review radiographs. If a request is made by the patient for the radiographs to be viewed by another care practitioner, the radiographs will be sent in a timely manner.
