Youth Residential Services Team Leader Position Description POSITION TITLE: EMPLOYMENT: Youth Residential Services Team Leader (North West) Full Time 76 hours per fortnight (1.0 EFT) Permanent Position CLASSIFICATION: Social and Community Services Employee, Level 6 Pay Point 1 to 3 based on qualifications and experience to be confirmed as per the Social, Community, Home Care and Disability Services Award 2010 SALARY PACKAGE: RESPONSIBLE TO: DAYS AND LOCATION: OVER AWARD: ONCALL/RECALL: Ordinary hourly rate from $36.83 to $38.46 plus 9.5% superannuation and on/recall allowances. POSITION INFORMATION The Youth Residential Services Team Leader will assist with the establishment of the Hope Street First Response Youth Service in Melton; provide leadership and direction to the supported crisis accommodation team including the Residential Support Workers, Case Managers and visiting team members. The Youth Residential Services Team Leader will ensure a best practice culture and commitment to the highest possible quality of service within Hope Street s defined policy and practice environment. The Youth Residential Services Team Leader will work closely with team members providing individual performance management as well as the delivery of the program models and strategic plan via annual individual work plans, supervision and effective communication mechanisms. The Youth Residential Services Team Leader will manage the day to day operational resources with the support and oversight of the Program Manager. The Youth Residential Services Team Leader will achieve optimum service delivery, program implementation and development as well as client outcomes. Compliance in all operational areas including human resource management, service delivery, continual quality improvement and risk management will also be achieved. Hope Street First Response Youth Support Service in Melton This is a new innovative program recently funded by the State Government in response to youth homelessness in the growth corridor of the City of Melton. bedroom unit for young people and their children/families; and an administration building. The design is based on leading specialist youth focused practice. Construction will commence early 2018 with completion by the end of 2018. In the interim a supported crisis response service will be provided operating from two offices in the City of Melton. Service delivery is based on the enhanced youth refuge model aiming to enhance outcomes for young people as a part of a homelessness prevention approach by: diverting young people from the service system where possible; making appropriate referrals to specialist support; responding to complex issues and needs, assisting with the transition to new living arrangements; and linking young people with supports in their local communities. Roles and Responsibilities: 1. Service Delivery Client Services: Ensure that all activities of client service delivery are met in accordance with standards, policies, procedures and practice manuals. Activities such as: Conducting assessments Developing, implementing and reviewing client-centred, case management/support plans determining the need for more specialist interventions (internal and external) provision of financial resources such as HEF, PRB, TILA, Centrelink Income, EYR Brokerage Lead in the support and delivery of therapeutic approaches to service delivery Managing program vacancies, referrals and intake within existing Hope Street and Homelessness Service System procedures When required work directly with young people and their families or support base, where more complex interventions and responses are identified in conjunction with the case manager Provision of a multi-disciplinary approach with internal programs and allied services. Teams: Lead and pro/actively support team members to ensure a high quality professional standards and competencies are met and practiced Maintain physical resources and equipment for safe/peak operation Support team members to build capacity to respond to young people experiencing trauma and complex and/or dual diagnosis related behaviours Conduct annual performance appraisals of team members Support team members with the development of individual workplans Deliver regular formal supervision to team members Make final determinations regarding clients entering or exiting programs Programs: Pro/actively support teams to ensure the highest possible service standards and levels of professionalism, continual quality improvement, compliance and client centred practice according to Hope Street s mission, vision, values, philosophy, standards, systems, policies and procedures Implement the programs within budget Oversee the implementation of program operational financial administration systems and practices to ensure compliance with relevant legislation, funding contracts and guidelines and Hope Street standards and policies and procedures Ensure the Youth Residential Program roster has twenty-four hour seven days a week coverage Reviewed by: CEO Approved by: CEO Date Approved: 22.09.2017 Date to be reviewed: Page 3/6

Perform shift coverage including sleepovers, in the Youth Residential Program when no other staff are available Maintain a safe and secure environment for young people, staff and visitors Facilitate relevant program meetings. 2. Community Capacity Building Establish and maintain effective working relationships with key homelessness and allied support services; In conjunction with the Program Manager lead community capacity building activities in the region and contribute to regional initiatives designed to improve responses to vulnerable young people at risk of transitioning into a cycle of homelessness with a focus on early intervention and diversion from the Homelessness Service System. 3. Professional Development Participate in internal and external professional development including formal supervision from the Programs Manager Develop and implement Annual Work Plan and Annual Performance Appraisal with guidance from the Programs Manager. 4. Administration Maintain and support the program teams to complete statistical records, program data and submissions required for use in Hope Street s internal and Australian Institute of Health and Welfare (AIHW) national data collection, Annual Reports and related activities Provide the Programs Manager with written monthly program reports Provide information that will assist the Programs Manager to prepare submissions targeting service delivery gaps and needs of programs Complete and or oversee the administrative duties to ensure maintenance of day to day program operations including Petty Cash, Time Sheets, Telephone, Emails, Post Vacancies as per Opening Doors Framework, Housing Establishment Funds (HEF) in accordance with the Hope Street Policy and Procedures Administer HEF, Youth Refuge Enhancement funds (Brokerage) and other client resources per Hope Street policy and procedures. 5. Other Other duties as negotiated from time to time with the Programs Manager and Operations Manager. Contribute constructively as a team member for the benefit of the whole organisation and the promotion of Hope Street in the wider community. Key Selection Criteria: 1. Demonstrated knowledge and ability to lead, supervise and motivate staff teams 2. Demonstrated understanding of delivering services to young people in crisis within a case management framework and in a supported crisis accommodation setting responsive to their needs 3. Demonstrated knowledge of client centred, therapeutic approaches within a multidisciplinary team 4. Ability to develop, implement and monitor youth centered programs and capacity building initiatives 5. Demonstrated understanding of the youth homelessness service system and allied services best placed to support and divert young people from homelessness 6. Experience working with partners in provision of integrated, coordinated and seamless responses to achieve referral pathways and service outcomes.
