Our Aim... Our aim in Calderdale is to provide person centred support to client s, with strong emphasis on promoting independence, making informed choices and working in consultation with individuals in order to enhance their quality of life

Our Commitment... St Anne s Calderdale Learning Disabilities Service is Committed.. To provide high quality individual packages of support to clients. To work in consultation with clients, their families, advocates and other professionals to create a unique Person Centred Plan. A plan that sets objectives which reflect the individual goals and aspirations of clients and the support required to meet these. To treat clients with dignity and respect at all times. To promote independence and support clients to enhance their abilities and skills in order that they can take a more active role in the community. To value the individual clients for who they are and promote diversity amongst clients and within the staff team. To encourage and support clients to make informed choices, in using community resources and participating in the life of the local community. To continually develop the skills of the staff team in order to effectively meet the needs of clients. Staff to understand common induction standards, and develop on existing skills and create new skills.

Heatherstones Nursing Service About the Home Heatherstones is a purpose built bungalow in a quiet residential setting overlooking Savile Park Moor. The home provides nursing care for eight adults with learning disabilities and is wheelchair accessible throughout. It has ceiling tracking in all eight bedrooms and in the communal areas. In addition the bathrooms are adapted to suit those with mobility needs. The home is divided into house 1A and 1B. Each property has accommodation for four people, with a separate kitchen, bathroom, living room and bedroom facilities. Heatherstones has been awarded Gold Standard Framework status in recognition of the high quality support it provides to people with complex health and palliative care needs. The service offers a relaxed and friendly home environment with a strong emphasis on providing individualised nursing care and support to the people living there. Heatherstones is able to provide a very specialist level of support to the people who live there. All clients have an individualised person centred plan which supports them to achieve their desired lifestyle. Everyone living at the home is treated as an individual and supported to make decisions and choices in all aspects of their life. Family and friends of clients are welcomed into the home and their contribution is valued.

Heatherstones Nursing Service Activities and Opportunities The home strongly promotes inclusion and people living there are consulted in decisions effecting them and their lives, regardless of their level of ability. The 24 hour nursing support available at the home enables people living there to have specialised treatment in a friendly and relaxed home environment, this extends to end of life and palliative care. But the emphasis is always on supporting people to live as active and fulfilling life as possible. Everyone living at Heatherstones is supported to pursue a range of leisure and social opportunities such as hydrotherapy, day trips, annual holidays and education and occupational activities. People living at the home contribute to the cost of an adapted vehicle to enable access to leisure and social activities.

Phoenix Court Nursing Service Phoenix Court is situated on the on the edge of Todmorden and is easily accessible by car or public transport. It is a registered nursing home providing support to people with learning disabilities. The property offers two separate levels to the building, with access to both via an adjoining staircase. Ground floor apartment, can accommodate people who have complex health needs. It has full access to a patio and garden area and is fully accessible for wheelchair users. Inside it is specially equipped throughout, to aid people to live as independently as possible. Upper level apartment, can accommodate people who are more ambulant. The home also offers support to those who challenge. It has full access to a small but quaint garden and patio area. Both apartments offer 24hour support by highly trained learning disability nurses and support staff who have or are achieving NVQ3. The staff team are also trained in a variety of other specialised subjects to meet individual needs. People living in the apartments are encouraged to personalise their rooms as they choose. Phoenix Court offers a warm, welcoming and friendly environment, with an ever open door, to family, friends and carers of people who live here. Phoenix Court promotes independence, choice and inclusion for all clients, via person centred approaches. Assessments of individual needs both generally and specifically are continually evaluated to ensure the effectiveness of agreed approaches. This evaluation includes the person themselves, their families, and close working along side other professionals.

Brook House Supported Living Service About Brook House Brook House is a grade 2 listed building situated in private grounds which has been split into 2 dwellings. Downstairs is 1 Brook House which is wheelchair accessible. Communal areas include a large dining kitchen and there are 3 spacious bedrooms. There is also a large wet room and separate toilet facility. 2 Brook House is situated upstairs with its own separate entrance. Again the communal areas are generous, having a large dining kitchen, 3 double bedrooms and there are two full bathrooms. Tenants hold assured tenancy agreements with the landlord, which is Space2live. We aim to support people to maintain their tenancy by providing excellent staffing 24 hours a day throughout the year. Each tenant has a person centred plan in which they can choose the way they would like to be supported in all aspects of their life, including planning for the future and trying to achieve their dreams and aspirations Activities and Opportunities Tenants are empowered to make as many choices as possible with as much support as is necessary. This support may involve family, friends, advocates and other professionals. Tenant meetings are held regularly using resources suited to individuals need and are recorded in accessible format Tenants cultural beliefs are observed and acted upon through support. There are many activities that individuals currently choose to take advantage of some of which are; Snoozelen (sensory room), going bowling, cinemas, adult education, hydrotherapy, eating out, enjoying holidays, shopping, day trips, pubs and much, much more. There is a vehicle which tenants can choose to contribute towards if they wish which allows access to both the local and wider community.

Thornhill Road Nursing Service About Thornhill Road Thornhill Road is a seven bedded nursing home for people with a learning disability. As it is a nursing home there is always a registered nurse (learning disability) on duty. The home provides support for people 24 hours a day 365 days a year. The home is a purpose built bungalow with 7 individual bedrooms, 2 lounges, 2 kitchens, 4 bathrooms, a conservatory and garden areas to front and rear. The home is equipped with specialist equipment including tracking, hoists and baths. Thornhill Road has been awarded Gold Standard Framework status in recognition of the high quality support it provides to people with complex health and palliative care needs. Each person has an annual person centred plan to ensure they are fully supported in all areas of their lives.

Hawthorn Street Supported Living Service About Hawthorn Street Hawthorn Street is a detached property in the residential area of Hipperholme. The accommodation is split into two houses that connect through the staff bedroom. The property can provide accommodation for up to seven people of varying ages and gender. The property is owned by St Vincent s and they provide assured tenancy agreements to the tenants living at Hawthorn Street. There are seven bedrooms and each house has a communal lounge, kitchen, bathroom, shower room and laundry.there is a private back garden with outside seating for tenants use. Hawthorn Street provides person centred support to individuals with learning disabilities and some challenging behaviour; we actively involve all tenants to be involved in the planning of their care and support them to be involved in any decision making. All tenants have a person centred plan and are supported to have an annual health check. Staff will also support clients to maintain a healthy lifestyle and promote good health. Staff are well trained and committed to supporting tenants with the upmost respect and dignity. Staff support tenants to use / explore the wider community by using the local amenities i.e. shops, pubs, barbers and chemist. We also support clients to fulfil their hobbies by going to adult education, theatres, football matches, and swimming, snoozeln and sports days. Staff will also support tenants on day trips to the seaside and longer trips, weekends away and holidays abroad and in the UK.

Hawthorn Street Supported Living Service Our Five Core Principles 1. We provide and can demonstrate that services are of the highest quality. 3. We learn to together by sharing, celebrating, and promoting good practice across the whole organisation. 4. We recruit, motivate, and develop a highly competent and compassionate workforce,. 5. We ensure St Anne s is financially sound and delivering value for money.

Sutherland Court Residential Service About Sutherland Court Sutherland Court is a purpose built bungalow in the quiet residential area of Lightcliffe. The home provides residential care for seven clients with learning disabilities. The home has a range of equipment which allows us to provide safe support to clients with added mobility problems and health issues. The home is divided into two properties both have their own front door but access can also be obtained internally. Each client has their own bedroom and has the use of a communal lounge, kitchen, laundry and bathroom. Each bungalow has their own enclosed garden area and there is good off road parking. All the clients at Sutherland Court have a person centred plan and are supported to review these 6 monthly, family are invited to be involved in the process. Clients at Sutherland Court are supported to make as many decisions as they can with regards to the care they receive; staff will also obtain added support from the advocacy service. Staff at Sutherland Court are committed to up holding clients dignity and promoting inclusion in all area of their lives. We support clients to have an annual health check and have close relationships with our GP and district nursing service which ensures that all our clients health needs are met. Clients can also contribute to the cost of an adapted vehicle which means they can access places further away for day trips etc..

Sutherland Court Residential Service Clients at Sutherland Court are supported to enhance their community participation by using the local amenities and the wider leisure facilities like, pubs, cinemas, theatres, swimming pools, adult education classes and bigger venues for concerts. Staff also support clients on holidays either overnight or Monday to Friday.

Cloughside Nursing Service Inclusion Cloughside is situated in the village of Walsden on the outskirts of Todmorden, West Yorkshire and provides support for people with a learning disability who challenge. As a registered nursing home there is around the clock qualified R.N.L.D nurse cover and Cloughside is intensively staffed to meet the diverse needs of clients. Each client is allocated a qualified named nurse who is responsible for assessment of needs. This assessment considers general needs and specific support around behaviours. Planning and implementation of support is through person centred approaches. The effectiveness of agreed strategies and protocols is continually evaluated through multi and inter agency working with other colleagues, professionals and families. Choice Strategies for managing behaviour are developed and undertaken with reference to the Lavigna-Willis Multi-Element Model which advocates a holistic approach to understanding the needs of an individual who displays behaviour that may challenge. By initiating behaviour supports behaviour is managed 80% pro-actively by, e.g. adaptations to the environment, skills teaching and focussed support. Any identified health needs resulting from the ageing process or physical illness are respected and considered on an individualised basis. The home has two people carriers which allow clients to access the wider community whilst excellent local public transport links also facilitate community participation and presence. Glenholme Respite Service About Glenholme is situated in a village on the outskirts of Halifax. The home provides respite for adults with a learning disability, living in the Calderdale area. There are also 3 short stay/emergency beds this is accessed via the community team. Glenholme is a large building with good sized rooms, there are 4 rooms downstairs and the remaining rooms are upstairs. These can be accessed by a lift. There are 2 large lounges downstairs. One is used primarily as a games room and there is also a smaller lounge upstairs. Activities During your stay at Glenholme we are able to offer a wide range of activities, pool, and air hockey, various crafts, games and we also have a computer for guests to use and a Wii games console The staff will support guest to enjoy leisure and social activities out in the community. Glenholme has a mini bus which can be used for social activities, for which there is a charge. Glenholme also have a sensory room for guests to use.
