Our Vision For Your Care: RECOM -GriPS As far as patient care is concerned there is consensus throughout Europe. The quality of healthcare should be increased continuously with close participation of all contributors concerned. To achieve this, you as a service deliverer - whether in a hospital, ambulatory service or in the residential care of the older adult depend on a continuous multilateral exchange of information. Patient-oriented and economically justifiable care will play an important role for you as a player both from a social-political as well as an economical view - independent of the type of institution and / or the treatment services. With RECOM -GriPS we have developed an innovative tool which will support you in achieving this objective. You will receive an expert, knowledge based nursing process orientated documentation system with which you will be able to make data-based decisions. Because, as you know, an optimal use of IT solutions clearly improves the structured, standardised and complete information transfer as well as the patients' safety and the quality of treatment due to an optimised flow of information. Through the interoperability of our tool the configuration of an Electronic Patient Record is within reach. Working interdisciplinary with RECOM -GriPS the continuity of care will be ensured through a standardised, clear language context: the nursing language ENP. ENP stands for European Nursing care Pathways and has been developed out of the necessity for adequate practice-oriented instruments for interdisciplinary communication during the treatment of patients. Meanwhile, institutions incorporating all fields of nursing presently work with ENP. Institutions such as in-patient hospital care, rehabilitation, psychiatry, short-term care, residential facilities, and outpatient care. But there is more: We are offering soon versions of RECOM -GriPS and ENP in English, French, and Italian. Thus, nursing care will become transparent and comparable on an international level! Planning, Documenting and Management with RECOM -GriPS and ENP The core of RECOM -GriPS is the nursing language European Nursing care Pathways (ENP ), which is used for setting up detailed nursing care plans. The system ENP is distinguished through its complexity of the contents, quality and structure, which makes comprehensible, up-to-date current nursing knowledge easily traceable. The version 2.3, ENP consists of 516 single ENP, each containing a nursing diagnosis with the corresponding objectives, interventions and characteristics, etiologies, and resources. These form a system of elements with a pathway-like structure, which we call nursing-related pathways. On a scientific level a comparison (cross mapping) of ENP with the nursing languages NANDA and ICNP has been carried out. The knowledge of the nursing language ENP is contained in the nursing pathways. Using the Buttons C-E-R (Characteristics, Etiologies, Resources) the patient's nursing needs can be further specified. RECOM -GriPS processes your given information for your nursing care plan. 3

Determination e.g. of the characteristics You are offered guiding interventions with the nursing diagnoses. Our search window with a comprehensible keyword list assists you in your search for nursing pathways to set up nursing care plans. During the everyday work with the nursing language ENP in RECOM -GriPS the following links have proven to be most useful: Link between the structured patient history and ENP Link between ENP and LEP time values and nursing staff regulations Link between ENP and ICD-10 and OPS (operation and procedure coding) Currently, we are developing a further function of RECOM -GriPS with which upgrading decisions can be documented using the link between the ENP nursing interventions and the time frames e.g. of the MDK (the German Medical Review Board of the Statutory Health Insurance Funds). A patient/resident-related MDK statistic is presented to you. This function will soon be available. 4

Nursing care plan Nursing careplan with guiding interventions. Nursing report and process record The report module is linked with the ward overview (a navigation platform for your convenience). During the handover it becomes visible at once for which patient/resident there are new report entries. The report entries are list chronologically. Additions to the plan can be entered; the entry is signed and clearly traceable. Assessment A detailed collection of information is the first important step to derive the care needed and to develop a constructive relation with the patient. Much importance is attached to the collection of information, because accurate and detailed data determine the further steps. The aim of the individual assessment of the patient at the beginning of the nursing process is to find out which needs exist and what kind of services the patient requires. Structured anamnesis The use of RECOM -GriPS makes it possible to collect all health relevant data in a structured history taking. Using the stored information, you are able to set up a patient history quickly and clearly. Our history provides you with more than 900 standardised text blocks. There are, of 5

course, fields for entering free text which leave space for individual entries along with the standardised formulations. The links between the structured history with the ENP nursing pathways lead to a differentiated preselection of single diagnoses stored in the system. You have access to all your patients'/residents' data any time and can make changes. The contents of the history comply with the corresponding relevant expert standards. 6

According to the favoured nursing model, RECOM -GriPS shows a preselection. RECOM -GriPS systematically shows the nursing diagnoses derived from the history recorded. The particular assessment of care levels can be entered also in the history record. There is no time-consuming formulating of individual specifications. Risk assessment For the systematic collection of the patients'/residents' individual risks there are numerous assessment tools available for you (pressure sore, pneumonia, consciousness, thrombosis, independence, etc.). Once you have chosen the chart you wish to use in your institution, you will be able to carry out assessments for your patient/resident. A part of the charts (Norton, Braden, Huhn, Tinetti, and Hendrich) is supplied with background information in a PDF file. This knowledge is available to you any time and is currently being completed for the other charts. You are always working with up-to-date nursing science knowledge. Records In RECOM -GriPS there are a number of records available to you, which assist you in your daily nursing documentation, e.g.: Record of fall with standardised text suggestions Drinking schedule a multi-daily plan, which contrasts the target amount with the actual amount taken Mobilisation plan enables also documentation. Vital sign record you are working not only with values, but you are also provided with qualitative information e.g. pulse quality, temperature measurement point Fluid balance sheet here you are also provided with an automatically generated balance of intake and output. Medication Documenting medication with RECOM -GriPS for your patient/resident is simple and safe. Only a few mouse clicks are needed to determine the medicine, the time period, the type of application, the dose together with times, etc., giving you a clearly arranged medication sheet, enabling for easy signing of the medication administered and also the giving and changing of medications. The most recent development: wound management The version 3.1.0.1 offers you a complex wound management instrument. Organised as an interdisciplinary instrument for the assessment and treatment of damages and changes to the skin, you can not only document, but also visualise the success of your treatments through the process illustration. Interventions and descriptions of the situation can virtually be contrasted. The continuity of care is ensured and double documenting is prevented. The essential tasks and objectives of a wound documentation are illustrating the diagnostic results, planning the therapy, and evaluating the effects of the treatment. You will have clear criteria for assessing the wound status and additionally have the opportunity to visualise the wound and to document wound healing. Measuring and storing of wound images is of course also possible. 8

Evaluation As mentioned above, working with RECOM -GriPS has a further dimension: In RECOM -GriPS, the nursing interventions are linked with values of the nursing expenditure, etc. For each patient the nursing input can be determined in minutes without any additional documentation. The data collected are useful for the institution to prove the existing nursing input. These time values can be collected for case cost calculations and personnel calculations of nurses/nursing time. These evaluations can be carried out ward- or living unit-related. The evaluation of LEP time values can be used to analyse capacities of the living units and possibly shift personnel resources. If an interface with the duty schedule is used, the existing personnel times can be viewed as a graph. All RECOM -GriPS data collected can be exported into the statistics program SPSS or Excel, so that individual questions, e.g., "Is the used nursing time distributed homogenously within a DRG?" can be answered. The data collected can be exported institution-related and be evaluated in a tool of one s own. If you are interested, we will be happy to inform you about the different options in RECOM -GriPS. Export of LEP time values can be carried out, as well, and the use of evaluation software for LEP is, of course, also possible. The Europe-wide use of RECOM -GriPS enables comparisons of data evaluations between different countries.
