INFORMATION ABOUT THE OMEGA BLACK COLLEGE TOUR (BCT) Xi Phi, a New York City-based Graduate Chapter of the Omega Psi Phi Fraternity, Inc. and Community Uplift through Perseverance Inc. (C.U.P) sponsors and underwrites the Omega Black College Tour. African-American high school students enrolled in schools in New York City and the broader metropolitan area will visit several Historically Black Colleges and Universities. Over the last 30 years, the vision that the African American experience for youth could be challenged through mentoring, leadership development and exposure to educational opportunities available outside the boundaries of New York City has become an attainable reality. This has resulted in transforming the futures of nearly 900 young people to date. PROGRAM OBJECTIVE The Omega Black College Tour (BCT) strives to stimulate an interest, among select students, in attending one of the publicly or privately supported HBCUs. The long-term objective of the project is to develop the ways and means to assure that qualified high school students in the New York area become familiar with the historic mission of Black Colleges and Universities, academic offerings specific to each institution and their broad range of high quality educational programs outside the classroom. Furthermore, the objective is to identify students who would benefit especially from the stimulating and supportive learning environments so characteristic of Historically Black Colleges and Universities. INFORMATION ABOUT THE BLACK COLLEGE TOUR The upcoming Omega Black College Tour (BCT) is scheduled to take place Saturday, February 18, 2018 through Friday, February 23, 2018, weather permitting. Our ongoing theme regarding this educational endeavor is "Keeping Our Traditions Alive. The reason for this theme is to ensure that our young people and their families continue to be educated about the societal relevance of our historically black colleges and universities. INFORMATION ABOUT THE APPLICATION PROCESS The Official Application Form for the Omega Black College Tour (BCT) explains that: 1. This is an APPLICATION Process for an interview. 2. The results of the interview are used to determine the applicant s qualifications for participation in the Omega Black College Tour. 3. Some of the factors that may determine the applicant s participation are: A. Having the potential for admission to a Historically Black College or University as reflected by your academic record in high school; B. Being involved in extracurricular activities in school. C. Being involved in volunteer work. INFORMATION ABOUT THE APPLICATION PROCESS In order to be considered, student candidates are expected to: o Understand all students selected must be dressed in business or business casual attire during the black college tour. o Complete all parts of the Omega Black College Tour application form and return it by Friday, December 1, 2017. Submit a typed and completed application for consideration. o Come to an in person interview with a parent or guardian physically present. o Submit an official high school transcript, PSAT, SAT or ACT scores if already taken and TWO letters of recommendation from a teacher, counselor, minister, priest, community leader, or education / non profit sector administrator. This individual must speak to your academic and leadership potential. o Arrive at least 15 minutes before your scheduled interview time. o Appropriate interview attire are as follows: Young Men dress shirt, tie, jacket, dress pants and shoes. Young Women blouse, pants suit or skirt (to knee) and dress shoes. Any applicant not dressed appropriately or without a parent or guardian physically present will be required to re-schedule the interview. NOTE: INCOMPLETE APPLICATIONS WILL BE RETURNED AND NOT ACCEPTED There is a non-refundable $25.00 application fee, which must be submitted with the application. INFORMATION ABOUT THE INTERVIEW After the completed application form has been received, applicants will be notified of the date, time and location of their interviews. Please make sure that all of the information on the application is correct. We must be able to contact you, as well as your parent or guardian for notification and confirmation of an interview. 4

PRE AND POST-TOUR WORKSHOPS Selected students for the Omega Black College Tour 2018 must attend all pre college tour workshops and one post college tour workshop. All workshops are designed to enhance a college-bound student s skills (study habits, test taking, etc.) and to get them familiar with the collegiate experience. This will also serve as an opportunity for the students to get to know each other. Please note that pre tour workshops are scheduled to begin tentatively on Saturday, January 6, 2018 and will be hosted every Saturday thereafter until Saturday, February 10, 2018 from 10:00 am until 5:00 pm (except for the first pre tour workshop concluding by 4pm). The closing ceremony is on Sunday, February 25, 2018 starting from 1:00pm 5:00pm. NOTE: The first pre tour workshop on Saturday, January 6, 2018 will be held from 10:00 am 4:00 pm. This workshop is an orientation regarding expectations during and prior to the tour experience for all selected students. All parents or guardians must be in attendance for the entire workshop. PLEASE NOTE THAT ALL SELECTED STUDENTS MUST BE DRESSED IN BUSINESS OR BUSINESS CASUAL ATTIRE DURING THE OMEGA BLACK COLLEGE TOUR & ALL Pre COLLEGE TOUR WORKSHOPS! In addition to the first pre tour workshop, parents or guardians must be in attendance for the Financial Aid workshop, question and answer period final workshop prior to the black college tour (parents or guardians will only be in attendance for less than half of these specific workshops) and closing ceremony during the dates outlined below. The date of the financial aid workshop will be discussed during the student interview. If there are any conflicts regarding workshop attendance, please inform the Co-Chairman of the Black College Tour immediately. Please note that the initial interview, Parent/Student Orientation, and closing ceremony cannot be excluded. Please note the following workshops dates are tentative and subject to change. 1. Sat. SUMMARY REGARDING THE OMEGA BLACK COLLEGE TOUR FEE AND NOTIFICATION OF ACCEPTANCE The cost of the Omega Black College Tour is $450.00 per student and includes transportation, lodging, and lunch served during the majority of pre - and post- Tour Workshops. Please note that the $450.00 tour fee is due by the first workshop date Saturday, January 6, 2018. Those selected to participate must pay for their own lunch and dinner during the tour. Students and their families will be notified of their acceptance by direct phone call and email. INFORMATION ABOUT THE 2018 APPLICATION PROCESS.IN CONCLUSION ALL COMPLETED APPLICATIONS ARE DUE ON FRIDAY, DECEMBER 1, 2017 PLEASE NOTE THAT ALL APPLICATIONS MUST BE SUBMITTED TYPED, NOT HANDWRITTEN. There is a non-refundable $25.00 application fee, which must be submitted with the application. An application without the appropriate documentation and application fee will not be processed. Box 616 New York, N.Y. 10031 Black College Tour 2018 CODE OF BEHAVIOR AGREEMENT FORM (Page 1) The purpose of the Code of Behavior Agreement Form to ensure that the selected student and his or her family understand behavioral expectations during all pre tour workshops and week of the Omega Black College Tour. Please read the following carefully, sign, notarize and mail with the completed application. The specific agreement will be discussed during the student interview and the 1 st workshop. I will clean up after myself at all times on the black college tour bus and hotels. I will observe the 11:00pm curfew. I understand that a violation of the curfew will result in my being sent home at my parents' expense. I will not take hotel property of any kind while on the black college tour. I understand that I must wear dress / professional attire while on this Tour Monday through Friday and during each workshop. I know that I must wear comfortable shoes. I understand that I must follow the instructions of all of all Omega Black College Tour Chaperones, as well as, the Omega Black College Tour Co-Chairman. Any conduct which jeopardizes the safety, security or integrity of the Tour will result in my being sent home at my parents expense. I understand that the black college tour s an educational trip and will complete all of the work assigned during the Pre-Tour Workshops, as well as, while on the Tour. I understand that, although I might not want to participate in scheduled and unscheduled activities, I must. I know that I cannot be left alone. I will remain with the Tour at all times. I know that it is inappropriate for me to disturb fellow black college tour program participants, chaperones, as well as, other hotel guests. Box 616 New York, N.Y. 10031 PERSONAL CONSENT AND RELEASE I hereby authorize Community Uplift through Perseverance, Inc and its affiliates, licensees, successors and assigns (collectively known as, CUP ) to make use of my appearance in connection with the 2018 Omega Black College Tour (the Program ), as follows: 1. To photograph me and any materials I include in my performance (e.g., photographs, audio or video clips); 2. To record my voice, conversation and sounds, including the performance of any musical composition(s), during and in connection with the Program; and 3. To edit, use, reproduce, exhibit and distribute, in whole or in part, in any manner and media now known or hereinafter invented, an unlimited number of times in perpetuity throughout the world, these photographs and recordings, in support of, or to promote, CUP s educational programs and mission. I hereby waive the right to inspect or approve any of the photographs or recordings. I understand that CUP will be the exclusive owner of all of the photographs and recordings. I also agree that CUP may use my name, voice, likeness and biographical material for promotion of the black college tour. I hereby release and hold harmless CUP and those acting pursuant to its authority from liability for any claims by me or any third party in connection with my participation in the Program or the actions of CUP in reliance of this consent and release.
