MFEI Background Medicaid Functional Eligibility Instrument The MFEI-LOC (Medicaid Functional Eligibility Instrument Level of Care) will be used to determine functional eligibility for long term services and supports (LTSS) to meet federal Medicaid law eligibility requirements. It will also meet the federal PASRR requirement. It will replace the current Functional Assessment Instrument (FAI) and Client Assessment, Referral, and Evaluation (CARE) assessment forms. The MFEI-LOC will be used for the following programs: Physical Disabilities (PD) Waiver Traumatic Brain Injuries (TBI) Waiver Frail Elderly (FE) Waiver Program of All-Inclusive Care for the Elderly (PACE) TBI Rehabilitation Facilities (TBIRFs) Nursing Facilities (NFs) (i.e., the CARE population) The MFEI project has also been extended to the HCBS-IDD and ICF-IID programs, but this version is at an earlier stage of development and testing. 3

MFEI Background Medicaid Functional Eligibility Instrument The functional eligibility portion of the tool is based on the interrai- Home Care tool. The revised PASRR portion of the tool is based on the current Kansas PASRR and leading examples from other states, as guided by the PASRR Technical Assistance Center (PTAC). The MFEI underwent field testing by ADRCs with a diverse group of 380 program participants/applicants from December 2014-April 2016. We are currently embarking on another round of testing to examine recent revisions and to verify previous results. Current projected statewide roll-out for the MFEI is early 2019. 4

PASRR Background Pre-Admission Screen and Resident Review The goal of the PASRR program is: 1. To identify individuals with mental illness and/or intellectual disability; 2. To ensure they are placed appropriately, whether in the community or in a nursing facility; and 3. To ensure that they receive the services they require for their mental illness or intellectual disability (wherever they are placed) (PTAC 2018). PASRR Background The PASRR screen identifies nursing facility (NF) applicants who have, or might have, a serious and persistent mental illness (SPMI) and/or an intellectual disability/developmental disability/related condition (ID/DD/RC). o In Kansas, the PASRR screen is part of the CARE program. The CARE Level-1 also determines the nursing facility levelof-care score (i.e., functional eligibility) o The CARE Level-1 is conducted by AAA/ADRC or Hospital based assessors. o NF assessors are allowed to complete the PASRR portion only, for special admission cases only. 6

PASRR Background A Level-2 assessment confirms or disconfirms a positive Level-1 screen, determines whether placement in a nursing facility is appropriate, and indicates the SPMI or ID/DD/RC services that the individual needs. If the PASRR indicates a nursing facility applicant has, or might have, SPMI or ID/DD/RC, states can implement a Level 1.5 desk review to determine if they qualify for any exceptions to a full Level-2 review. Exceptions include primary dementia and the nursing facility special admissions. 7

PASRR Updates The CARE Level-1 program, including the updated PASRR screen, will now use the MFEI assessment. A PASRR for NF assessors form was developed for special admission cases, separate from the MFEI system. This form asks about special admission categories such as terminal illness, temporary medical rehab stay, temporary respite stay, and emergency admission. This form excludes the functional eligibility portion of the MFEI. The PASRR screen, as advised by PTAC, now captures: Suspected SPMI or ID/DD/RC Substance Related Disorders Primary Dementia Diagnosis 8

Kansas PASRR Content Validity Evaluation: Key Steps Complete the questionnaire and cover sheet, and submit to us on/by June 22, 2018. o Complete your review and questionnaire independently of other panel members. o It s OK to consult with colleagues who are not on the panel. o Comments are encouraged, especially when you see room for improvement! o Please keep in mind that we want to keep the PASRR screen reasonably brief.
