myspeare Patient Portal Homepage Your name is displayed If you are a Proxy, click to view their Portal Click to Log Off of the Portal Your upcoming appointments at the Hospital New information on your Portal within the last 7 days. You can click on an item or the button with the red dot to view. Last 3 Hospital visits After logging into the myspeare Patient Portal with your Logon ID and Password, the first screen you will see is the Homepage. The information displaying in the myspeare Patient Portal is based on the data that is included in your personal medical record here at Speare Memorial Hospital. Your name will display at the top of the page. All of the items on the page are clickable allowing you to view the detail. New Information displaying in your Portal within the last 7 days will be listed on the page, and is noted with a red dot. When new information is posted to your myspeare Patient Portal, you will be sent an email message to notify you. There is button in the top right corner to Log Off the Patient Portal. ****************************************************************************** pg. 3
Proxy Patient Access If you have received authorization in person from the Speare Memorial Hospital Health Information Management (Medical Records) to be Proxy to another person s portal, you will see a button to Change Person in the upper right corner of the myspeare Patient Portal Homepage. Once you select Change Person the portal(s) for the patient(s) that you are proxy to will then display. Click on that person s name, and then you are taken to their myspeare Portal homepage. Simply click on the Change Person button once more to return to the portal access selection screen. pg. 4
Contact Us Please note that there is a Contact Us link at the bottom of each page of the myspeare Patient Portal. When you click on the Contact Us button a text box will open for you to type a question about your myspeare Patient Portal. Please include your patient name in your message, this is a secure email. Click on the Send button when finished with your message, and please allow up to 2 business days for a response. pg. 5
What s New When new information has been posted to your myspeare Patient Portal, you will be sent an email to notify you. New information within the last 7 days will display on your Portal with a red dot symbol. The new information will display as bolded text. After you view the new data, the bolding will no longer display. The red dot will also be removed from the button once you have viewed all the detail. 1. The Profile page allows you to view your demographic information, including your address, phone number, birth date, primary care physician, next of kin, etc. The off white text box at the top of the Portal pages have instructions to assist you in navigating through the screens. The Update Profile button allows you to request an update of certain fields in your profile. After you update your profile information on the screen, click on the Submit button at the bottom of page 2. Please note: You will not see the changes display immediately and we ask that you allow 3 business days for your information to be updated by Health Information Management. pg. 7
2. The Billing page allows you to view your previous visits to the Hospital. It will display the Visit (account) Number, the Date of Service, the Total Balance and the amount you are Responsible for. If you click on a Visit (Account) Number you will then be launched to an itemized screen. pg. 8
3. The Appointments page allows you to review your upcoming appointments at the Hospital and to Request an appointment. Click on the appointment to view the details of the appointment. pg. 9
To Request a New Appointment, click on the button and fill out the questions on the screens. Items noted with a red * are required fields. You may request an appointment be scheduled on days and times of the week that work best for your schedule. We will do our best to accommodate your requested days/times. Select the day(s) and time of day(s) that work best for you. You may also enter in comments for the scheduler. pg. 10
Click on the Submit button to send your request to the scheduling department.. Once your appointment request has been submitted, you will be contacted by that department to book the appointment in the system and verify your information. Your request will display on the myspeare Patient Portal as pending until it is booked by the scheduling department. You may click on the appointment at any time to view the details. pg. 11
4. The Medications button contains a list of your medications documented by your Inpatient physician or your ED Physician during your hospital visit at Speare Memorial Hospital. There is always an option to print the information. Click on the medication to view the detail. To navigate back to the previous screen, there is a button on the toolbar to the right, on most of the Portal pages, to go Back to the previous screen. Do not use the back button on your browser, as this will log you out of the Portal. pg. 12
5. The Health Record button contains several different buttons of information within it. They are Health Summary, Allergies, Results, Medications, Reports and Visit History. pg. 13
Allergies & Conditions Your allergies and conditions that have been recorded during your Hospital visit will display here. Clicking on any item listed will reveal more detail, such as the reaction and/or severity of the specific allergy. pg. 14
Results Your Laboratory and Microbiology results are listed within this button. The results are available in the Portal as soon as 36 hours after your tests have been completed. The default sort of the screen is chronologically by Date. Click on any test to see the result, reference ranges and high (H) or low (L) flags. pg. 15
In the Results button, you also have the ability to view the results by the TEST header, which will sort them alphabetically by test name. Medications Your same Medications are also listed with the Health Summary section of the Portal (you may have already viewed your medications under the Medications button from the Portal Homepage). pg. 16
Reports Provider Reports are listed within this button, including your inpatient visit discharge summaries, Cardiology Reports and Radiology Reports. These reports will be available, at the earliest, 36 hours after being signed by the physician. ** Please note that Radiology Images are not available to be viewed on the myspeare Patient Portal** Click on the scrollbar to view the entire report. pg. 17
Visit History Each of your visits to Speare Memorial Hospital will be listed within the Visit History button. Click on each visit to display the details, including the list of discharge medications, forms and patient instructions. pg. 18
Each of these discharge documents can be clicked on to view the information. This is one of the Patient Instructions that was give to you, as the patient, upon discharge. You may also print from the myspeare Patient Portal. pg. 19
Health Summary The Health Summary button allows you to view or print your Patient Health Summary (PHS). This document contains portions of your Speare Memorial Hospital Medical Record, including Allergies, Medications, Procedures, Family History, Problems, Vital Signs and Immunizations. Future Enhancements The myspeare Patient Portal will be enhanced with more functionality in the future as it becomes available, such as Bill Pay, allowing you to pay your hospital charges on-line. Coming in the future will be the ability to view, download or send your Continuity of Care document from the Health Summary button. This will provide you the ability to send your hospital information to your primary care physician.
