Measuring Quality Between Oct 1, 2015 and Sept 31, 2016... 94% of overweight or obese patients were counseled on nutrition and weight management. 74% of patients screened and found to have depression (PHQ9 score > 10) were referred to behavioral health services. 45% of diabetic patients who were seen for 2 or more clinic visits achieved a HA1c value < 9. 12
