II. Service, Activity & Leadership Roles List all the community service activities you have participated in that have benefited the school and/or the community. All service hours are those accrued commencing with freshmen year of high school. Service to BOTH school and community is a requirement for consideration for acceptance in NHS. For each service-oriented activity, have the advisor/sponsor sign your form to verify your active participation. Service-oriented activities that do not bear an advisor s signature will NOT be considered. Service is work performed to help or benefit others. Extra-Curricular and Leadership Activities Document your extra-curricular activities and leadership accrued commencing with freshmen year. List all activities, inside and outside of school, in which you have participated during high school. For example, clubs, teams and musical groups. List all elected or appointed leadership positions held in school, community, or work activities. For each activity have the advisor/sponsor/teacher signatures to verify. Activities that do not bear signatures will NOT be considered. (Family members are NOT eligible for references.) DO NOT list academic honors as these do not constitute service. IV. One-Page Candidate Essay Type a one-page letter to the Faculty Council explaining why you are a great candidate for NHS. Include any information you feel that this form does not cover but is important to your submission of data to the GHS-NHS Faculty Council. This one-page typed letter of clarification may contain information on an aspect of your background, such as family hardships or school transfers that may have limited your involvement in school or community activities, or may be used to clarify further how you fulfill the NHS criteria of Service, Character, and Leadership. The beginning of the letter must bear your name and the end of the letter must again bear your name and your signature. This letter of clarification MUST accompany the application. V. Teacher Recommendation Ask one teacher to complete the recommendation form to evaluate your character, leadership, and service. Submit the form and an envelope to your teacher, who will return the sealed envelope to you. You are responsible for obtaining the recommendation form and submitting it with your application. The envelope must have the teacher s signature across its seal. VI. Signatures I understand that completing this form does not guarantee selection to the Honor Society.
