Fylde Coast Community Dermatology Service Referrer Information Guide

Our services Welcome to our Fylde Coast Community Dermatology Service. This service will deliver a high quality dermatology service to the Blackpool, Fylde and Wyre community via a team of carefully selected specialist dermatologists and nursing staff. The aims of our service are to: Provide a single point of access for all Dermatology referrals (both soon and routine); Treat a range of minor and moderate skin diseases and conditions in a single appointment; Provide a high quality, safe and effective service for patients with routine dermatology conditions; Manage referrals effectively, ensuring urgent cases receive priority attention; Offer convenient clinics at times, including weekend sessions to suit patients; Waiting times maximum four weeks, majority of patient appointments within two weeks. How to refer GPs can refer patients into this service by: NHS e-referral Service (ERS) Once we receive a referral it is assessed against CCG policies and protocols, and then clinically triaged to determine if the referral is routine or urgent. To assist with the triage process, please state on the referral form whether they are urgent or routine. If you have any concerns or wish to discuss a patient s condition or symptoms, please contact our GP Support Line on 01253 231 911. E-mail: thepractice.dermatology@nhs.net Post: The Practice Dermatology Service, Rose House, Bell Lane Office Village, Bell Lane, Little Chalfont, Amersham, Bucks HP6 6FA Examples of conditions we treat Chronic inflammatory dermatoses not requiring consideration of phototherapy, day unit treatment or systemic treatment; Undiagnosed rashes in otherwise well patients; Infections and infestations; Lesions (all lesions should be accurately measured by the referrer and documented). Referrer must also include past history to any services for the same condition; Precancerous skin lesions (actinic keratoses); Bowen s disease; Facial rashes; Undiagnosed skin lesions (except two week wait skin cancer); Urticaria; Pruritis; Nail, hair and scalp disorders, non-scarring alopecias; Low risk BCCs as specified in NICE guidance; Moderate conditions where diagnosis in doubt; Pigmentary disorders; Pigmented lesions where two week fast track referral not indicated. Exclusions The service will not include: Treatment for referrals that do not meet CCG Commissioning Policy Criteria. Such cases will be rejected and referred back to the referrer. Phototherapy; Contact allergy patch testing; Surgical procedures that are considered to be plastic surgery; Removal of large excisions.

Referral Pathway for the Fylde Coast Community Dermatology Service DERMATOLOGY REFERRAL PATHWAY Patient attends GP surgery and requires referral to the Dermatology Service (Referrals from either GP of Nurse Practitioner) Incomplete/rejected referrals return to GP Clinical Triage of Referral within 72 hours of receipt Direct to Secondary Care under 2 Week Rule - suspected Cancer Patient suitable for intermediate service Patient suitable for secondary care only If unable to contact by phone, invite to call letter send to patient Patient contacted by phone and offered choice of intermediate service/location/site Patient contacted by phone and offered choice of secondary care provider Patient contacts provider phone and offered choice of intermediate service/location/site Patient informed that provider will contact them to agree appointment Complete referral on to chosen secondary care provider Provider contacts patient to offer choice of site and book appointment (within 4 weeks) Patient seen in clinic Treatment Community provider offers choice of secondary care provider to patient Discharged Letter of discharged/treatment Plan sent back to patient, GP and initial referring clinician Patient followed up, follow-up appointment to be booked at treatment attendance Onward referral form completed and sent to secondary care provider NB. Fast Track Referrals excluded - if referral made and found to be cancerous, referral must be re-directed within 24 hours

Providing a quality service Our service offers patients a local dermatology service and shorter waiting times, aimed at improving conditions and levels of patient satisfaction. We continually ask our patients how they feel about their care and use this feedback to improve and perfect our service. When a patient visits our service, they will be asked to complete a simple patient satisfaction survey, the results of which will be accurately processed and shared with referrers on a regular basis.
