POSITION: RESPONSIBLE TO: Senior Speech Pathologist Speech Pathologist Manager Allied Health & Equipment Solutions KEY RELATIONSHIPS: People we support Families Support Staff and Community Support Managers Manager Allied Health & Equipment Solutions Other Cootharinga Allied Health Staff External Therapists Community Allied Health Services Medical Personnel JOB PURPOSE: Provide Speech Pathology services to target individual s group.

DUTIES OF SPEECH PATHOLOGIST: 1. Assist people we support, families and/or support workers to identify and prioritise individual s therapy needs. 2. Facilitate the setting of goals of people we support in relation to the Allied Health Service team with the individual, family and/or support worker by: a. Provision of information. b. Undertaking relevant assessment. 3. Collaborate with the people we support, family and/or support worker to develop mutually agreed therapy services. 4. Provide speech pathology, utilising the current Cootharinga model of service delivery, to support the objectives identified by the Allied Health Service. 5. In consultation with individuals, family and/or support workers review therapy services and adjust as required. 6. Participate in the team decision making process. Attend and contribute to team meetings. 7. Maintain accurate records on individuals assessment, intervention, programs and progress. Provide written reports as required. 8. Participate in research and quality assurance activities including the collection and recording of statistical data. Assist in the development of the Allied Health Service. 9. Participate in the development and implementation of relevant staff training programs. 10. Maintain and improve professional knowledge and practice by participating in relevant courses of study, lectures, and teleconferences and through Allied Health Service meetings. 11. Undertake administrative tasks as required. 12. Act in accordance with Workplace Health and Safety requirements and abide by safe working practices. 13. Other duties as required by the Manager Allied Health & Equipment Solutions. ADDITIONAL DUTIES OF SENIOR SPEECH PATHOLOGIST: 1. Monitor and evaluate the effectiveness of services provided by the Speech Pathologist in the achievement of relevant objectives identified by the Allied Health Service. 2. Senior Speech Pathologist to provide professional supervision and general support to Junior Speech Pathologists. Supervise Speech Pathology students as required. 3. Assist the Manager Allied Health & Equipment Solutions in the recruitment and induction of Therapists.

KEY PERFORMANCE INDICATORS FUNCTIONS 1. Assist individuals, families and/or support workers to identify and prioritise individual s therapy needs. KEY PERFORMANCE INDICATORS i) Complete a plan for person we support as part of the prioritisation process for all individuals for whom a request for therapy is received. ii) Through close consultation ensure individuals, families and/or support workers agree with therapy service being delivered. 2. Facilitate the setting of goals of people we support in relation to the Allied Health Service with the individual, family and/or support workers. 3. Collaborate with the individual, family and/or support workers to develop mutually agreed therapy services. 4. Provide Speech Pathology services, utilising the current Cootharinga model of service delivery, to support the objectives identified by the Allied Health Service. i) Undertake relevant assessment of individuals. ii) Provide information to assist individuals, families and/or support workers to understand individual s goals. i) Through close consultation ensure individuals, families and/or support workers agree with therapy service being delivered. i) Complete your role as a therapist by meeting the agreed objectives within specified time frames. ii) Monitor and evaluate the effectiveness of Speech Pathology in the achievement of relevant objectives identified by the Allied Health Service. iii) In consultation with individuals, families and/or support workers review the delivery of therapy services and adjust as required. 5. Maintain accurate records on individual s assessment, intervention, programs and progress. Provide written reports when required. i) Document 100% of individual s assessment results in the individual s therapy file within 15 working days of the assessment being undertaken. ii) Document 100% of therapy intervention in the individual s file within five working days

KEY PERFORMANCE INDICTORS FUNCTIONS 6. Participate in the team decision making process. Attend and contribute to team meetings. KEY PERFORMANCE INDICATORS i) Attend 90% of team meetings, including teleconferences and the Annual Staff Conference at least once every two years. ii) Provide input and advice, when required, on matters relating to Allied Health Service policies and supporting procedures. iii) Attend regional meetings as required. 7. Participate in the development and implementation of relevant staff training programs. i) In cooperation with the Manager Allied Health Service, facilitate the proactive development of training relevant to therapy services. 8. Maintain and improve professional knowledge and practice by participating in relevant courses of study, lectures, teleconferences and meetings. i) Attend at least three courses/seminars per year, which will assist in maintaining or improving your professional knowledge. 9. Undertake administrative tasks. 10. Comply with Cootharinga s Workplace Health & Safety Procedures. i) Complete administrative tasks allocated to you by the Manager Allied Health & Equipment Solutions. ii) Record and provide relevant statistical data to the Manager Allied Health & Equipment Solutions. i) Use of appropriate written reporting procedures. ii) Demonstrate an adherence to safe working practices.......... SELECTION CRITERIA (Senior Speech Pathologist) ESSENTIAL: 1. An appropriate qualification ie. Degree in Speech Pathology. Evidence of Membership with Speech Pathology Australia. 2. Possession of a current Queensland C class drivers licence or equivalent. 3. Clinical skills gained through experience and/or post degree study, which are relevant for working with children, particularly those with an Autism Spectrum Disorder and/or Cerebral Palsy. 4. Ability to exercise initiative and an innovative solving approach to meeting goals of person we support. 5. Well developed interpersonal and communication skills, with the ability to speak and write clearly and concisely. 6. Ability to work effectively within an interdisciplinary team. 7. Ability to achieve agreed objectives under general guidance and limited professional supervision. DESIRABLE: 1. Knowledge of human resource management including Workplace Health and Safety requirements. 2. Managerial skills, including supervision of staff caseload/time management, prioritisation and submission development. 3. Ability to operate computer systems. Notes: 1. Good interpersonal and communication skills, with the ability to speak and write clearly and concisely. DESIRABLE: 1. Ability to work effectively as a member of a multidisciplinary / interdisciplinary team. 2. Ability to operate computer systems. 3.
