The post will be available from 1 st August 2011 to 31 st July 2012. Mental Health services in Northeast Glasgow are provided by integrated health and social work multidisciplinary Community Mental Health Teams (CMHT)s, supported by a Crisis Team and a primary care mental health team. The post is based at the Kirkintilloch Care and treatment centre Community Mental Health Team (CMHT base and at Hunter street homeless team. Admission wards are at Mc Kinnon House Stobhill Hospital), and comprises of 10 sessions in total. The postholder is supported by appropriate medical secretary time, and by junior doctor input for inpatient sessions. Responsibilities will be shared between a total of 19 general adult psychiatrists in Northeast Glasgow, and the successful applicant would be expected to work closely with colleagues to provide a full range of services to inpatients and outpatients. Job Description Locum Consultant Psychiatrist in General Adult Psychiatry Kirkintilloch CMHT Full-Time 10 PAs Current Psychiatric Services Northeast Glasgow Area The psychiatric services in Glasgow are split into three areas of a city wide mental health partnership (MHP); two areas North of the River Clyde and one to the South. The Northeast Glasgow area lies between the River Clyde to the South, the River Kelvin to the West and the Campsie Hills to the North. It covers a population of approximately 350,000 with areas of significant deprivation. Community Services in Northeast Glasgow are well developed. The general adult services are provided by CMHT s based in 6 community resource centres: Kirkintilloch Centre / Strathkelvin Locality (G33, G65, G66, G69), population 85,000. Springpark Centre / Springburn/Possil Locality (G21, G22, G33), population 50,000 Shawpark Centre / Maryhill Locality (G3, G4, G20, G23), population 53,000 The Acute Adult Inpatient Beds are presently located on 2 sites: 1) MacKinnon House on the Stobhill General Hospital site and 2) Parkhead Hospital in East Glasgow. Arran Centre / City Centre Locality (G1, G2, G3, G4, G21, G31, G40) population approx 50,000 Anvil Centre / Mid Area Locality (G32, G33) population approx 50,000 Auchinlea House / East Area Locality (G33, G34, G69, G71) approx 50,000 Stobhill Hospital MacKinnon House is a purpose built acute admission unit and was opened in April 2000 following the closure of Woodilee Hospital. The unit is 6 miles from Parkhead Hospital and 7 miles from the Larkfield Centre. The unit comprises 3 twenty-bedded locality wards (Broadford, Armadale and Struan Wards) and a 12 bedded IPCU (Portree Ward). There is an ECT Suite with dedicated sessional staff, a Life-Skills Department, a mental Health Tribunal suite, Library and on-call facilities. Eriskay is a 15 bedded inpatient unit located directly opposite Mackinnon House on the Stobhill site. This houses addiction in patient beds and day patient facilities for the Northeast Area. The Stobhill site will be housing a number of additional services, these include: New Adolescent Unit, Medium Secure Unit, New building to accommodate inpatient services for east Glasgow to replace Parkhead hospital as well as additional support facilities for these developments. The North Psychotherapy and Psychology Department is adjacent to MacKinnon House. On-site are the Old Age Psychiatry Wards and Day Hospital, Addictions Psychiatry Offices, Liaison Psychiatry Offices, Addiction Inpatient Services, Inpatient Support Services and the Area Management Team/Clinical Director. 6

Stobhill Hospital Site Stobhill hospital comprises Mackinnon House with 3 adult admissions and a 12 bed IPCU, 2 Old age wards, an addiction unit (Eriskay House), psychotherapy, psychology and liaison psychiatry departments are on site. There is an active lifeskills unit attached to Mackinnon House with Occupational Therapy and Physiotherapy/gym. ECT for north of the river is all done in a purpose built ECT suite. There is a medical library and on-call accommodation for SHOs. There is a suite for Mental Health Tribunal hearings Greater Glasgow Health Board s Modernising Mental Health Services strategy is resulting in major investment in new and innovative mental health services, including early intervention and perinatal mental health services. The perinatal mental health services have opened in the Southern General Hospital to the south of the city and Consultants have recently been appointed to roll out the early intervention service for psychosis city wide. A primary care mental health strategy has been established to improve the management of patients whose needs are best met within primary care rather than secondary services. New crisis intervention services are now in place for each CHCP. Description of the Service Homelessness Health Services provide city wide community based health and social care services to people who are homeless in emergency or temporary accommodation. These services include mental health, addictions, GP practice, community nursing, allied health services and multi-disciplinary teams for specific target groups such as homeless families. 7

The Homeless Mental Health Team (HMHT) provides direct access to homeless people and provides assessment and treatment for the whole spectrum of mental health problems. Services are delivered in hostels, homeless units, emergency accommodation units, temporary furnished flats, bed and breakfast accommodation as well as in the premises of voluntary organisations. Services are also delivered in clients own tenancies and in supported accommodation during clients initial resettlement period. Many clients find it difficult to engage due to their chaotic and often transient lifestyles. The service operates on a pro-active, assertive outreach basis. Clients presenting problems are often complex and there is a high level of co-morbidity within the client group. The HMHT provides individual mental health assessment, diagnosis, treatment, information, support, advice and monitor symptoms and side-effects of medication. They also provide targeted interventions e.g. anxiety management. The HMHT operates Monday-Friday 9-5pm Aim of service The aim of the service is to provide direct access to mental health services for homeless people. The team works with people with both severe and enduring and mild to moderate mental health problems. Referrals The team operates an open access referral system. Clients are seen wherever is most convenient to them. Referral Criteria 1. Currently homeless 2. Person with known or suspected mental illness 3. Person aged 16 or over Team Structure 1 Nurse Team Leader 4 Community Charge Nurses 1 Health Care Assistant 0.6 WTE Consultant Psychiatrist 1 Medical Secretary 0.5 WTE Team Secretary Kirkintilloch Clinical Services The Strathkelvin catchment area includes part of East Dunbartonshire and North Lanarkshire, with an 18 65 population of 60,000. The area is a mixture of rural, semi rural and urban communities, ranging from affluence to pockets of deprivation. Much of the service is delivered by the team in or close to the patient s home. However out-patient clinics take place within the centre. There are good links with primary care, and also voluntary and statutory providers within the locality. East Dunbartonshire has a long history of social work involvement within the team, with the team based at Larkfield. There is also an inpatient multi-disciplinary team. The in-patient service has recently benefited from additional funding to improve the nursing staff/patient ratio and 8

improve the therapeutic activities on the ward following the TIDAL model of patient care. Inpatient and community co-operation is further enhanced by a dedicated crisis response team based at Kirkintilloch Initiative Integrated Care Centre which promotes early discharge from hospital and a reduction of admissions where possible. The post holder will be expected to provide additional medical leadership to the teams and assist with the further development of the service. The service is looking to undergo a comprehensive review of configuration of service delivery with the successful applicant being involved in assisting shaping of core services fit for purpose for the 21 st century style of mental health care. Supporting Staff The community multidisciplinary team is based at, Kirkintilloch Initiative Integrated Care Centre. A new purpose integrated care centre within the new developments surrounding the Kirkintilloch Initiative. The team comprises community psychiatric nursing, occupational therapy, psychology and social work with input from pharmacy, dietetics etc and supported by the Operations Manager. The inpatient multi-disciplinary team is presently based at Struan Ward, MacKinnon House, Stobhill Hospital and comprises nursing, lifeskills, pharmacy, with input from community-based psychology, social work, nursing and occupational therapy. The post-holder will develop the new post in consultation with the other medical consultants in post along with other core staff from the CHP and locality. Duties of the Post The post holder will have responsibility for providing a comprehensive range of services in conjunction with the multi-disciplinary team to patients with severe and enduring mental health problems within the Kirkintilloch locality and within the Homeless Mental Health Team for people between the ages of 18 to 65. At Kirkintilloch the consultant will provide clinical leadership, jointly with the other consultants, both for the CMHT and the Crisis team. This will include being involved in allocations meetings, review meetings, out patient work, domiciliary visits and complex case reviews. He/ She will provide consultant care for inpatient in Struan ward Stobhill Hospital. He / She will be an active member of the local management team and assist in supporting service developments and achieving health targets. He or she will be expected to cover for colleagues during absences in line with the arrangements set out in the National Terms and Conditions. He or she must have or be eligible as soon as possible to be approved under s22 of the Mental Health (Care and Treatment) (Scotland) Act 2003 and would act as RMO and AMP to patients. At the Homeless Mental Health Team the consultant will provice a similar service the this client group. The Kirkinilloch/Homeless split will be reflected in the job plan. The post holder will, with the agreement of the clinical director and medical director agree a job plan. An example of programmed activities (4 hours) commitment would be: 9

expected to become actively involved in undergraduate teaching. Each Consultant can expect up to 3 attachments per year, of one student at a time, for 5 weeks. In recognition of the teaching commitment it is likely that the University of Glasgow will grant to the post holder the status of Honorary Clinical Senior Lecturer in Psychological Medicine. Postgraduate In addition to the educational supervision of the attached trainee, the post holder will be expected to contribute to the Post-Graduate Teaching programme of journal clubs, case presentations etc. at the North/Internal Teaching sessions and may also be asked to contribute to the Glasgow Wide/External Teaching Programme. Internal teaching alternates with External teaching on Thursday mornings during term time. Multidisciplinary Teaching There is opportunity for multidisciplinary teaching within the service from time to time and this is encouraged as a contribution to team / staff development. Research There are a number of research projects on going in NorthEast Glasgow and the post holder will be encouraged to undertake/become involved in research where appropriate. Continued Professional Development Study Leave There is a generous study leave budget and medical staff are encouraged to ensure that they keep their practice up to date. Consultants Clinical Governance Meetings There is a regular programme of fortnightly meetings on External Teaching Thursday mornings during term time on topics relevant to consultants and non-training grade staff Multidisciplinary Clinical Meetings Clinical Audit Meetings These multidisciplinary meetings takes place on the second and fourth Tuesday of the month and are attended by managerial and clinical staff on topics relevant to the service. CPD There is a West of Scotland CPD Programme with which the North Consultants Clinical Governance Meetings dovetail. Systems have been implemented to support Consultants appraisal within the Trust. Managerial / Administrative work The Mental Health Division has recently undergone a management restructure primarily to support clinical governance and to focus on clinical service delivery. 11

The current Area Management Team would encourage the post holder to be active in clinical input to managerial planning. Clinical Business Meeting The point of contact between managers and clinicians is at the monthly business meeting where current clinical service issues are discussed. Northeast Division of Psychiatry The post holder would be a member of the Northeast Division of Psychiatry and may represent clinicians views to the Division via the Division s Psychiatric Advisory Committee. The post holder will be encouraged to be an active member of the Division. TERMS AND CONDITIONS OF SERVICE 1. Terms and Conditions of Service The terms and conditions applicable to this post are those of the NHS Greater Glasgow and Clyde. 2. Superannuation There is an NHS Superannuation Scheme, which involves employee contributions of 6% of salary. Employees may opt out of the NHS Superannuation Scheme if they wish. 3. Salary 74,504-100,446 per annum (pro rata for part time) 4. Annual Leave The full annual leave entitlement appropriate to this grade is 6 weeks. In addition there is entitlement to statutory and public holidays as determined for Health Board staff. 5. Hours of duty 10 programmed activities per week. A programmed activity is 4 hours duration. 6. Period of Notice This post will be subject to three months notice on either side 7. No Smoking Policy In recognising the danger to health of smoking and the right of non-smokers to enjoy smoke free air, the Health Board has implemented a smoking ban throughout all Health Board premises, with limited exemption for patients in psychiatric units. 8.
