Program Overview Training format will allow for two separate cohorts of health care professionals to attend targeted Critical Access Hospital (CAH) accreditation training 1. The standards of accreditation training is an 8-hour course that covers core chapters in The Joint Commission (TJC) CAH accreditation manual 2. The Primary Care Medical Home (PCMH) training is a 4-hour class that covers the requirements for TJC PCMH certification and assessment for readiness 3. Arizona Flexibility (AzFlex) program is a 4-hour training on CAH quality indicators Who Should Attend? Any team member who provides guidance and oversight of organizational and departmental TJC regulations/standards managers, supervisors, those appointed to acting supervisory/managerial roles, and any key department staff who need to be familiar with the assessment of TJC Accreditation and readiness for PCMH certification. Why Should You Attend? This training is tailored for Indian Health Service Critical Access Hospitals (CAH), Tribal 638 facilities and regional facilities in northern Arizona. Training attendees will gain increased knowledge about the CAH accreditation standards and Primary Care Medical Home (PCMH). Information Fees will be paid by registering hospitals on behalf of participants; no individual registration fee is required. Complete the attached registration form no later than June 10, 2016. You will receive an e-mail to confirm your registration. Cancellation notice of cancellation must be provided no later than June 20. Limited Seats Available registration will cut off at 75 confirmed participants.
