Patient and Family Centered Care Institute for Patient- and Family- Centered Care: An approach to the planning, delivery, and evaluation of health care that is grounded in mutually beneficial partnerships among health care providers, patients, and families. It redefines the relationships in health care. The IOM (Institute of Medicine): "Providing care that is respectful of and responsive to individual patient preferences, needs, and values, and ensuring that patient values guide all clinical decisions." ... Care must be delivered by systems that are carefully and consciously designed to provide care that is safe, effective, patient-centered, timely, efficient, and equitable. Provide easy and accessible opportunities for patients and families to ask questions of doctors and nurses. Provide care that respects patients values, preferences, and expressed needs.

Provide emotional support in relieving fear and anxiety that accompanies an injury, illness fear of pain, disability or disfigurement, loneliness, financial impact, or the effect of illness on the family.
