your referrals come from? A35 Wound clinics are held for patients under the care of this hospital, held in our outpatients department. Referrals are taken for current in-patients from Consultants, often to facilitate earlier discharge or monitor after discharge; or after patients are discharged here, from GPs, District Nurses, patients self-refer, Cardiac Rehab. *figures provided relate to the period 01/04/2015-31/12/16 when the Trusted implemented new software. Information for the period 01/01/2015-31/03/2015 has been exempted under Section 12 of the Freedom of Information Act. We estimate that the appropriate limit set out by the Act would be breached in order for us to determine whether this historic information is held and if held to then locate, retrieve and extract the relevant information.
