2. Context and Scope HammondCare is an independent Christian charity that exists to improve quality of life for people in need. We specialise in aged and dementia care, palliative care, rehabilitation, mental health services for older people, and other related health and aged care services. HammondCare seeks to embed evidence based best practice in its services and we provide these services through home care, subacute hospitals and residential aged care. Bringing these health, hospital and aged care services together, HammondCare has been able to develop innovative, flexible care models designed to serve people with complex health or aged care needs, regardless of their circumstances. Summary of Position The role of the Allied Health Assistant (Speech Pathology) is to work within the Mission and Mission in Action of HammondCare to ensure a high standard of service is provided to patients of the Hospital, along with appropriate support and advice to their carers. The Allied Health Assistant will participate as a member of the multi-disciplinary team to ensure a patient centred, evidence based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare. The essence of this role: Assist as directed in the provision of effective and efficient Speech Pathology service to clients and appropriate support and advice to their carers Participate as a member of the HammondCare team in the application of the organisation s Mission and values to the pursuit of its objectives in a client centred, continuous quality improvement work environment 3. Key responsibilities Responsibilities Capability Outcomes 1.0 Client Care Treat clients under the direction of a Speech Pathologist with a high standard of professional practice, maintaining the ethics and standards of HammondCare Contribute to client care as part of a multidisciplinary team and maintain good communication with medical, nursing, allied health and other staff and voluntary workers Provide assistance in patient care and/or equipment management during therapy sessions Provide administrative support as required Respect and maintain clients privacy Assist in the preparation for and participate in conducting group sessions as required Assists in clients treatment as directed and in consultation with therapy staff; including but not limited to: o Communication disorders o Swallowing disorders Builds trusting relationships with patients and their families/carers to support effective care provision. Instructs clients in the use and care of communication aids and appliances as instructed by therapist.

Responsibilities Capability Outcomes Assist in preparing and transporting patients to and from therapy sessions Assist in the development of resources for therapy programmes Set priorities in the daily work schedule in consultation with the therapist in the assigned area Observe and report on the status of patients to the treating therapist and other staff as appropriate Clearly document intervention in the patient s clinical records in accordance with documentation guidelines Complete such Speech Pathology competencies as shall be deemed appropriate by HammondCare. Assists with setup of equipment for patient use as requested by therapists Assists in the monitoring of the progress of various treatment modalities and advises therapists of any problems or changes. Completes documentation in medical files in accordance with HammondCare and department policies Completes general office duties Cleans equipment and storage areas are kept tidy as required. Undertakes shopping for the department when required 2.0 Operational Management Activities Attend and participate in team, case, department and other meetings as needed. Reads and understand the Policy and Procedures for the Department and HammondCare, and work within the principles contained therein. Maintain accurate and timely records, reports and statistics on patient care. Participate in department and team education programmes Maintain department equipment, supplies and materials and notify senior therapist of the need for additional supplies, replacements and repairs. Prepare and tidy treatment rooms Participate in planning for and implementation of the Speech Pathology business plan. Behaviour and performance are consistent with professional expectations, HammondCare s MIA and Capability Framework. Attends and participates in relevant meetings. Monthly statistics are completed in specified time frame and will accurately reflect workload. All stores/supplies are checked and provides lists for re-ordering All department equipment and accessories are kept clean and well maintained 3.0 Professional development/education Identify skills and knowledge required to fulfil the role and to attend appropriate staff Participates in department professional development program and activities. Participates in clinical supervision with Speech Pathology Manager

Mandatory training is completed within the required time frames. 4.0 Other Ensure the efficient use of supplies. Ensure all interactions portray HammondCare as a professional and caring provider of Palliative, Rehabilitation, Aged Care Psychiatry and Aged Care services. Comply with formal complaints and grievance procedures of the organisation Communicate openly and honestly with supervisor, manager and colleagues. Notifies supervisor and manager of any incidents, issues or complaints immediately and participates in subsequent investigation and response process.
