1 Welcome to P.R.E.P.! Tolleson Union High School District is committed to supporting optimum health and wellness for district employees. P.R.E.P. (Prevention Reward Employee Program) encourages members to get healthy and stay healthy. Members are encouraged to read the P.R.E.P. guide to gain an understanding of the program specifics, requirements and available options. Why Prevention? Annual recommended wellness screenings take only about eight to ten minutes and can alert you and your doctor of signs or symptoms of serious disease or illness, including diabetes and pre-cancerous cells. According to the CDC, 7 out of 10 deaths among Americans each year are from chronic diseases and 1 out of 2 adults has at least one chronic illness such as cancer or heart disease, many of which are preventable. When found early, most conditions and cancers are highly treatable. One in eight women in the U.S. will be affected by invasive breast cancer in her lifetime, and thirty women each day are diagnosed with cervical cancer. Despite increased awareness and education surrounding women s health, breast cancer is still the leading cause of death among women. Heart disease is still the leading cause of death among men, followed by cancer. Don t wait! 2 P. R. E. P. Program Overview P. R. E. P. is designed to make participation as streamlined as possible while focusing on key practices of good health. Members are asked to complete three (3) wellness activities to qualify for your reward. Think of it as a 2 + 1! Primary Requirements: (Instructions for completing each item can be found on pages 3-5) or benefits are terminated prior to the end of the P.R.E.P. plan period, rewards are forfeited. How long do I have to complete my P.R.E.P. items? TUHSD s plan period for P.R.E.P. is April 1, 2015 through March 31, 2016. You may obtain your wellness exam, take your health survey, and complete one of the other nine options anytime during the P.R.E.P. plan period. 1. Annual Wellness Exam 2. Rally Health Survey 3. Additional Options (choose one) Complete a Smoking Cessation Program (State, county, or district sponsored) Enroll in DPCA Program if applicable (Diabetes Prevention & Control Alliance) Enroll in Healthy Pregnancy Program if applicable Flu Vaccination Preventive Cancer Screening as recommended based upon age & gender (Mammography, Colonoscopy, Prostate Screening, Skin Cancer Screening). For more information about preventive care, visit www.uhcpreventivecare.com Complete eight months of the Work It Off Exercise Program (10 hours of self-reported exercise per month) All three wellness activities must be completed by March 31, 2016 to be eligible for the reward. All documentation must be provided to the P.R.E.P Coordinator (see page 8) by April 15, 2016. How to Participate Review the P.R.E.P. guide and supporting materials to familiarize yourself with the options, instructions and benefits. Decide which of the optional items you would like to complete and review the correlating section of this guide to assist you in successfully completing your choice. Attend 1 or more District-sponsored wellness classes Eligibility & Enrollment TUHSD employees who are enrolled in the District 2600 HDHP Medical Plan are eligible to participate in P.R.E.P. Separate enrollment is not required. Participation is voluntary and at the sole discretion of the employee. See page 7 for reward information. Annual Wellness Visit (Documentation necessary from employee) The primary purpose of obtaining your annual wellness exam is to discover potential risk factors or conditions that require attention, management and/or treatment. If caught early, many diseases or conditions can be successfully treated, controlled or eliminated by following your doctors recommendations. Remember, with Health Care Reform preventative care changes, your annual wellness visit is at no cost to the member. To complete your P.R.E.P. Wellness Visit, you will need to contact your primary care physician and schedule your visit. If you do not have a primary care doctor, you can search for an in-network physician at www.myuhc.com. Use the Provider Search Tool at www.myuhc.com to find a doctor conveniently located in your area, call, make your appointment and be certain to inform the office that your visit is for your Annual Wellness Exam. You can also call United Healthcare s customer service at: 1(866)314-0335 for assistance in finding a physician. For more information regarding preventative care, visit www.uhcpreventivecare.com. Rally Health Survey (No documentation necessary from employee) Take your first step towards a healthier life by taking a personalized health survey at www.myuhc.com. By taking the Rally Health Survey, you can learn your Rally Health Age, engage in personal health missions, and use gaming and social media to support your health journey. The survey takes approximately 10-15 minutes to complete and you will be provided with immediate feedback on the current state of your health. To get started, visit www.myuhc.com. Once you are logged in on the home page, click on the Rally Health Survey link on the right side, or, on the rotating RALLY banner. See pages 14-16 of this guide for detailed flyers and instructions for taking your Rally Health Survey. You do not need to send in proof of completion. Your P.R.E.P. Coordinator receives a quarterly report from UHC with the names only of those who have completed the survey. No scores or survey details are shared with the P.R.E.P. Coordinator or your employer. If you are physically unable to complete the Health Survey, you can fill out the alternate request form (located on page 9) and choose one of the alternate P.R.E.P. options to complete in lieu of the survey. You will need to provide a copy of your EOB (explanation of benefits) via mail, e-mail or fax to the P.R.E.P. Coordinator as proof of completion. Please see page 8 for contact information. Your EOB can be located online at www.myuhc.com.

Additional P.R.E.P. Options Congratulations! You are only one option away from earning your reward! Choose any one of the items below to meet your final goal of 2 + 1. Preventive Cancer Screenings (Documentation necessary from employee) Screening means checking your body for cancer before you have symptoms. Getting screening tests regularly may find breast, cervical, skin, and colon cancers early when treatment is likely to work best. EOB (explanation of benefits) is required for proof of completion. (See page 14 for details on how to obtain your EOB.) Fax, email, or mail to P.R.E.P. Coordinator. Work it Off Exercise Program (Documentation necessary from employee) We know that regular exercise and a commitment to fitness is a vital component of healthy weight loss and overall wellness. The Work it Off Program allows members the freedom to choose their preferred method of exercise. Members must log 10 hours of exercise per month for 8 months to meet program qualifications. Exercise logs can be turned in monthly or at the end of March to the P.R.E.P. Coordinator. A Work it Off log can be found on page 10. DPCA Program (Documentation necessary from employee) Diabetes is one of the fastest growing diseases in the country. More than one in four adults is pre-diabetic and one in ten adults is diabetic. Fortunately, there are actions that can be taken to reduce the prevalence of prediabetes, and the clinical complications associated with diabetes. The Diabetes Prevention and Control Alliance, an innovative new program from United HealthCare, is at the center of the solution. at lowering patient costs and complications related to dia- P. R. E. P. 4 betes. If you have been diagnosed with diabetes or prediabetes, call the appropriate number below to find out if you qualify to enroll. 1(800) 650-2885 for diabetics 1(800) 237-4942 for pre-diabetics If it is determined that you are a qualified candidate for the DPCA program and enroll, please print the DPCA enrollment form verification on page 11 and have your DPCA counselor/advisor sign and return the form via email, fax, or mail to your P.R.E.P. Coordinator. Healthy Pregnancy Program (No documentation necessary from employee) It s more important than ever to take care of your health and your baby s health during your pregnancy. United Healthcare offers this program to provide you with useful information regarding proper nutrition, warning signs, preparing for childbirth, things to avoid during pregnancy and how important it is to exercise during pregnancy. This program is at no cost to you and offers 24 hour toll-free access to experienced maternity nurses, website access for pregnancy and childbirth, educational materials, resources and a complimentary book: Your Journey Through Pregnancy. To enroll, call (800) 411-7984. For more information, visit www.healthy-pregnancy.com. You are not required to send in proof of enrollment. Your P.R.E.P. Coordinator receives a quarterly report of those who have enrolled from UHC. Members of United Healthcare who are diabetic or prediabetic are invited to participate in a free program aimed

5 P. R. E. P. Additional P.R.E.P. Options continued... Flu Vaccinations (Documentation necessary from employee) It s more important than ever to take care of your health. By getting a single flu shot you can protect yourself against the seasonal flu, H1N1, and other forms of the flu virus. There are several ways you can obtain a flu vaccine: Visit your physician or call the member number on your health plan ID card to find a provider near you. EOB (explanation of benefits) is required for proof of completion. Visit myuhc.com to locate your EOB. Fax, email, or mail to P.R.E.P. Coordinator. Visit a network Convenience Care Clinic or one of the participating national United Healthcare network. EOB (explanation of benefits) or receipt of service is required for proof of completion. Fax, email, or mail to P.R.E.P. Coordinator. Visit a school site within your district when notified vaccinations are available on site. Please fax, email, or mail your proof of program completion to P.R.E.P. Coordinator. District-Sponsored Wellness Classes or Wellness Fair (No documentation necessary from employee) Tolleson Union High School District provides employees opportunities throughout the school year to attend free classes on different wellness topics. Employees are required to attend at least one class to qualify for the reward. Classes are typically an hour long. Times, days, and locations are determined by the District and are subject to change. A list of classes can be obtained through District communications or from the District Office. A sign-in sheet will be provided at each class or event for your signature. You must sign in and stay for the entire class to receive credit. Please print legibly. One out of five deaths in America can be attributed to cigarette smoking. Now is the perfect time to make the decision to quit and get paid for making a lifelong commitment to your health! Complete a state, county, or district sponsored cessation program to meet your additional P.R.E.P. requirement. Visit the links below to find a program option that is right for you.

P. 6 REWARDS Congratulations for completing your 2 + 1 P.R.E.P.! 2600 HDHP with HSA Employee Only Tier Reward: $350 HSA Deposit Once verification of completion of all three components of the P.R.E.P. program has been received by the P.R.E.P. Coordinator at the end of the program, members enrolled in the High Deductible Health Plan with a Health Savings Account will receive an additional deposit of $350 in their Optum HealthBank Savings Account in July 2016. 1 Remember you have until April 15th to submit all of your documents to earn your rewards! *All EOBs and claims summaries can be found online at: www.myuhc.com (See page 13 for instructions on how to obtain EOBs.) 1 Valley Schools and TUHSD reserve the right to alter reward date(s) if necessary.

Wellness Exam If a member is unable to complete an Annual Wellness Visit, a physician s note is required confirming the member s inability to complete the visit. Health Survey If a member is physically unable to complete the Health Survey online, the member can fill out the alternate request form at the back of this guide and choose one of the alternate P.R.E.P. options to complete in place of the survey. 3. DPCA Program The DPCA Program is open only to members who are diabetic or pre-diabetic. If a member does not qualify for the DPCA program, they can elect to complete one of the other alternate P.R.E.P. options. 4. Smoking Cessation If a member does not smoke, they may choose to complete one of the other alternate P.R.E.P. options. 5. Flu Vaccinations If a member is physically unable to receive a flu vaccination, they may choose to complete one of the other alternate P.R.E.P. options. 6. Work it Off Exercise Program If a member is unable to participate in the self-reported Work it Off Exercise Program, they may choose to complete one of the other alternate P.R.E.P. options. 7. Healthy Pregnancy Program If a member is unable to participate in the program, they may choose to complete one of the other alternate P.R.E.P. options. 8. Cancer Screenings If a preventive cancer screening is not recommended based upon the person s age and gender, the member may choose to complete one of the other alternate P.R.E.P. options. 9. Wellness Classes or Fair If a member is unable to attend one or more of the District-sponsored wellness classes, or the wellness fair, they may choose to complete one of the other alternate P.R.E.P. options. *If a member is physically unable to complete any of the P.R.E.P. options, please contact the P.R.E.P. Glossary 8 Glossary and Contact Information Annual Wellness Visit Physical examination conducted by your physician to detect possible disease states or risk factors. Some examples of a wellness visit would be: annual physical, well woman exam, well man exam. HDHP High Deductible Health Plan District Sponsored Health Plan Option that focuses on consumerism and is often paired with a health savings account. DPCA Diabetes Prevention and Control Alliance United Healthcare special program to assist and educate diabetics and pre-diabetics. Health Survey Health Assessment An online survey which provides members a score representing their current state of health & alerting members of possible areas for improvement. HSA Health Savings Account Tax free savings account associated with members who are enrolled in a high deductible health plan or consumer driven health plan. Tracking Is Simple, Fun and Full of Rewards Health trackers monitoring weight loss, physical activity and more are tightly integrated with motivating messages and personally relevant information, to keep you inspired. You can also connect with consumer wearables like FitBit and JawBone as well as mobile access. To help you create new healthy habits, coins are awarded every time you engage even in small ways essentially rewarding you every time a health or tracking activity takes place. Coins can be used to enter sweepstakes or to earn company-sponsored rewards. An email notification tells if a reward activity is complete or if you have registered for a sweepstakes. Coins are earned every time a health or tracking activity takes place. Making Healthy Connections With Rally, you can also join an online challenge, share your accomplishments with others through moderated health communities, choose an Avatar, connect with a personal wellness coach or join a competition to increase the fun. Your health survey responses will be kept confidential in accordance with the law and will only be used to provide health and wellness recommendations or conduct other plan activities.
