EMPLICITI caregiver Taking some time out to enjoy the day Tom and Hilde THE STORY BEHIND THE SELFIE I didn t want my mom Hilde to have to face fighting the disease alone; that s why I decided to move back home to care for her. The only thing I wanted her to worry about was fighting her disease. As my mom s caregiver, it was really important that she saw things were getting taken care of, even the little things like buying a gallon of milk. Growing up my mom made many sacrifices and did everything for me, so it s my turn to take care of her. TOM S TIPS As a caregiver, it s important to be the rock, to have everything under control so your loved one never has to worry Find joy and happiness in your life and don t forget to take some time for yourself 3

Dana J. EMPLICITI caregiver Simple acts go a long way towards making him feel comfortable HOW CAN I HELP MY LOVED ONE? Sometimes, help can be as simple as a reassuring touch. Other times, you ll need to take a more active role. Here are some ways to get involved: Encourage your loved one to talk about their illness and concerns. Let them know you re available when they need a listening ear Go to appointments together. You can be there for emotional support or help ask questions and write down information Help your loved one with household chores Plan dietary menus with your loved one 4

EMPLICITI caregiver I ll always be here to support her Wade and Angela THE STORY BEHIND THE SELFIE Nobody wants to get that phone call or hear that your friend has been diagnosed with a serious illness. My best friend Angela needed me and it was my job to help and support her, just like she supported me after my wife died. I didn t think I had to be super strong for her; I would just try to make life as normal as possible that was our normal and that made us more comfortable throughout her treatment. WADE S TIPS Take notes during appointments because there s a lot of information being given and it s hard to remember every detail There will be good days and bad days. You get through the bad days by knowing you re helping your loved one as they take on treatment 5

WHAT SHOULD WE EXPECT? Dana J. EMPLICITI caregiver He didn t always want to discuss his disease and I needed to be okay with that From diagnosis to treatment, there is a lot of important information to understand. The doctor will work with you and your loved one to create a treatment plan The treatment might be given at the doctor s office, a hospital, or a treatment center Your loved one will need periodic tests to see how their treatment is working While on treatment, your loved one may experience side effects Call the doctor for medical advice about side effects If side effects occur, you can help your loved one keep track of any side effects and report them to the doctor Understandably, there will be emotional ups and downs good days, bad days, and in-between days. Be sure to be a good listener and offer a shoulder to lean on no matter how they are feeling 6

Here are some things you ll want to keep track of as you help your loved one throughout their care: Managing appointments: You may need to help schedule doctor visits and keep all their medical records in one place Daily needs: You may need to help with meals, errands, or take on some of your loved one s tasks Transportation needs: You may need to plan for how you ll get them to appointments. If you don t drive or appointments are far away, there are services that can help 7

EMPLICITI caregiver Taking good care of them means taking care of yourself Here are some tips to keep your energy and spirits up: Don t forget to schedule some me time, and do things you enjoy: Stretch Exercise Watch television Go to the movies Talk on the phone Keep up with your own health, including checkups, medications, and other medical needs Reach out to friends and family for additional support. You don t have to be a superhero Join a caregiver support group online. You can learn from and share with others who are in a similar situation. You re not alone. Important Facts About EMPLICITI This is a summary of important information that you need to know about EMPLICITI. Your healthcare team can work with you to help answer any questions you may have about EMPLICITI. Keep this information in a safe place, so you can refer to it before and during your treatment. Look out for these icons as you read: Talk to your healthcare team Call a healthcare provider right away Helpful information to remember What is EMPLICITI? EMPLICITI is a prescription medicine used to treat multiple myeloma in combination with other medications: EMPLICITI (elotuzumab) REVLIMID + + (lenalidomide) Remember to read the safety information for the other medications you are receiving. What are the possible side effects of EMPLICITI? dexamethasone EMPLICITI is for patients who have received one to three prior treatments for their multiple myeloma. EMPLICITI is for adults. The safety and effectiveness of EMPLICITI has not been studied in children. EMPLICITI may cause side effects, some of which can be serious. Your healthcare team will work with you to manage any side effects you may experience throughout your treatment with EMPLICITI. Serious side effects Infusion reactions can happen during your EMPLICITI infusion or within 24 hours after your infusion. Symptoms of an infusion reaction may include: fever chills rash trouble breathing dizziness light-headedness Before every EMPLICITI infusion, your healthcare team will give you medications to help reduce the risk of infusion reactions. If you have an infusion reaction while receiving EMPLICITI, your healthcare provider will slow or stop your infusion and treat your reaction. If you have a severe infusion reaction your healthcare provider may stop your treatment completely. Call your healthcare provider or get medical help right away if you have any of the above symptoms after your infusion with EMPLICITI. Infections may develop in patients who receive EMPLICITI with REVLIMID and dexamethasone, and can be serious. Symptoms of an infection may include: fever flu-like symptoms cough shortness of breath burning with urination painful skin rash Call a healthcare provider right away if you have any of the signs and symptoms of an infection. Developing new cancers (malignancies) is a risk in patients who receive EMPLICITI with REVLIMID and dexamethasone. Your healthcare provider will check you for new cancers during your treatment with EMPLICITI. Talk to your healthcare team to learn more about your risk of developing new cancers if you receive EMPLICITI with REVLIMID and dexamethasone. Continued on next page. 9

Serious side effects (cont d) Liver problems may occur in patients who receive EMPLICITI. Symptoms of liver problems include: tiredness weakness loss of appetite yellowing of your skin or eyes color changes in your stools confusion swelling of the stomach area Your healthcare provider will do blood tests to check your liver function during your treatment with EMPLICITI. Talk to your healthcare team if you have any of the signs and symptoms of liver problems. The most common side effects fatigue diarrhea fever constipation cough numbness, weakness, tingling, or burning pain in your arms or legs sore throat or runny nose upper respiratory tract infection decreased appetite pneumonia These are not all of the possible side effects of EMPLICITI. Call your doctor for medical advice about side effects. You can report any side effects you experience to the FDA by calling 1-800-FDA-1088. What should I discuss with my healthcare team before starting EMPLICITI? Talk to your healthcare team about all of your medical conditions, including if you have an infection. Let your healthcare team know as soon as possible if you are a woman who is pregnant, planning to become pregnant, breastfeeding, or planning to breastfeed. It is not known if EMPLICITI can harm an unborn baby. However, REVLIMID (lenalidomide) may cause birth defects or death of an unborn baby. It is not known if EMPLICITI passes into breastmilk. You should not breastfeed during treatment with EMPLICITI with REVLIMID and dexamethasone. Before receiving EMPLICITI with REVLIMID and dexamethasone females and males must agree to the instructions in the REVLIMID REMS program. The REVLIMID REMS program has specific requirements about birth control (contraception), pregnancy testing, blood donation, and sperm donation that you need to know. Talk to your healthcare team to learn more about REVLIMID. Interactions with other drugs may affect the way EMPLICITI works in your body. Talk to your healthcare team about all of the medicines you are taking before starting EMPLICITI, including: prescription medicines over-the-counter medicines vitamins herbal supplements Please click here to read the Patient Information in the Full Prescribing Information. 10
