REQUEST FOR PROPOSAL FIRE SYSTEMS INSPECTION SERVICES The Folsom Cordova Unified School District, located in Folsom and Rancho Cordova, Sacramento County, California ( District ) is requesting Proposals from qualified firms and individuals to provide inspection of fire alarms and related equipment at District school sites. This Request for Proposals does not commit the Folsom Cordova Unified School District to award a contract or pay any costs incurred in the preparation of a Proposal in response to this request. The District reserves the right to accept all or part of any Proposal or to cancel in part or in its entirety this Request for Proposal. The District further reserves the right to accept the Proposal(s) that it considers to be in the best interest of the District. INSTRUCTIONS FOR SUBMITTAL OF PROPOSALS I. GENERAL INSTRUCTIONS A. Submittal of Proposals Proposals should be reviewed for accuracy before submission to the District since said document may not be adjusted after submission to the District. The District will not be responsible for errors or omissions in any Proposal. The District reserves the right to reject any and all Proposals, or to waive any irregularities, or informalities in the Proposals B. Signatures All Proposals must include a signature of an authorized officer of the firm. C. Disqualified Proposals Any PROPOSAL received after 3:00 p.m. June 12, 2018 shall be returned to the firm unopened. D. Withdrawal of Proposals Firms may withdraw their SOQ, either personally or by written request, at any time prior to 3:00 p.m. on June 12, 2018. E. Copies of Proposals Each firm submitting a Proposal includes the original plus two (2) printed copies of the signed original along with an emailed pdf copy. F. District Required Forms Your response must include the District Required Forms that are enclosed herewith in Appendix 1, which include the Offer to Enter Into Contract; Terms and Conditions; Qualification Certification and Non-Collusion Affidavit. H. Rights of the District/Responsibilities of Submitters The District reserves the right to incorporate terms and conditions it determines to be proper or necessary into any contract negotiated as a result of a proposal in response to this RFP. The District reserves the right to award one or more contracts, or not to award any contracts, pursuant to this RFP. The Folsom Cordova Unified School District Board of Education reserves the right to accept or reject any or all Proposals, with or without interviews, and to negotiate with one or more respondents. Submitters shall be responsible for any expenses that they may incur in preparing Proposals. Responses will be used as the foundation for the development of an agreement and contract with specific provisions subject to review, negotiations, and approval of the Folsom Cordova Unified School District Board of Education and Assistant Superintendent Business Services. I. Licensing Requirements/DIR Registration District facility and maintenance service providers must hold current licenses, registrations and certifications pertinent to their field or trade, and must register yearly with the CA State Dept. of Industrial Relations (DIR). Please furnish evidence of such registrations/licenses numbers with your Proposal. J. Prevailing Wage District facility and maintenance service providers are required to pay Prevailing Wage rates to employees. K. Fingerprint/DOJ Clearance In conformance with CA Ed Code Sec. 45125.1, employees of consultants and contractors must be fingerprint-cleared before working at any school site. Instructions will be provided in contract documents. Page 3 of 13

II. OVERVIEW OF THE DISTRICT As of August 1, 2017, the District supports 20 elementary schools, one K-8 charter school, four middle schools, three comprehensive high schools, two continuation high schools, an independent study high school and a community day school. These sites make up a total enrollment of approximately 21,000 students. III. DESCRIPTION OF SERVICES REQUIRED The District requires, at a minimum, the following: Annual Fire Alarm Inspection per NFPA requirements at all of the sites listed on Exhibit A, attached. Annual inspection and testing of all sites and equipment listed on Exhibit A shall begin upon District s notification to the selected contractor on or around June 22, 2018, and must be completed no later than August 15, 2018. Contractor shall then provide its full written report of the inspections, no later than September 14, 2018. If discrepancies are found during the testing, the Contractor is to notify the District with a detailed written report as soon as possible, and not later than 30 days from the discovery date. Testing shall include diagnostic tests of control panel equipment, auxiliary monitoring functions, functional test of audible and visual signaling devices and smoke/heat/duct detectors, inspection of manual pull stations, water flow and tamper switches. The District requires a signed Inspection and Test Report listing all devices checked In addition to the above annual inspections, the District is seeking a firm equipped to provide Emergency Service Response, whenever needed, on a 24/7 basis. The above is a general list of responsibilities. The District may add to this list, depending on additional needs and requirements. Page 4 of 13

IV. CONTENTS OF REQUESTS FOR PROPOSALS Your proposal should contain the following: A. Submittal Letter Include a brief cover letter, signed by a principal of the firm. B. Table of Contents Include complete and clear listings of headings and pages to allow easy reference to key information. C. Description of Firm This section should provide the District information regarding the size, location, nature of work performed, years in business, and approach that will be used in meeting the needs of the District D. Licensing and DIR Registration Requirement Please furnish copies/numbers of all pertinent licenses and your current CA DIR Registration number. E. References Identify school district clients of similar size and provide names/emails of your contacts. F. Fees Please provide your quote for yearly services for all sites listed on Exhibit A. Additionally please provide your fee for emergency response services. G. Legal Issues Please respond to each of the following questions: Is there now pending any legal action pending against your firm by a public agency related to services, or lack thereof, provided by your firm? Have there been any settlements or judgments involving such actions within the last ten (10) years? Please describe each such settlement or judgment, including the nature of the action and the amount of recovery? H. Other Each firm is encouraged to provide any additional information or description of resources that are pertinent for this project. Page 5 of 13

V. EVALUATION OF PROPOSALS Proposals will be reviewed and evaluated by a panel of District stakeholders based on the following criteria, and the District s required services, described in Sec. IV above. SELECTION CRITERIA Understanding of, and responsiveness to, this RFP Knowledge, training and experience of your team Past experience working with school districts, with facilities similar to Folsom Cordova Unified School District The ability to provide high quality services in a timely and cost effective manner Strength of references VI. AWARD OF CONTRACT In addition to review of the Proposals, the District may conduct optional interviews, either in person or by phone. TERMS AND CONDITIONS 1. INSTRUCTIONS AND USE OF FORMS. In order to preserve uniformity and to facilitate the award of contracts, Respondents shall complete and return the enclosed District-required forms with their RFP responses. 2. ACCEPTANCE OR REJECTION OF RESPONSES. The District reserves the right to reject any and all Responses that are incomplete, contain errors, arrive after the due date/time or are submitted by unqualified firms. The District reserves the right not to award t a contract if the District, in its sole discretion, deems the responses received pursuant to this Request for RFPs lacking in any respect or insufficient to meet the District s requirements and needs. 3. PROPOSAL PROTESTS. The following instructions must be followed by a Respondent who wishes to challenge the District s selection and award of any contract pursuant to this RFP: a) Any protest must be submitted in writing to the Assistant Superintendent Business Services, Folsom Unified School District, 1965 Birkmont Drive, Rancho Cordova, CA 95742, before 3:00 p.m. on the fifth (5 th ) business day following the District's notification of its intention to award a contract pursuant to this Request for RFPs. b) Only vendors who submitted a proposal in response to this RFP may file a protest. c) Protests must contain the following specific information: Protestor's name, address, telephone number and email address; Date on which protestor's Response was submitted to the District; Protestor's specific, detailed basis for the protest, which must be supported by facts and/or documentation. Protests based on hearsay, feelings or opinions not supported by facts, will be deemed invalid. d) The protestor shall send a copy of the initial protest document and any attached documentation to all other parties that may be affected financially by the outcome. e) The District will review and evaluate the protest for validity, including, if required, review by outside counsel. The District and/or counsel will provide a response within ten (10) days of review of the protest letter. f) If upon review, the proposal protest is found to be frivolous or lacking validity, the protest will be rejected and the protesting party may be deemed ineligible to participate in future District RFPs or contracts. 4. ASSIGNMENT PROHIBITED. No contract awarded under this proposal shall be assigned without the express, prior written approval of the District. Any attempted assignment in violation of the provision may be voided at the option of the Board of Trustees. 5. NO CONTACT WITH BOARD OF TRUSTEES. Respondents may not contact any member of the Folsom Cordova Board of Trustees (or any District employee, except as provided herein) regarding this RFP, unless specifically invited to an interview conducted by the Board. 6. NON-DISCRIMINATION. The Folsom Cordova Unified School District does not discriminate in the selection, acceptance, or treatment of any contractor based upon race, color, national origin, religion, sex, sexual orientation, handicap, age, veterans status, medical condition as defined in Section 12926 of the California Government Code, ancestry, marital status, or citizenship, within the limits imposed by law.
