SECTION B Please submit one (1) complete copy of your scholarship packet for each scholarship you qualify for in Section B. Coast Hills Gold Standard Scholarship - $2,500 Must be an entering student from a Ventura County high school to Oxnard College Earned a minimum cumulative GPA of 3.0 Pursuing a degree or transfer to a baccalaureate institution Be a U.S. citizen or have permanent resident status In addition to the Scholarship Essay Questionnaire, you must submit a one-page essay titled How the CoastHills Gold Standard Scholarship will help me overcome challenges and pursue my passion. CoastHills committee selects recipient CoastHills Gold Standard Scholarship Awards Luncheon event at Cliffs Resort, Shell Beach on June 10, 2018. The luncheon will include the recipient and up to 3 guests. An overnight stay at the Cliffs (1 room) is included with scholarship award, with breakfast for registered guests the next morning Lori Colby Memorial - $500 Must be entering student at Oxnard College Earned a minimum cumulative GPA of 3.0 Must be pursuing a degree or transfer in one of the below STEM Majors: Math, Physics, Chemistry, Computer Science/Information Systems, Engineering: Electrical, Computer, Chemical, Mechanical, Civil and Nano Material Sciences: Computer, Environmental Lori Colby committee selects recipient Culinary Restaurant Management Scholarship for CRM Entering Student - $500 Must be an entering student majoring in culinary and/or restaurant management at Oxnard College Earned a minimum cumulative GPA of 2.5 Must be enrolled in two CRM classes at Oxnard College 1 st year student Culinary/Restaurant Management Department selects the recipient John K. ADDITIONAL ESSAY REQUIREMENTS Please note that additional essays and/or other documentation is required for many of the scholarships offered. VIII. APPLICATION CERTIFICATION (Please save and print the Original Application at this point, packet must be signed and dated. Use the Original Application to make (1) complete set for Section A and (1) complete set for each scholarship that you re applying for in Section B. Each copy is then distributed to the individual donors for their selection committees. I certify that I have completed the original scholarship essay; I met all qualifications for each of the scholarships marked in my application and all statements in this application are true and correct. I understand that falsification is cause for voiding this application. I hereby grant permission to Oxnard College and the Oxnard College Foundation to share and disclose personal and financial information with members of the appropriate selection committee. I hereby authorize Oxnard College and the Oxnard College Foundation to release any information contained on my Scholarship Application, including my GPA, to any organization legitimately wishing to consider me as a scholarship recipient. I understand that Oxnard College and the Oxnard College Foundation will publicize this information. I release the right to use my name and photograph for all publication, reports and press releases. I understand the Oxnard College Foundation scholarships will reimburse one computer per student, not to exceed the amount of the original scholarship award amount, no replacements, and no exceptions. To claim OCF Scholarship Reimbursement Funds, you MUST submit your out-of-pocket receipts which cover tuition, fees and/or books and educational supplies. Proof of enrollment is required and in some cases, unofficial transcripts and class syllabus. All unclaimed funds revert back to the OCF Scholarship Fund after two (2) years.
