Quality Improvement Experience UKAS /ISO 15189 Cheshire Pathology services is a collaboration between east Cheshire NHS trust and Mid Cheshire NHS trust, I have been the clinical lead and Laboratory director since December 2013, since then all disciplines have achieved CPA and UKAS accreditation, one of the first few laboratories in the UK to have achieved in the first inspection following the introduction of UKAS and ISO. Microbiology modernization and service transformation Actively led on a microbiology service transformation, including combining two laboratories on a single site which has led to improved efficiency better turn around times and improved resilience. Microbiology service is now provided on a single site for two acute trusts and primary care for Cheshire. This has also resulted in efficient and resilient clinical service, collaborative working, standardization and compliance with RCPath KPIs. Audits Blood culture Contamination (July 2014) Conducted an audit on Blood culture contamination at Macclesfield Hospital followed by a quality improvement project, which was presented at FIS 2014 as a poster. Point Prevalence Audits (Yearly ) Yearly point prevalence audits on compliance with antibiotic policy, and presented at the Northwest infection control group and the antimicrobial pharmacist group. Audit on Root Cause Analysis of MRSA/MSSA Infection The audit looked at the data gathered from the Root Cause Analysis forms of MRSA and MSSA infections from the Southern Trust. Data was analysed from January 2009 to September 2009. Following the analysis new RCA forms were devised and reaudit is planned in January 2010.
