Who is eligible to apply for a Chapter Grant? Eligibility Funds must be directed to Minnesota... Low income / free and reduced lunch students At-risk youth, grade school through high school Underserved youth Youth with a sudden or unexpected family income change (e.g., job loss, serious health condition. Grants may provide financial support for the following purposes: Clinic insurance, field rental, necessary equipment to run clinics Free, gently used or new equipment PE equipment (soft stick) donated to elementary and middle schools Financial aid/scholarship for a player s high school team fees Partial assistance for a high school student to become an official or umpire US Lacrosse membership fees to participate in community or school youth league Grants may not be utilized for the following purposes: Booster club fees, dues, team clothing Off-season club, select, elite, or travel teams Tournament travel Coaching stipends or salaries Development of a new team or association 1
Capital projects Any purpose that is not a direct benefit to the intended target audience for this gift When do ATG Legacy Project Grant Requests need to be submitted? ATG Legacy Project Grant requests may be submitted quarterly during the calendar year. Grants submission dates include: January 15 th April 15 th July 15 th October 15 th How do I apply for an ATG Legacy Project Grant? Applicants are required to provide a formal grant request, which contains the following information: Cover letter which includes a contact name and mailing address Request Document must include the following headlines and supporting information: o Grant Request Statement Specifically state what you are requesting (financial support, in-kind donation, equipment, etc.) State how the grant will be utilized and identify your target audience o Where applicable, an Executive Summary Provide information pertaining to the formation of your organization, (when incorporated, why, etc.) Provide non-profit status, EIN number (if applicable), and USL membership number Mission statement o Where applicable, Organization Information Provide names, titles, and affiliations of your board of directors or other volunteer members 2
Identify any salaries or stipends paid out to members of your board or paid staff Detail all avenues of financial support you receive Donations, fundraising events, other grants o Where applicable, summary of organization s most recent initiatives Are participants USL members o Where applicable, summary of upcoming year goals and objectives If your ATG Legacy Prject Grant request is for: 1. Introductory Clinics a. A portion of the funds shall be used to run free introductory clinics to low income, at-risk, or underserved youth i. Funds may be used for clinic insurance, field rental, necessary equipment to run clinics, or other items as appropriate for the direct benefit of the targeted audience 2. Sports Equipment a. Free, gently used or new equipment should be made available to youth or high school players i. A request form must be completed by the player s family or their coach based upon the following criteria 1) Player must be on free or reduced lunch OR 2) Player family may have a sudden or on-going financial emergency OR 3) Player family may have a sudden or on-going medical situation which has a financial impact OR 4) Player parent(s) are on active military deployment 3
b. PE equipment (soft stick) donated to elementary and middle schools i. Schools should have a free or reduced lunch program of 75% or higher 3. Financial Assistance a. Through an application request form, players may request financial aid/scholarship b. Partial Assistance for a player s high school team fees i. Request form submitted by family or coach ii. Request must be endorsed by the student s principal or athletic director to ensure the student meets at least one of the requirements as outlined in 2.a.i.1-4 c. Partial assistance for a high school student to become and official or umpire i. Request form submitted by player family, coach, or UMLOA/MULA Executive Board Member ii. Request must be endorsed by student s principal or athletic director to ensure the students meets at least one of the requirements as outlined in 2.a.i.1-4 d. MLA to provide US Lacrosse membership fees to participate in community or school youth league i. Requesting families must meet at least one of the requirements as outlined in 2.a.i.1-4 Is there a limit to the number of ATG Legacy Project Grant requests I can make a calendar year? Yes. You may not submit more than one ATG Legacy Project Chapter Grant to the Minnesota Lacrosse Association per calendar year. When will I know if I have been approved for an ATG Legacy Project Grant? Grant applicants will be notified by USPS mail and/or email within 60 days of the submission deadline. Awardee Grant Requirements: Grant awardees are required to meet all Restricted Gift directives as indicated in the grant award letter. Once funds are utilized by the organization or individual (as the case may be), the Grant Follow-Up Form located on the Minnesota Lacrosse Association website must be completed and emailed (mnchapterlacrosse@gmail.com ) to the MLA within 60 days. Failure to complete this form will make the organization ineligible to apply for a grant the following calendar year.
