Acadian Care Telemedicine Welcome to Acadian Care s telemedicine services. Telemedicine offers many advantages for our patients and is an excellent medium to complement traditional in person care. Some of the advantages for our patients are: saving time, being able to address problems faster (as opposed to scheduling a an appointment and coming in for that appointment), keeping patients off of the roads and highways, flexibility to keep appointments if there are weather conditions or transportation issues, being able to work an appointment into a patient s busy day, etc. Please know, telemedicine, by law, will be covered by your insurance at the same rate as in office visits. Acadian Care will not charge extra for telemedicine services. Please take the time to read through this document including important telemedicine disclosure information at the end of this document. How to Use Our Telemedicine Services Telemedicine only works on the following devices: iphones, Android phones, and PC s with the Chrome Browser. Please make sure you re on wifi or a direct internet connection as this program/app does use data. For iphone: Go to App Store and download the app doxy.me
Run the app on your phone (remember to connect to wifi) Select Patient Enter room URL, type: ac.doxy.me/k Type in your real name so we know who you are and the password is the word green, all lower case. Press the button Check in or Enter Room Read the Terms of Service It should say that you are in the waiting room with a green light that your clinician is online. If you see the red light, we are not online. Wait about 5-10 minutes, to see if we show up and recognize you and call the clinic to let them know you are ready. If we are online (green light), then we see you, and you may be waiting with other patients and will be helped in the order that you connected. You can send a message by pressing the message button in the upper right corner of your app. While waiting: Please read through the disclosure, it s important information Position yourself so there is bright filtered light (like a lamp or window) in front of your face and behind your device so you can be more easily seen. Please do not stare into bright lights directly. Look for messages sent to you by your clinician while you re waiting. Wait a few moments, look for your picture on the webcam display. If you don t see yourself, your webcam may not be enabled, and you won t be able to use this service. Possibly switch to a smartphone and follow the instructions above. Wait about 10 minutes and then call the clinic to see if they are running late. If they are online, you may be waiting with other patients and will be helped in the order that you connected. Telemedicine Disclosure Telemedicine is an optional service, you may decline to receive telemedicine service and withdraw from the telemedicine service at any time. When utilizing telemedicine, you must agree to contact this clinic using telemedicine services only while you are physically located within the state of Louisiana. Please do not attempt to utilize telemedicine services when you are in another state or country. You are consenting to telemedicine services for the following clinicians: Richard F. Follow up care may be provided in person at any of Acadian Care s clinics or you may continue to utilize telemedicine. Please know, if you are taking a controlled substance (like many ADHD medicines) then you will be asked to return to the clinic regularly between telemedicine appointments. For medical records requests, please contact our medical records department at 985-781-7353. If we lose contact due to a technology or equipment failure, you may contact us again using the telemedicine service, call our office at 985-781-7353, or if it is an emergency, proceed to the nearest emergency room or call 911. Alternatively, you may also come to the clinic to be seen in person. Privacy Practices: Our Notice of Privacy Practices provides information about how we may use and disclose protected health information about our patients. By using this service, I consent to the use or disclosure of protected health information by Acadian Care LLC for the purpose of diagnosing or providing treatment to the designated patient, obtaining payment for services provided, or to conduct healthcare operations. I understand this protected health information includes medical, billing, and demographic information about the above patient collected from me and created or received by Acadian Care for treatment, payment, and healthcare operations. I understand that diagnosis or treatment of the above patient by Acadian Care may be conditioned upon my consent as evidenced by my signature on this document. I understand I have the right to request a restriction in writing as to how my protected health information is used or disclosed to carry out treatment, payment, or healthcare operations of the practice. Acadian Care is not required to agree to the requested restriction. However, if Acadian Care does agree to the requested restriction, the
restriction is binding. I have the right to revoke this consent, in writing, at any time. The revocation will be effective upon receipt, except to the extent that Acadian Care has taken action in reliance on this consent. I understand I have the right to review Acadian Care s Notice of Privacy Practices prior to agreeing to this service. The Acadian Care Notice of Privacy Practices has been made available to me. The Notice of Privacy Practices describes the types of uses and disclosures of my protected health information that will occur in my treatment, payment on my account, or in the performance of health care operations of Acadian Care. This Notice of Privacy Practices also describes my rights and Acadian Care s duties with respect to my protected health information. Acadian Care reserves the right to change their privacy practices described in the Notice of Privacy Practices. I may obtain a revised notice of privacy practices by contacting the office and requesting a revised copy be presented to me at my next appointment or to be sent to me by mail.
