Conditions: You are assigned as a Human Resources (HR) Specialist in the S-1 section in a deployed environment. You receive notification a Soldier in your unit has become a casualty and must prepare a casualty report. You have access to the Soldier's Enlisted Record Brief (ERB)/Officer Record Brief (ORB), DD Form 93 (Record of Emergency Data), SGLV 8286 (Servicemembers Group Life Insurance Election and Certification), DA Form 1156 (Casualty Feeder Report), AR 638-8 (Army Casualty Program), DA PAM 638-8 (Procedures for the Army Casualty Program), FM 1-0 (HR Support ), the Defense Casualty Information Processing System-Casualty Forward (DCIPS-CF) system, User Guides, and standard office workstation and supplies. This task should not be trained in MOPP 4. Standards: Without error, identify the correct type of casualty report to submit, extract and verify data from the DA Form 1156/Soldier's HR record, and prepare a casualty report using DCIPS-CF. (4) SSN - Enter the casualty s SSN, to include dashes. (5) Rank - Select the casualty s rank. (6) Name - Enter the casualty s last name/first name/middle name/suffix (e.g., Jr., Sr., III). Page 3
(7) Service - Enter the casualty s component (U.S. Army). (8) Unit - Enter the casualty s unit of assignment. (9) Inflicting Force (hostile) - If casualty occurred as a result of hostile activities, enter the inflicting force code (Enemy, Ally, US (buddy), or UNK). (10) Incident Date/Time - Enter the local date and time (24 hour) of incident. Do not use Zulu time. Use format (YYYYMMDD/HH:MM). (11) Place of Incident - Enter the city/state/country location of the incident. (12) Circumstances - Report all known facts. c. Verify data from a Soldier's ERB/ORB. (1) Ensure the name on the ERB matches DA Form 1156. (2) Ensure the rank on the ERB matches DA Form 1156. (3) Ensure the current unit of assignment in Section IX, Assignment Information on the ERB matches with DA Form 1156. d. Verify data from a Soldier's Record of Emergency Data (DD Form 93). (1) Ensure the Social Security Number on the DD Form 93 matches DA Form 1156. (2) Ensure the name on the DD Form 93 matches DA Form 1156. (3) Ensure the rank on the DD Form 93 matches DA Form 1156. e. Verify data from a Soldier's Servicemember's Group Life Insurance (SGLV-8286). (1) Ensure the Social Security Number on the SGLV-8286 matches DA Form 1156. (2) Ensure the rank on the SGLV-8286 matches DA Form 1156. (3) Ensure the name on the SGLV-8286 matches DA Form 1156. 3. If the Soldier fails any performance measure, show what was done wrong and how to do it correctly. Evaluation Preparation: This task can be evaluated by use of performance measures. This method of evaluation is appropriate if the Soldier performs the task on the job. Allow the Soldier to practice until the Soldier feels qualified and prepared for the evaluation.
