O II. bjectives I. Review process for the Collection of Type & Screen Specimens. II Be able to identify and follow the Type & Screen process at BHSF. III. Know the appropriate transfusion process and documentation. a. Be able to identify if the new Securline Bar Code Blood Band used at Baptist Health. b. Learn how to use Securline Bar Code Blood Band

Process: Collecting Type & Screen Specimen Review Physician Initiates Order to Type & Screen patient. Phlebotomist RN / CP Collects Blood Sample for the Type & Screen. Phlebotomist RN / CP Verifies Identifiers and places Blood Band on patient.

What You Need to Know When a Type & Screen Is Ordered

1. Select the Correct Vacutainer V Adult & Pediatric Newborn acutainers Lavende er 6.0 ml EDTA VACUTAINER MICROTAINER EDTA MICROTAINER Know the size & color tube for your patient.

2. Obtain Labels for Specimens Labels 1 Net Access RNs and CPs may use Net Access labels for Type and Screen Specimens. 2 Horizon Phlebotomists may use the Horizon label for Type and Screen specimens.

3. Match Label to ID Band Identifiers 1 2 3 ASK MATCH VERIFY Blood Transfusion Services requires 3 Patient Identifiers: 1. Name 2. Date of birth 3. Medical record number RN, CP and phlebotomist follow all required patient identification steps to verify the (2) pre printed labels match the patient s identification armband. Blood Administrationis is high risk for patients. Please refer to entity guidelines for the administration of high risk therapies. When pre printed labels are not available (downtime or trauma), handwrite the patient identifiers.

4. Place Label on Specimen Collection specimen 1 Place label vertically. 2 Write date and time. 3 Initial the label. Use 1 st pre printed patient label and place vertically on vacutainer and apply the control number to the vacutainer. Write the date and time of specimen collection. Wit Write your initials. iti (i.e. person collecting specimen). RN, CP or phlebotomist will label specimen infront of patient. 4 Verify patient data on label. Verify name, date of birth, medical record number, blood band control number, date and time of collection and initials of the person collecting specimen.

5. Place Label on Blood Band B lood Band 1 Place label on armband. 2 Write date and time. 3 Initial the label. Place 2nd pre printed patient label in the center of armband. Write the date and time of specimen collection. Write your initials (i.e. person collecting specimen). RN, CP or phlebotomist will label armband in front of patient. 4 Verify patient data on label. Verify name, date of birth, medical record number, blood band control number, date and time of collection and initials of the person collecting specimen.

6. Place Blood Band on Patient Changes All Patients will now wear a Securline Bar Code Blood Band as part of the Type and Screen process at Baptist Health.

S 1 afety Why is the New Blood Band Important to Patients? Pti t? 1 Provides Positive Patient Identification: Increases accuracy by streamlining both the patient identification and blood transfusion workflow process. Uses pre printed printed bar code in an alphanumeric format for use with existing LIS or blood banking systems. 2 Improves Care & Safety: Waterproof armband minimizes a transfusion delay due to repeat Type and Screens caused by unreadable armbands. Minimizes time consuming paperwork and maximizes time at the bedside. Instantly and accurately captures and verifies data for transfusion verification and specimen collection.

7. Request for Blood/ Component Pickup for BHM and BCH R 1 Blood request will be made using the equest pink pickup slip shown on the right at all times. 2 To fill out the form, the patient s nurse must place a Bar Coded Blood Band control number sticker or handwrite the 4 numbers and 3 letters (NNNNLLL) on the Request for Blood/ Component Pickup form in the presence of the patient. This form is used at BHM and BCH *This process is subject to change in the future pending Information Technology updates.

8. Request for Blood/ Component Pickup for DH; HH; MH; SMH; WKBH R 1 Print fromnetaccess bloodrequest for equest blood/components pick up slip form. 2 To fill out the form, the patient s nurse must place a Bar Coded Blood Band control number sticker or handwrite the 4 numbers and 3 letters (NNNNLLL) on the Request for Blood/ Component Pickup form in the presence of the patient.

9. Complete Transfusion Slip Check Prior to administering blood product, two RN s must verify all steps of the Pre Transfusion Check. RN will complete lt Transfusion Slip once the transfusion is started. RN completes

10. Document on Blood Transfusion Record Record Two RN s/lmp will follow clinical guidelines for the administration of blood/ blood components for their entity. (Clinical procedures are outside the scope of this training). Please see entity / unit policy(s) for step by step clinical resources and protocols regarding the administration of Blood Products. RN must know the signs and symptoms to watch for and appropriate clinical steps. Reactions to blood or blood products can happen during the course of any transfusion. TheBlood Transfusion Record is used to document the entire blood administration process, including patient education, vital signs and noted reactions to transfusion.

11. Complete Post Transfusion Documentation C hart Upon completion of transfusion therapy, RN completes post blood administration process, including completion of Transfusion Slip. Transfusion Slip chart copy The completed top copy of the Transfusion Slip is to be attached to the Blood Transfusion Record. Attach here Completed Blood Transfusion Record must be inserted in the laboratory section in the patient s chart.
