Transition Planning Survey Rationale The monitoring priorities and indicators of the OSEP State Performance Plan (SPP) mandate that youth with IEPs aged 16 and above have an IEP that includes appropriate measurable postsecondary goals that are annually updated and based upon an age, appropriate transition assessment, transition services, including courses of study that will reasonably enable the student to meet those postsecondary goals, and annual IEP goals related to the student s transition service needs. 8/25/2017 2
Transition Planning Survey Rationale There also must be evidence that the student was invited to the IEP Team meeting where transition services are to be discussed and evidence that, if appropriate, a representative of any participating agency was invited to the IEP Team meeting with the prior consent of the parent or student who has reached the age of majority. Replacing Students Students are randomly selected from the data reported in FTE 1 in October. Because students may withdraw from your school or exit special education (no longer eligible or parental revocation) by November 13, the beginning of the Transition Planning Survey: The application allows for a replacement student to be selected. You may not replace a student just because his/her transition plan is non-compliant. You will be asked to provide the withdrawal date or exit date. 8/25/2017 8

Transition Planning Survey Process Verification of Compliance DOE reviews each of the transition plans that were submitted for compliance. District submission due date: December 1, 2017 (Timely and Accurate Reporting, Indicator 20) Verification will begin on December 6, 2017 District determination of compliance will be made after this review December 22, 2017: notification will be sent through the portal to districts that were not 100% compliant 8/25/2017 15
Calculating Compliance Determination All plans compliant 100% AA-A size district 5 plans: 4 compliant = 80%, 3 compliant = 60%... B size district 4 plans: 3 compliant = 75%, 2 compliant = 50%... C size district 3 plans: 2 compliant = 67%, 1 compliant = 33% D & E size district 2 plans: 1 compliant 50%, 0 compliant 0% Transition planning is important!
