Completing the Online Health Assessment You must complete the requirements for the Building Better Health Credit 1 in order for your spouse 2 to receive the biweekly payroll credit for you. If you complete your online Health Assessment by Dec. 5, 2014 3 or within 15 days of enrollment for new hires enrolling outside annual enrollment your spouse (the associate) will receive the spouse credit. You will also need to complete the Hidden Health Risk Screening step by Feb. 6, 2015 for your spouse to continue receiving the credit for you, and earn the Bronze point level or higher in the Home Depot Health Challenge in summer 2015 to continue receiving your credit. 4 1 Available to associates and spouses/domestic partners enrolled in a Home Depot medical plan, except for the Bronze High Deductible Health Plan (HDHP). Once you complete the Health Assessment, your credit will be started as soon as administratively possible. 2 Or domestic partner 3 Spouses newly enrolled in Aetna coverage during annual enrollment will not be able to take the Health Assessment until January and therefore will not need to complete this step until Jan. 31, 2015. Spouses newly enrolled in coverage with the other carriers will still need to complete it by Dec. 5. 4 If you enroll in the medical plan after Jan.1, 2015, you will not need to complete the Hidden Health Risk Screening step. If you enroll on or after June 1, 2015, you also will not need to complete the Health Challenge step. Getting Started! Once your spouse has enrolled and elected that you will complete the Health Assessment on the Your Benefits Resources Website, go to Aetna.com and take one of the following paths to complete your health assessment to receive the credit. The following pages provide instructions for completing your 2015 health assessment: 1. Aetna and Non-Aetna Enrollees: Spouses Who HAVE Completed a Health Assessment Before. 2. Aetna & Non-Aetna Enrollees: Spouses Who HAVE NOT Completed a Health Assessment Before. Your Health Assessment Results Are for You Alone! Please note that the results of your Health Assessment are completely confidential. The Company will not have access to your results and your results will have no effect on your spouse/domestic partner s employment or your medical coverage. 1
Aetna and Non-Aetna Enrollees: Spouses/Domestic Partners Who HAVE Completed a Health Assessment Before Step 1 First, you will need to go to Aetna.com. Click Log-in to begin the process. Step 2 Click on Take a Health Assessment. Step 3 Then select either Update Current or Create New to get started. Now you are ready to complete your Simple Steps Health Assessment! Once you complete the Health Assessment, you will receive an overall health score. Be sure to scroll down to the bottom of the screen to see how your results have changed over time. Aetna and Non-Aetna Enrollees: Spouses Who HAVE Completed a Health Assessment Before To confirm completion of the Health Assessment, click on the My Completion Certificate tab. Click on My Action Plan to see the actions you can take to improve your health and take advantage of the online healthy living programs. 2
Aetna and Non-Aetna Enrollees: Spouses/Domestic Partners Who HAVE NOT Completed a Health Assessment Before Step 1 First, you will need to register on Aetna.com. Click Register Now to begin the process. Step 2 On the personal information page, enter either the member ID number or the associate s Social Security number. If you do not want to enter the associate s Social Security number, you can call Aetna member services at 1-800-695-9744 to get the Aetna member ID number. Then enter your name, date of birth and Zip Code. Enter your Member ID Number Or enter your Social Security Number Step 3 If you see the Dependent s Information screen to the right, enter the associate s information as the subscriber. The subscriber s member ID number can be completed using the member ID you entered in the prior screen or the associate s Social Security number. If you see the Create a Secure Log In screen, go to Step 4. Enter the subscriber s Member ID Number Or enter the subscriber s Social Security Number 3
Step 4 Next, create a user name and password, and complete the security question section. Please keep your user name and password you will need them again to log back into this website. NOTE: If you get a message indicating that you already have a user name and password, select Reset User Name and Password and create a new user name and password. Step 5 Complete the Preferences, Terms and Conditions and Your Profile and click Save. Review your profile changes and click Submit at the bottom of the page. Then click Continue. Step 6 Then click Take a Health Assessment. Step 7 Enter your preferred phone number and click on Full-Time Associate or Spouse, review your email address and click Submit. Aetna & Non-Aetna Enrollees: Spouses Who HAVE NOT Completed a Health Assessment Before Now you are ready to complete your Simple Steps Health Assessment! 4
Step 8 Once you complete the Health Assessment, you will receive an overall health score. Be sure to scroll down to the bottom of the screen to see how your results have changed over time. To confirm completion of the Health Assessment, click on the My Completion Certificate tab. You can also click on My Action Plan to see the actions you can take to improve your health and take advantage of the online healthy living programs.
