This RFQ includes the following: Submittals - Attachment I Scope of Work - Attachment 11 Selection Criteria - Attachment Ill Location Maps - Attachment IV Request for qualifications should be presented in accordance with the specified requirements in Attachment I. Supplementary material such as exhibits, biographical information, and other documentation may also be submitted separately bound from the request for qualifications. Six copies of the request for qualifications must be submitted to the Director no later than Friday October 28, 2016, 5:00 pm CST. A non-mandatory pre-rfq meeting will be held on Tuesday October 18, 201 6, 10:00 a.m. at City of Laredo Utilities Department Conference Room located at 5816 Daugherty Avenue, Laredo, TX 78041. Any questions relating to the RFQ shall be submitted via email to Riazul I. Mia, P.E., C.F.M. on or before Friday October 21, 2016. All addendum and response to questions from consultants will be posted in the City's website. Request for qualifications will be evaluated by Department staff in accordance with the selection criteria specified in Attachment Ill. Once the consultant has been selected, the contract will be negotiated. It is expected that the selected consultant should be ready to proceed on or before January 2017. ATTACHMENT I PREQUEST FOR QUALIFICATIONS SUBMITTALS A. Request for qualifications Format The request for qualifications is required to contain the following information and should be no more than twenty pages (excluding cover, title sheets, dividers, etc.) SECTION 1 Approach Provide an approach for completing the Scope of Work for (Type of Services). (See Attachment II). Present project review and quality assurance program. Discuss any unique ideas/concerns relating to the project. SECTION 2 Project Organization Lead firm (or firms in the case of joint ventures) - Names(s) - Address (es) - Telephone number(s) - Name of responsible corporate officer(s) Sub consultants - Name(s) - Address (es) - Telephone number(s) - Name of responsible corporate officer(s) Organization Chart Brief description of responsibilities and summary resumes of key professional personnel emphasizing experience directly relating to responsibilities on this project, as well as any specialty certification which demonstrates special qualification related to this project. Indicate which team members will participate in each area of the work. SECTION 3 Schedule The respondent firm shall be capable of completing all applications and submitted to TCEQ by March 01, 2017. SECTION 6 Experience and References Principal-in-charge Brief description of not more than five (5) projects similar to the work to be undertaken within the last five (5) years performed by the Principalin-charge. Project Engineer Brief description of not more than five (5) projects similar to the work to be undertaken within the last five (5) years performed by the proposed project engineer(s). ATTACHMENT II SCOPE OF WORK The scope of services shall include the preparation of wastewater treatment plant permit renewal applications for SLWWTP, NLWWTP and PWWTP and submittal to TCEQ no later than March 01, 2017. The following services shall deem to be the minimal for the work: Engineer is expected to meet with the City and TCEQ state office regarding the permit renewal. Assemble all necessary documentation and submittals, and complete forms for the permit renewal applications. The permit renewal must be submitted to the TCEQ no later than March 1, 2017 (180 calendar days before the current permit expiration date of September 1, 2017). The City will provide information available as required by Engineer to complete and prepare the forms as required for permit renewals. Conduct site visit to the plants as needed. Provide Draft of the Permit Renewal Packages in PDF format to the City for review. Finalize permit applications for submittal to TCEQ. Follow up with TCEQ until permit renewal is approved. Deliverable: One (1) original, one (1) electronic (pdf), and six (6) printed copies of the application materials for the WWTP s. The TCEQ requires submission of one original application and three (3) printed copies. Engineer will coordinate with the City for the submittal of permit applications to TCEQ. City shall be responsible for the accuracy and completeness of all requirements, programs, instructions, reports, data, and other information furnished by City to Engineer pursuant to this service, and for payment of all required fees that must be submitted with the application.
ATTACHEMENT III SELECTION CRITERIA Selection of the consultant(s) will be based on the request for qualifications. Request for qualifications Evaluation Criteria 1. Specialized Experience of Firm ( 30 Percent) This includes specialized experience directly relating to these projects, demonstrated ability to complete the work within the required schedule and budget, quality control/assurance programs, and the depth of in-house and sub consultant support. 2. Project Personnel ( 40 Percent) This includes the role, commitment, and qualifications (including possession of Applicable licenses and certifications) of key personnel and their experience in the areas assigned. Approach to Work ( 25 Percent ) This includes the approach to the project scope in Attachment II, the understanding of the project scope and schedule of work, and the interfacing of tasks. 4. Other Considerations ( 5 Percent ) This includes comments received from references and unique/creative approaches to work.
