You will remove the blue text and insert your completed information. **The Log must be typed.** Service Learning Experience Log each activity as a separate row. Each labeled row is a mandatory activity. Date Hours Knowledge What knowledge from the course did you apply? How did you learn more about this content? Use words like: Describe, Discuss, Explain, Identify, Recognize, Review Behaviors What actions/behaviors did you observe in stakeholders/staff? How did they explain their role in patient outcomes? What values did they demonstrate/express? Reflection What do these actions/behaviors tell you about how this organization values EBP? What did your learn about the staff s role in EBP? How you will apply it in clinical practice? Required: Consult with reference librarian 1/15/15 1 hr I was able to apply this knowledge to efficiently search for RTC s for my research paper Contemporary Work Environment of Transformational Nursing Leaders & Clinical Nurses 1/23/15 1.5 hr This lecture discussed the culture of magnet hospitals The advancement of EBP has provided better outcomes for patients admitted to these type facilities Decisions in magnet hospitals are weighed heavily by the process of EBP Warming Infants 1/29/15 1 hr How to use this evidence to Their role in patient outcomes It showed that this 1

decrease hypothermia in infants were to implement the interventions to solve the problem of hypothermia organization values the importance of initiating change for the betterment of positive patient outcomes EBP in the Clinical Environment 2/4/15 1.15 hrs This presentation taught me how to incorporate EBP in my work (clinical) environment. The presenter was passionate about how EPB can change how efficiently we can treat our patients if we use this process in standard practice This presentation taught me the importance of advocating for change. In other words, you must prove that you have solid evidence in most cases to initiate those changes Simulated Clinical Experiences as an Equitable Substitution for Traditional Clinical Experiences for Undergrad Nurses EBP in the Clinical Setting 2/11/15 1 This presentation discussed the importance of simulation for BSN students. 2/12/15 1 This lecture discussed the EPB process in the hospital setting The staff who presented to the students were very passionate regarding this topic. Simulation is a big concept at USF CON. It was emphasized how effective EBP enhanced patient outcomes in the clinical environment I learned that simulation is an important foundation for students and enhances our skills in addition to the hands on experience I learned that it takes a process to actually make changes in patient care. EBP is where we start Evidence Based Initiatives in NSICU 2/17/15 1 This lecture focused specifically on neurosurgical EBP measures and outcomes The presenter discussed why patients should ambulate as soon as permitted to prevent muscle atrophy I learned that ambulating ICU patients provided positive outcomes. However, I rarely see this intervention whenever on an ICU 2

Survey Prevalence Skin Assessment 2/18/15 4 Students across all cohorts at TGH were assisting wound care nurses in assessing patients for decubitus ulcers. Data acquired from these assessments were measured TGH against other hospitals for the number of patients with pressure ulcers I will apply the knowledge I learned from these wound care nurses to become better at assessing skin breakdown in my future patients Ventilator Acquired Pneumonia 3/12/15 1 I was fortunate able to witness this in at TGH in clinical Implementing the interventions and protocols in the ICU for patients on ventilators Monitoring for the signs of pneumonia within the first 48 hours after admission Protecting Human Research Participants Your TOTAL hours Log all hours.
