The ToC programme is the new process by which international adaptation nurses can convert their current local nursing registration so that they can practice in the UK. This replaces the current ONP programme. Simply put, International adaptation nurses will undertake the following process; 1. The international nurse will apply to the NMC to undergo the ToC process. Upon assessment of the application, the NMC will accept allow the nurse to undergo a computer based exam (undertaken locally to the international nurse). 2. Upon successful completion of the exam, the nurse will be offered a date where they can attend a practical Test of Competent assessment at the Northampton University. 3. At this point the nurse can be brought to the UK by a sponsoring NHS Trust to prepare for the ToC assessment (a single day assessment). Current rules dictate that the nurse must undertake the first assessment within 12 weeks of arriving in the UK. The nurse will then have up to 3 further attempts to pass the ToC assessment should they fail the first attempt. 3 P a g e
4 ToC Assessment Day Requirements 1 6 stations in total each lasting no more than 15 minutes 4 stations focusing on; Holistic patient centred assessment Implementation Planning Evaluation 2 stations testing clinical practical skills. May include Vital Signs Calculating Drug Doses IM / SC Injections BLS (registration specific) Safe Sharps disposal Meds administration Peak Flows Wound care Urinary catheterisation Hand Hygiene 5 RSCH ToC Process 5.1 ToC Assessment Bookings and arrival in the UK The international nurses will arrive in small cohorts only once a date for the ToC assessment at Northampton University has been confirmed. Northampton have advised that it will be possible to batch book the assessment day. It is estimated that most cohorts will then arrive at the Trust 5-10 weeks prior to the assessment date. 5.2 Toc Assessment Preparation & Training Upon arrival at the Trust, the international nurses will undergo a training programme to prepare them for the Assessment at Northampton University. Currently this will work as follows: 1. The nurses will be allocated to their ward/work area and undergo HCA Induction training. This training will be enhanced with a further 5 days of practical based adaptation nurse training focused to provide the enhanced training to ensure best preparation to facilitate successful outcome of the ToC assessment day 2. When not undergoing training, International nurses awaiting their assessment day will work as HCA s in their allocated area to gain real world experience of the UK healthcare system. 3. Upon successfully passing the assessment day, the international nurses will receive their registration (pin) and be registered to work in the UK as qualified nurses turnaround has been advised at 5 working days. 4. Following recommendations from the Northampton University, the international nurses will undergo an enhanced preceptorship programme to facilitate safe and effective integration into the UK healthcare system based upon the existing ONP training programme which has been proved to be effective. 1 University of Northampton booklet for the NMC s Test of Competence. 6 Resources and Training Materials 6.1 Equipment Northampton University have provided a list of equipment that International nurses may be asked to use during their assessment day. The PD Team have purchased or are sourcing all items as advised by the Northampton University to ensure the international nurses can gain practical experience and become familiar in using all the equipment they encounter during assessment. 6.2 Training Booklet An evidence based training booklet is being provided to each nurse on arrival at the trust to help prepare the nurses for the ToC assessment day as well as prepare them for working at the RSCH The Booklet includes; 1. Milestones Checklist for the ToC training programme 2. Managers Timetable 3. ToC Links and training resources 4. Reading Guidance and recommendations for the ToC Assessment Day 5. Core RSCH Competencies 6. PDR 7 Housekeeping 7.1 Uniforms During the ToC training and preparation period, the nurses will be employed and will work as HCA s and so HCA uniforms are being provided to each nurse by the PD Team upon arrival at the Trust. Staff Nurse Uniforms will then be issued upon confirmation of successful completion of the ToC assessment Day. Each ADAPTATION Nurse will receive 2 tunic tops (theatres ADAPTATIONs will also be provided with 2 trousers). Ordered via SBS ordered through PA Julia McCluskey. Cost code: 497950 (sub) 7.2 HCA Bank ToC adaptation Nurses will be able to undergo Bank shifts upon completion of the HCA induction programme 7.3 Leave Guidance Please note that the manager s timetable has been developed to help managers understand when certain events related to the TOC programme will occur. Unlike the ONP programme, Leave recommendations are not made, individual adaptation nurse managers will manage provision as they would with all their staff. 5 P a g e
