Dear Welcome to Great Ideas in Nutrition! You now have at your disposal a team you can trust and interact with that you will find is positive, supportive and rewarding. You will get the most up to date tools and ideas provided in a supportive environment to help you become the best you physically can be. Your appointment is scheduled at on with Amanda Clark / Kimberley Aalders / Laura Hosking. FORMS TO COMPLETE Enclosed are your registration forms to save you time on the day. Please complete these and bring them with you in your folder. Also included is a food and exercise diary template. Consider recording your food intake and exercise along with any symptoms you re wishing to discuss from now until your appointment. This will provide additional information for your practitioner to use in determining the recommended course of action. Please read the following information prior to your appointment, and sign the client agreement form on page 7. WHERE WE ARE Our office is at Coolangatta, just across from the beach and the Coolangatta Surf Club, in Dutton St, near the corner of Marine Pde. There s a map on the business card on the front of your folder too. 1

WHAT YOU LL FIND ON OUR BUSINESS CARD Often our clients get noticeable results quickly and friends and family show interest in coming on the journey too! Feel free to pick up some extra cards for friends when visiting the clinic. Referrals often get excellent results because they have already seen the positive changes in you. It s also worth noting that people who embark on making lifestyle changes often are more successful and find it easier when they have a Buddy to come on the journey with them. 2

SERVICES Please read the following information and make sure you have been booked into the appropriate service level. If you would like to switch between services, phone us immediately to alter your appointment timing. Complete relevant forms, and bring them with you in this folder to your appointment.. QUALITY SERVICE Optimum quality service requires a full 45 minute initial consultation to - Review your health history - Assess your usual food & exercise habits - Determine your goals - Plan a course of action - Schedule optimal follow up. You ll get reports on body composition, exercise prescription and computer nutritional analysis as deemed appropriate for your needs (see p5). There are NO additional costs for these additional services as they re included in your consultation fee. To assist in your health management, a report is made back to your referring doctor soon after the consultation where appropriate. FEES: Payment of fees are due on the day of consultation. You can choose cash, cheque, Visa, Mastercard or EFTPOS. Please note we do not accept AMEX or Diners. HICAPS is available for those with private health insurance. Online Medicare claiming is available and DVA referrals are accepted. The current Medicare rebate is $52.95 for those on a Team Care Arrangement. Consult your private health fund as rebates differ for each fund. BULK BILLING SERVICE This no frills service provides access to high quality advice in a brief consultation time. Available at no out of pocket expense to those with a chronic condition, who are financially challenged and have a Team Care Arrangement referral and careplan from their GP. Available times and choice of practitioner may be limited. The initial consultation provides a brief, 20 minute appointment and requires the completion of additional forms to collect information prior to the consultation. The consultation will focus on review of your documented history, determination of the first most effective change that you could make to your diet or lifestyle, followed by education and goal setting around this one change. Additional reports or services are not covered within this consultation and may be available at additional charge (see p5 for details of additional service options). FOLLOW UP Clients have varying needs for frequency of follow up attendance. Follow up appointments are of 15 20 minutes duration. The usual levels are: INTENSIVE: Fortnightly visits for 3 months followed by monthly consultations until goal is reached. This approach is common for weight control or investigation of food intolerance. Telephone consultations can form part of an intensive follow up relationship and may be substituted / incorporated into the plan if appropriate for food intolerance investigations. REGULAR: Monthly visits for the first 3 months followed by 6 monthly review. This approach is common for diabetes (without weight control) and other specific problems with limited opportunity for monitoring by the health professional. SPORADIC: For the client who seeks information only, and wishes to return for support as required. All levels are valid and suit different client s needs. FOLLOW UP PACKAGES Most weight loss or food intolerance client s interventions fit into the intensive category above. In recognition of this fact, we offer a package option whereby advance payment provides tangible benefits along with motivation and a clear intervention span. Visits are scheduled fortnightly and content is individualised You can access this package option after your initial visit, once comfortable with your experience and you ve discussed your needs with your practitioner. Ask for full details after your initial visit. Package clients attending for weight control: you will receive the Portion Perfection Pack, including Amanda Clark s Portion Perfection book, a melamine plate and bowl and The Australian Healthy Snack Bible. This kit is valued at $59.95. Package clients undergoing investigation of food intolerance: you will receive a choice of Sue Shepherd s Food Intolerance Management Plan with recipes valued at $35 or a copy of Royal Prince Alfred Hospital s Friendly Foods, valued at $29.95, depending on your planned intervention. Package clients are also welcome to email for support, any time between consultations and also receive up to 15% discount off any other products purchased during their package eg. Cookbooks, Pedometers or Resistance bands. 4

OTHER PRODUCTS AND SERVICES Body Composition Analysis: InBody segmental body composition analysers are used to monitor body fat and muscle levels in each arm, leg and around the middle. This state of the art equipment is used at most consultations where a weight change is being monitored and some change has been achieved. For bulk billed clients, this service is available at an additional non-rebateable fee of $20 per test during a bulk-billed consultation ask reception prior to your consultation. Nutritional Analysis: A detailed nutritional analysis of your intake is completed using Foodworks software which produces a graph of your needs for a range of nutrients against your analysed intake. A simple analysis may be able to be carried out within the course of a standard consultation, however depending on the circumstances we may require you to keep a weighed food record for 3 days to be followed up with an additional review consultation time analyzing the data in readiness for your next consultation. A fee equal to an additional review consultation will apply to this service if provided outside of a follow up package. Exercise Prescription: If you have only been referred for dietary services you may benefit from our exercise prescription services. An exercise physiologist will assess your needs and devise an individualised exercise program to suit your circumstances. This may include a structured resistance training exercise program as deemed appropriate. This service requires a full 45 minute consultation and initial fee and a printed exercise prescription is provided. Package clients may use two review visits to access a full initial exercise assessment with the exercise physiologist. Supermarket Tours: These are run in various locations on the coast. A BIT MORE ABOUT YOUR SUPPORT TEAM Philosophy: Behavior change is difficult. Some people find that the task of improving health is insurmountable. In over 25 years of practice the aim has been to divide the task into manageable steps, addressing one issue at a time. Many people who attend purely on their doctor s request with no intention of changing, leave with a renewed sense of power to change because they possess a clear and achievable goal. It is well accepted that counseling for eating and exercise behavior change enhances outcome. Your relationship with the team should be ongoing, similar and complementary to the ideal relationship with your General Practitioner. There will be times when motivation or intervention is intense and times when it is more sporadic. You may find at times that you feel guilty or uncomfortable about attending follow up due to feelings of failure or circumstances conducive to poor dietary behaviour. It s very important to note from the outset that these are normal feelings and the most effective way to revert to the desired behavior is to discuss the lapse and identify ways to deal with these situations differently in the future. The message here is NEVER to feel uncomfortable to return! As professionals of the industry you can be assured it will never be an uncomfortable experience for you. As lovely as it is to see you when you re doing well, the fact is the best time to see us is when things aren t going so well. Contact with your support team is imperative! Be assured that others before you have encountered most problems, and there are strategies to choose from to help. each other s time. A minimum of 48 hours notice of an inability to attend your appointment is required so that we may offer that time slot to someone else. A system is in place where you will receive a reminder of your appointment around 24 hours ahead of time by phone / email / sms. From time to time issues may arise at short notice preventing your attendance, which affects the clinic schedule. In these circumstances simply phone with as much notice as possible and we may even be able to substitute a telephone consultation (or see if there is another time on that same day to shift to). Occasionally you may be asked if you re available to shift your appointment to allow a smooth running day in light of another s cancellation. If you are unavailable to shift, then your appointment will remain at the allotted time. Please keep track of times made on your appointment card provided. YOUR PRIVACY Your privacy is important and handled professionally. In order to provide you with the highest standard of dietetic care, this practice is required to collect personal information from you. This information covers basic details such as your name, address and telephone number, but it is also necessary to obtain details regarding your general health, past medical and surgical events and social circumstance. There are safeguards to protect this information and all staff are trained to respect your privacy at all times. Information and feedback may be shared with your General Practitioner or Specialist if they are your referral source, or with your permission. CANCELLATION POLICY To help provide comprehensive and cost effective services to our clients please ensure you attend at your appointed time. Every effort is made to run to schedule to respect your time also. Currently a cancellation fee is not enforced as it is preferred to foster a mutual respect for 6
