Incentive Breakdown 1) Early Retention Incentives: New Intakes Week 1 patient rewarded $2.00 metro card incentive for each day of attendance. Metro card distributed by primary addiction counselor. Initial 10 days after the first week (wk1 mentioned above) patients rewarded by point system based on attendance of initial 10 days after the first week (this makes a total of 15 days in treatment). Daily attendance earns 10 points exchanged for $7.00 metro card incentive. In addition, patients must meet this goal to participate in the larger Bi-weekly fishbowl incentive. Orientation Group meets 4 consecutive sessions 1x @ week for completion. Upon completion the Orientation Facilitator will award participants $7.00 metro card incentive. This group is facilitated in both Spanish and English and opened to new admissions. 2) Fishbowl: Bi-Weekly Recognition and Motivational Fishbowl All program patients enrolled and attending longer than 3 weeks participate in this incentive (as mentioned above patients must achieve the goal of 15 days in treatment prior to participating in this fishbowl). Every other Friday this fishbowl will be held. Incentives included in this draw are movie tickets w/popcorn and soda for 2, movie ticket gift cards valued at $20.00, and supermarket gift certificates, McDonald s gift certificates, and Metro cards. Aforementioned incentives vary in amounts. Also, included in the fishbowl are words of encouragement. Chances at the fishbowl are determined by earned points form attending program services and for every 18 points patient earns one draw and 30 points earn two chances at a draw (maximum draws 2 x). Accordingly, patients that have favorable treatment outcome usually earn points bi-weekly for incentives in the range of intensive treatment 36-52 points and moderate treatment 18-26 points. Evening Program Monthly Recognition and Motivational Fishbowl Evening program patients participate in a monthly fishbowl held on the last Thursday of each month. A draw in the fishbowl is determined by earning the maximum points for the month. Patients in the evening program must attend 2x a week and earn 24 points for the month. ** Receive metro $7.00 incentive **Note- patients that attend evening program attend mostly due to employment, training, and other work related activities. Therefore our evening patients are also eligible for work related activities and employment incentives.

3) Behavioral /Motivational Incentives: Addiction counselors are in responsible for the distribution of these incentives to their assigned caseload. Primary counselors are identifying achievements and accomplishments base on specific goals that promote, encourage and reinforce behavioral change as well as fostering motivation. The methodology and rewards are based on patient s recovery process through goal setting and attainment according to DiClemente and Prochaska s Model of Behavior Change, primary counselors observe behavioral and motivational improvements as indicated by patient s stage of change progress within the span of their individualized treatment and discharge plan. Trainings include FEGS/BTW/WEP 2. Educational include literacy, ESOL, Basic Ed, GED and others 3. Care coordination activities medical/mental health/housing/legal/ HRA/ACS/others Follow-up and resolution Completion of (#1) Training Completion of(#2) Education Issues of (#3) Most of referral incentives are $4.00 metro or $5.00 McDonalds gift certificate. Follow-up and resolution incentive is larger movie tickets for 2 w/popcorn and soda, or metro $17.89 a $20.00 value, or supermarket gift certificate. Employment Securing Employment 30 Days Employment Retention 60 Days Employment Retention 90 Days Incentive are $20.00 movie gift certificate, or metro $17.89 a $20.00 value, or supermarket gift certificate. Addiction Counselors keep a binder of their caseload incentive/motivational distributions that are then collected on a quarterly basis.
