The Blue Foundation for a Healthy Florida Sapphire Award Recognizing Community Health Excellence 2010 Nomination Information OVERVIEW The Blue Foundation for a Healthy Florida is the philanthropic affiliate of Blue Cross and Blue Shield of Florida (BCBSF). Created as a 501(c)(3) philanthropic foundation in 2001, The Blue Foundation supports community-based solutions that address the many different health care challenges facing Florida. The mission of The Blue Foundation is to enhance access to quality health-related services for Floridians, with a particular focus on the uninsured and underserved. Purpose of The Sapphire Award The Sapphire Award is designed to recognize and promote excellence in community health programming in Florida. Award winning programs and organizations will have demonstrated impact in improving the health-related outcomes of Florida s at-risk populations and communities. The Award s recognition of effective initiatives is meant to promote greater and more lasting support for improved services and systems that can be sustained over time. Strategies that build on community assets, enhance organizational capacity, foster systemic change, and/or lead to lasting policy changes that improve health will be especially sought after for this Award. Successful nominations will support the mission of The Blue Foundation for a Healthy Florida to enhance access to quality health-related services. Nature of The Sapphire Award and Award Process The Sapphire Award recognizes and awards community health organizations and programs that have demonstrated success and high merit. The Sapphire Award is not a grant program. While there is a financial award, nominations should not be based on the idea of raising funds without regard to recognizing demonstrated success. The Executive Director or President from the nominated organization or program and three persons familiar with the organization or program (e.g., a board member, patients/clients, partner organizations, funders, etc.) are required to nominate the organization or a program of the organization for The Sapphire Award. Up to three Winners will be selected annually, each receiving a financial award toward their defined community health objectives, in an amount not to exceed $120,000 per organization. Other levels of recognition with varying award levels can be identified. In addition, limited promotional support will be made available to help educate the public and various stakeholders about the honored programs. An independent Selection Committee made up of medical, public health and health communication experts from across Florida and the nation will decide Award honorees and funding levels. Award announcements will be made at an awards program in late 2010. 1

GUIDELINES Eligibility Only organizations and programs deemed eligible by The Blue Foundation for a Healthy Florida will be considered. Please review the following information to determine if your organization or program is eligible for nomination: Organizations may be nominated for their health programming in its entirety, or for a single health-focused program. Only one entry from an organization will be accepted during each nomination cycle. Organizations must have a proven record of improving the health of their community through measurable outcomes in programs involved with health-related services. Organizations must be recognized as nonprofit and tax-exempt under section 501(c)(3) of the Internal Revenue Service Code (IRC) and defined as a public charity under 509(a) 1, or 2, or be a unit of government. Organizations must officially be located and incorporated in the state of Florida. Only completed nominations, meeting all eligibility requirements, will be reviewed for consideration. Current and former recipients of grants from The Blue Foundation may be nominated as long as the organization being nominated meets eligibility requirements. Organizations named as Finalist, Honorable Mention or Award of Distinction honorees in past competitions are eligible for nomination in 2009 as long as they meet eligibility requirements. Organizations named as Winners (the highest level of recognition) of The Sapphire Award are not eligible for nomination. Type III Supporting Organizations and private foundations are not eligible. Nominator Eligibility We respect that nominations take time away from other activity so we want to ensure that this Award s goals are clear. We have attempted to identify below who can and cannot serve as a nominator, and thereby help this program realize its goals in effectively and fairly identifying outstanding community health initiatives. Nominations can be initiated by: The organization, as long as the nomination materials from the other three nominators are created by the nominators themselves. Only one nominator can be from the paid staff of the nominated organization. One current member of the board who knows first-hand the work of the organization. A health professional, volunteer or client who knows first-hand the work of the organization. A representative of a partnering organization, such as another nonprofit organization or government agency. A representative of a foundation, corporation or other funding institution. Someone else who has direct knowledge of the work and impact of the organization. The following cannot serve as nominators: More than one paid staff or one current board member of the nominated organization, including development or public relations staff, consultants or 2

professional grant writers. In that spirit, we ask that forms not be ghost written by others, and that the other three nominators affirm their individual completion of the forms. Employees, retirees and board members of The Blue Foundation for a Healthy Florida, Blue Cross and Blue of Florida (BCBSF), or the subsidiaries and affiliates of BCBSF, and their family members. Current members of The Sapphire Award Selection Committee and their family members. Decision-Making Process The Blue Foundation uses a multi-step process to evaluate nominations for The Sapphire Award. Upon receipt of the nominations, Foundation staff confirm that the organizations and programs are eligible for nomination and that the nominations are complete. During the next phase, an external evaluation consultant reviews the nominations against the criteria. In the final phase, the independent Selection Committee meets to identify the organizations and programs to be recognized. Criteria Fits Award Guidelines The nomination is Florida-bounded, a 501(c)(3), has a health focus, serves Florida's uninsured and/or underserved populations, not a fundraising proposal. Demonstrated Success Demonstrated ability to significantly improve access to health care and improve the health-related outcomes of the uninsured and/or underserved. Leadership Extent to which the nominee s leaders mobilize the organization, engage the community, leverage resources and earn trust. Capacity of leaders to innovate and overcome obstacles to improve access to quality care. Sustainability Degree to which the nominee builds on existing community assets and resources, fosters systemic change and/or develops lasting policy changes that can sustain the organization and improve health for uninsured and the underserved. Community Outreach Proven strategies that ensure awareness and availability of organizational resources, and the means for the uninsured/underserved to access such resources. Standards Extent to which organizational activities and programs are professionally recognized and uphold/model established professional standards for excellence. Ways in which program activities and organizational practices are researchbased, serve as a model for others and are replicated by others. Program Evaluation Clear evidence of systematic and continuous examination of organizational activities and goals, and client outcomes. Specific measures and methods used to track, analyze and use data to improve the organization and its outcomes. Culturally Competent Extent to which the nominee embraces and respects the unique strengths and needs of diverse populations. (e.g., organizational programs and policies are 3

affordable; provide spoken and written language assistance; involve client participation in program development and implementation, etc.). Accountability Financial and administrative practices meet professional standards. Independent audits reveal thorough, accurate and honest financial and administrative practices. NOMINATION PROCESS The Blue Foundation for a Healthy Florida has launched online processes for its grantmaking and awards programming. If you cannot complete an online nomination, please contact us at (800) 477-3736, ext. 63215, or thebluefoundation@bcbsfl.com. Nomination Dates The Sapphire Award nomination process opens April 9, 2010, and closes at 5:00 p.m., May 12, 2010. Honorees will be recognized at The Sapphire Award Symposium and Awards Presentation in late 2010. Before You Begin Before you start this nomination process, please carefully read the Overview, Guidelines and Nomination Process for The Sapphire Award. The nominations are competitive, so carefully reading this information may prove extremely useful in crafting a strong nomination. To find information on The Sapphire Award, click on the Help and Guidance link at the top of the page. The online nomination form walks you through the submission process. Before you begin, we suggest the following steps: Check your Internet browser to make sure it is compatible with The Blue Foundation Sapphire Award Online Nomination. The following are compatible: Internet Explorer 7.0 (for PC); Safari 2.0 (for Mac); Mozilla Firefox 2.0. Enable cookies: The Blue Foundation Sapphire Award online nomination process places a cookie on your computer to enable the Save & Finish Later functionality. Please check the user s manual for your Internet browser to enable cookies if you receive an error message. Know your Employer Identification Number (EIN) and/or Tax Identification Number (TAX ID). It is required to submit the nomination. If your organization is new, please contact us at (800) 477-3736, ext. 63215, or thesapphireaward@bcbsfl.com. Review the Mission and Guiding Principles of The Blue Foundation (at www.bluefoundationfl.com) and the Guidelines of The Sapphire Award (earlier in this document or at the Foundation s Web site). Review the Nomination Overview and gather all of your attachments. Download to your computer the Nominator Questionnaire and read the instructions. Full instructions for uploading completed attachments are included in the online nomination form. 4

Nomination Overview To begin the nomination process, determine who will be completing the forms. At this time, only one person per nomination can access the forms. If you applied online for an IMPACT Health Care grant from The Blue Foundation during the Winter 2009 or Summer 2010 grant cycles, please use the same login information. If you have not previously applied for a grant from The Blue Foundation using the online application process, you will be required to create an online account. You will receive an email notification that includes the email address and password required to access your saved nomination and account. The email will also include the Web address of the Account Login page: https://www.grantrequest.com/sid_1085?s=am. All nominations must use this address to access your account and continue any saved nominations, review submitted nominations and/or change email addresses and passwords. If you do not use the above link, you will be prompted to start an entirely new nomination. Once you have created your Account Login, you may proceed with the nomination, save and finish later, or review and submit. You will be able to print a copy of the nomination as you are filling out the form. Click on the Printer Friendly Version link located in the top right hand corner of each nomination page. The nomination can be completed in one online session or over several sessions. Just click the Save & Finish Later button located at the bottom of each nomination page. Once the nomination is completed online, you will click the Review & Submit button at the bottom of the last page of the nomination to submit your nomination electronically on the Internet. Once your nomination has been submitted, a confirmation email will be sent to the email address that you used to create your online nomination account. Overview of information requested The information below gives an overview of the information you will be asked to provide: Eligibility Quiz questions related to the eligibility requirements noted in the Guidelines. Cover Sheet general information on the organization (date, name, address, etc.). Contact Information for key staff. Contact Information for the three nominators. Organization Information includes mission, goals and objectives, population served, programs and activities, history and accomplishments, board of directors, annual operating budget and donors. Must be completed for all nominations. Program Information To be completed if a specific program is being nominated and not the organization in its entirety. Information includes program title, description, population served, history and accomplishments, annual program operating budget, donors. 5

Nomination Questionnaire completed by the organization CEO/Executive Director, and the three other nominators. Completing the Nomination Please do not use your browser s back button at any time. To navigate through your nomination, please use the page numbers listed at the top of each page. Clicking the Save & Finish Later button during the nomination process will save your responses. We suggest you do click Save & Finish Later frequently to avoid losing any of your information. You can save your nomination at any time and return to it later by following the same steps you used to start your nomination. You will need to re-enter the email address and password by revisiting the Sign In page where you created your account. In addition to saving your responses frequently to prevent data loss, we also recommend typing your answers to the nomination questions into a word processing document first. You can then copy and paste your response into the appropriate fields/boxes on the online nomination forms. At any time during the nomination process, you can use the Review My Application button to review your responses, and/or the Printer Friendly Version button to print out your nomination. Attachments The Blue Foundation requires receipt of certain documents before a nomination can be considered. Please download to your computer the forms and documents required. Following are brief descriptions and acceptable file formats: Completed Nominator Questionnaires from the Organization Representative and three non-staff nominators as four separate documents. A copy of the Nomination Questionnaire can be downloaded from The Sapphire Award section of The Blue Foundation Web site (www.bluefoundationfl.com), or by clicking on the Nominator Questionnaire button located on the top of each page of the online nomination document. IRS Tax Status Letter or Government Entity Document as an Adobe PDF document. Organization s operating budget (revenues and expenses) for the current fiscal year as a Microsoft Word document or Excel spreadsheet, or Adobe PDF document. Program s detailed budget (revenues and expenses) for the current fiscal year if you are nominating one program from among many that your organization offers. Attach as a Microsoft Word document or Excel spreadsheet, or Adobe PDF document. Financial Statements. Attach the profit and loss statement and balance sheet for the most recently completed fiscal year as a Microsoft Word document or Excel spreadsheet, or Adobe PDF document. Audited Financial Statements. If your organization s previous fiscal year s financial statements have not been audited, or have not finished being audited at the time of your submission, please attach a copy of your organization s most recent audited financial statements. We recognize that these statements may not exist or apply to certain organizations. If no audited financial statements exist, please attach a letter stating such and the reason(s) why. Please attach as a Microsoft Word document or Excel spreadsheet, or Adobe PDF document. 6

How to attach (upload) a document 1. From the drop-down Title menu below, select the type of document you are attaching/uploading. You should select the specific type of document for each attachment. 2. Click Browse to search through your computer s files to find the document you want to attach, similar to how you would attach a document to an email. 3. Once located/identified, double click on the document you wish to attach; its name will appear in the File Name box. 4. Click Upload to attach the document to your nomination. 5. Repeat steps 1-4 for all other required documents. Upload The maximum size for all attachments combined is 25 MB. Please note that files with certain extensions (such as.exe,.com,.vbs, or.bat ) cannot be uploaded. Sending Us Your Nomination When you have completed the nomination forms and attached the appropriate documents, click SUBMIT to send us your nomination. You will only be able to submit your nomination when you have filled in all the relevant parts of the nomination and uploaded all required attachments. When you click SUBMIT, you will receive an email confirmation that we have received your nomination. This is why it is very important to enter your email address accurately on your nomination. If you do not receive an email confirmation, please contact us right away. Other Materials such as videos, reports and publications should be submitted ONLY at our request. Appointments for presentations or on-site visits will be scheduled ONLY at our request. Because of the large number of nominations we anticipate receiving, we will not have an opportunity to discuss with you at length your nomination ideas before submission. Nominations become the property of The Blue Foundation for a Healthy Florida will NOT be returned. The Blue Foundation for a Healthy Florida reserves the right to share the nominations with Blue Cross and Blue Shield of Florida or other unaffiliated grantmaking institutions. We make no representations or warranties regarding whether or not those organizations will provide funding for or recognition of your nomination.
