Family Guidance Centers (FGC) has been providing substance abuse services on site at St. Bernard Hospital (SBH) since 2002. SBH located in the Englewood Community at 63 rd street off the Dan Ryan. Introduced SBIRT Program to SBH in June, 2014 as a way in which the hospital could provide more of an integrated approach to patient services for the community. Focus on patients currently occupying Med Surg beds. Patients in the ER and the Med Detox units were not part of this program.
FGC employees a FT CADC to perform the SBIRT screens. Each morning the FGC staff person meets with the Chief of Nursing/Nursing Supervisor and MDs and is presented with a listing of patients and room location. A minimum of 5 7 patients per day are screened.

Initial hospital administration and medical staff introduction High rate of positive screens, low rate of individuals attending treatment recommendations. With such a high rate of positive screenings, FGC will be employing a case manager who will assist in a smooth transition from hospital setting to community based substance abuse treatment much like the success we have with the Pilot Program.
In discussion with SBH Administration to expand SBIRT services (adding staff). Assist in the development and implementation of physician and nursing education at SBH.
FGC has been providing substance abuse services at Methodist Hospital of Chicago since March, 2014. On March 17, FGC will begin implementation of SBIRT at Methodist Hospital of Chicag0. The program design will initially be the same as at SBH.
Began serving patients in January, 2014 Implemented at three of the Cook County hospitals identified by HFS as having over utilization of hospital based detoxification services billed to Medicaid. Patients were cycling in and out of hospital detoxification units with few referrals to community based treatment and recovery support services. SMART Act limited the number of hospital based detox episodes to one every 60 days. Needed to co locate community provider staff at hospitals in order to engage patients and provide a warm hand off.
The general objectives of the Screening, Intervention and Engagement Continuum of Care pilot project are designed to reduce barriers to accessing substance abuse treatment, mental health treatment and primary care treatment and enhance the patient s opportunities for long term recovery.
Family Guidance Center CADC staff currently on site at St. Bernard, Thorek and St. Elizabeth s hospitals. 750 unduplicated patients were admitted to detox units since January,2014 at all 3 hospitals: 86% received a Brief Intervention while in detox 79% were assessed for placement into a level of care while in detox 71% of those assessed were successfully engaged and began receiving substance abuse treatment in the community
Identify hospital detox patients who are willing to accept the next appropriate level of substance abuse treatment using Stages of Change Model before discharge. Provide preliminary level of care placement for patients ready to enter community based treatment. Link individuals with co occurring and substance use disorders to mental health programs. Transport willing patients from hospital site to substance abuse treatment programs. Link individuals to recovery support services. Link individuals in substance abuse and recovery support treatment to primary care services Provide medication assisted treatment as appropriate.
