To participate as a skilled nursing facility in the Medicare program or as a nursing facility in the Medicaid program, a provider must be in substantial compliance with each of the requirements established by the Secretary of Health and Human Services found in 42 CFR part 483, Subpart B. Based upon your facility being in substantial compliance, we are recommending to CMS that your facility be recertified for participation in the Medicare and Medicaid program. WING 10/14/2014 COMPLETION K 000 INITIAL COMMENTS K 000 A Life Safety Code Survey was conducted by the Minnesota Department of Public Safety. At the time of this survey Boundry Waters Care Center was found in substantial compliance with the requirements for participation in Medicare/Medicaid at 42 CFR, Subpart 483.70(a), Life Safety from Fire, and the 2000 edition of National Fire Protection Association (NFPA) Standard 101, Life Safety Code (LSC), Chapter 19 Existing Health Care. Boundry Waters Care Center is a 1-story building with no basement. The building was constructed in 1968, with an addition in 2002. Both buildings are of Type II(111) construction, therefore the building was inspected as one building. The building is fully fire sprinkler protected. The facility has a complete fire alarm system with smoke detection in the corridors and spaces open to the corridor, that is monitored for automatic fire department notification. The facility has a licensed capacity of 40 beds and had a census of 33 at the time of the survey. If, upon reinspection, it is found that the deficiency or deficiencies cited herein are not corrected, a fine for each violation not corrected shall be assessed in accordance with a schedule of fines promulgated by rule of the Minnesota Department of Health. The Department of Health shall provide technical assistance regarding implementation of the guidelines. (b) Written compliance with this subdivision must be maintained by the nursing home. This MN Requirement is not met as evidenced by: Based on interview and document review, the facility failed to provide tuberculosis (TB) screening for 4 of 5 newly admitted residents (R41, R20, R47, R54). In addition, results of the two-step tuberculin skin test (TST) were not documented according to facility guidelines for 2 of 5 residents (R20, R47) had documented results of a according to the facility's guidelines. Findings include: R41 was admitted on 3/20/14. The TB symptom screening was completed, but not dated. R20 was admitted on 10/8/13. The TB symptom screening was not completed and the first and second step skin test results were not documented. R47 was admitted on 5/29/14 T. The TB symptom screening was completed, but not dated. The director of nurses (DON), interviewed on 10/16/14, at 2:17 p.m., stated the provided TB control plan directed TB symptom screening and two step TB skin testing. The DON stated that screening was to be completed and dated upon admission and TST results should be documented. The facility policy titled Tuberculosis Control Plan dated reviewed 6/12, identified in Section V. part A. Patients Tuberculosis Program: "TB screening at admission... the TST readings shall be recorded in millimeters of induration." SUGGESTED METHOD OF CORRECTION: The director of nursing (DON) or designee could review and/or revise policies/procedures to ensure newly admitted residents receive timely baseline tuberculosis (TB) screening upon admission including a screening for TB history and symptoms of TB; a two-step tuberculin skin test (TST); and to ensure documented results of the TST's include millimeters of induration and negative results according to the current CDC guidelines.
