These services are provided to primary care services, mental health services for older people, general hospital services and other services within CADS. The role operates within a multidisciplinary team which provides a collaborative model of working. Page 1
KEY TASKS Clinical The Mental Health Nurse provides nursing care that reflects contemporary nursing practice, applying knowledge and skills from nursing theory and research. EXPECTED OUTCOMES Carry a clinical caseload. Maintains a comprehensive understanding of mental illness (including addiction), signs and symptoms, side-effects of psychotropic medications and contemporary treatment models. Develop specific skill-based interventions for working with older persons with Alcohol and Drug disorders, particularly comorbidity with other mental disorders. Incorporate working with older persons into Alcohol & Drug competencies (in conjunction with Lead Clinician and Clinical Advisor). Other tasks as instructed by the manager or delegate. Consultation Liaison Contribute to a clinical consultation liaison service to primary care services, mental health services for older people, general hospital services, NGOs and other services within CADS. Develop and implement specific competencies and training for Alcohol & Drug clinicians working with older people. Training Contribute to an older persons training strategy to CADS and to older people s services. Documentation and Promotional material Contribute to clinical assessment criteria in conjunction with the lead clinician. Contribute to existing clinical pathway documentation to determine appropriate amendments and/or specific older peoples clinical pathway, in conjunction with the lead clinician. Develop elderly specific service promotion material including clinical and training services (in conjunction with Lead Clinician, Clinical Team Leader). Partnerships / Strategic Relationships Participate in partnerships with some key DHB providers of older persons care with a focus on consultation and referral access to CADS. Develop an integral relationship with CADS staff to ensure that CADS progressively works with older persons and their families, including a mobile aspect to service provision. Explore opportunities to share information and/or initiatives with Te Atea Marino and Tupu. Foster partnerships and integral service delivery with services and agencies for older people from a range of ethnic and immigrant groups (e.g. Asian population). Quality Participate in and contribute to organizational reviews of Policy/Procedure, Practice Guidelines and Practice Standards. Participate in and contribute to the development and implementation of unit planning. Participate in and contribute to quality initiatives within the unit and as required by the Quality Team. Medico-Legal Is legally and ethically responsible and accountable for own nursing practice. Practice reflects relevant legislation as well as values and moral principles that promote the interests and rights of consumers. A working knowledge of the Mental Health Act and the Protection of Property and Personal Rights Act will be expected. Practices in accordance with Nursing s body of knowledge and legislation. i.e. Mental Health Act, Privacy Act, and Waitemata Health policies and protocols. Receive Training and Supervision Continue to develop own knowledge base and skills as required to promote personal and professional growth. Participate fully in all requirements of the CADS Performance Management System and competencies. Attend all WDHB orientation requirements and CADS COAT staff training. PERSON SPECIFICATION POSITION TITLE: Community Mental Health Nurse / Minimum Preferred Qualification Registered Nurse Post Graduate study in Mental Health, Addiction and / or Gerontology Experience Recent clinical experience with older peoples mental health experience. A preference for working with elderly clients and an interest in their special problems of health care and rehabilitation.. Recent Old Age Psychiatry inpatient or community experience Training provision. Supervision provision. Skills/Knowledge/Behaviour Knowledge of strategies of working with older people. Knowledge of concepts of risk assessment, management and crisis intervention. Demonstrated skills in liaison. The ability to work independently. Demonstrated flexibility and high level of initiative.
