Cre functins f the rle include training and educatin, prmting rutine enquiry and develping referral pathway t specialist dmestic vilence services @ The Prtal. This service is prvided by CGL s specialist dmestic abuse service delivered in East Sussex as part f The Prtal (http://theprtal.rg.uk/). The Senir Practitiner will be linked t general practices in either Eastburne r Hastings and be based in the specialist service. Training t practices will be cdelivered in partnership with CCG Clinical Leads. A. AIMS OF THE POSITION: T supprt and prvide n-ging training fr health prfessinals in general practice and primary care services n understanding and respnding t dmestic vilence and abuse (DVA). T encurage health prfessinals t ask patients abut their experience f abuse and respnd, recrd, safety check and refer n t specialist supprt. T build and maintaining an effective relatinship with general practice teams. T prvide individual needs-led infrmatin, supprt and advcacy (including sign-psting) fr wmen and men wh disclse past r current experiences f DVA. 1
B. SPECIFIC AREAS OF RESPONSIBILITY: B.1. The Senir Practitiner will: Deliver practice-based capacity develpment wrk: Deliver training t health care prfessinals abut DVA in participating general practices. Prmte awareness f the experiences and needs f thse experiencing DVA, particularly in relatin t their health. Develp a gd relatinship with all general practice staff and wrk effectively as part f the practice team. This includes: prvisin f systematic supprt t the primary care teams f participating general practices, cmmunity based health services including meeting with individuals and small grups f clinical and administrative staff. Wrking clsely with practice champins in each practice. Attending relevant meetings. Develp effective assessment and mnitring prcedures and ensure that clear accurate recrds are kept and maintained. Practively wrk t establish a high rate f referrals frm clinicians and administrative staff in participating general practices. B.2 Advcacy and supprt service Prvide supprt t wmen and men experiencing DVA referred by primary health care clinicians frm participating practices. Prvide supprt t wmen and men experiencing DVA wh self-refer frm participating practices. Prvide direct casewrk supprt, advice, infrmatin and advcacy t wmen and men experiencing DVA thrugh telephne cntact, and/r meetings at the relevant practice. Wrk with wmen and men experiencing DVA t increase their persnal safety (and that f any children) and infrm them f their rights and ptins in terms f husing, legal and welfare rights. Fllw CGL safeguarding prcesses and ESCC child prtectin prcedures and prtectin f adults at risk prcedures at all times. Screen, prvide advice and infrmatin, and signpst patients t specialist case wrkers within The Prtal as apprpriate (men, LGBT and BaME). Keep accurate recrds f all wrk dne with r n behalf f clients and prduce written reprts as requested, including a mnthly reprt f referrals frm participating clinicians/ administrative staff, and the take up f these referrals and case wrk advcacy including utputs and patient utcmes.
