Our Mission To serve the residents and needy students to the best of our abilities, through programmes and activities, with professionalism in promoting social and multi-racial community bonding. 1 P a g e

Chairman s Message 2014 has been a fruitful year for Loving Heart Multi-Service Centre (Jurong) in our efforts to reach out to more residents in Jurong GRC and Yuhua SMC. Project Big Heart, a major initiative for the year was launched in collaboration with Jurong Central GRC and Fullerton Healthcare under the able guidance of our Adviser, Mr Ang Wei Neng, and MP for Jurong GRC in January 2015. This free medical service benefits Pioneer Generation /Community Health Assist Scheme card holders who have chronic illnesses. Our school based programmes have been constantly expanding and improving to reach out to more students who require support and care from us. Pupil Breakfast Scheme (PBS) is expanding assistance to more schools. PBS now covers all Primary and Secondary Schools in Jurong GRC and Yuhua SMC, including 2 Special Needs Schools. This year we have catered for 745 students over 22 schools. We are inspired to do more as we see the growing needs. We intend to grow our free tuition programme to accommodate more needy children. Thus, we are considering the possibility of partnering with other VWOs in Jurong to expand this service. In addition, we also intend to launch the community-based S.T.A.M.P. and S.T.R.I.V.E. in partnership with other Grassroots organisations in Jurong. About Us Loving Heart Multi-Service Centre (Jurong) (LHMSCJ) was officially opened on 4 December 2004 by Mr Lim Boon Heng, Former Minister, Prime Minister s Office (2001-2011), former MP for Jurong GRC. Set up as a one stop social service centre, located at Block 316 Jurong East Street 32 #01-279 and undertakes social projects within Jurong GRC and collaborates with relevant organizations to better serve the community. The on-going services and programmes are: Pupil Breakfast Scheme [PBS] Sports.Tuition.And.Mentoring.Programme [S.TA.M.P] S.T.R.I.V.E 3 year S.T.A.M.P programme Free Traditional Chinese Medicine [TCM] Clinic Free Tuition Service for Needy Students Family Life Circle Project Home Sweet Home Case Management In 1 September 2006, with the initiation of the student youth programme, the Loving Heart Youth Centre was open at Blk 312 Jurong East Street 32 #01-319. In 2012, our organisation name was change from Loving Heart Multi-Service Centre (Jurong Central) to Loving Heart Multi- Service Centre (Jurong). Both centres are currently managed by staff strength of 8. LHMSCJ continues to promote residents bonding for the neighborhood. It is our aim to focus our services yet maintaining the one-stop service centre to serve the community in Jurong GRC. LHMSCJ Management Committee consists of 15 members and the present term of office is from 1 June 2013 to 31 May 2015. 10 P a g e

Health Care Services Loving Heart Cheng Hong Satellite Free TCM Clinic Partnered with Cheng Hong Welfare Service Society, Loving Heart launched a free Traditional Chinese Medicine (TCM) Clinic on 4 January 2010 to provide free consultation and medication for all patients. The clinic underpinned by the growing demand for TCM was equipped with quality medicines and served by two TCM practitioners, Mr Ngoh and Ms Wong along with a pharmacist, Mr Lee. It was well known among those patients who were seeking remedies and effective acupuncture treatments in TCM. TCM Outstanding Volunteers All donations received during the sessions go to Cheng Hong. Health Care Services Project Big Heart This was a combine effort between Jurong Central, Fullerton Healthcare and Loving Heart Multi-Service Centre (Jurong). This project was initiated to assist needy individuals, holding either CHAS or Pioneer Generation (PG) cards. Individuals who were diagnosed with chronic diseases would be given free consultation and medicine every 3 months. A small fee was imposed to those who were not under the CHAS and PG schemes. The flat fee was inclusive of consultation and medication 17000 information packs were distributed to all the residents of Jurong Central to notify them about this outreach programme. A total of 199 residents came forward for the registration. Consultation Rooms were made available at: Blk 316, Loving Heart MSC (Jurong), #01-279 Blk 312, Loving Heart Activity Centre, 01-319 Patients getting their medication Session 1 and 2 199 and 170 patients received treatment and complimentary medication from Project Big Heart from the first 2 sessions respectively. Most of the patients received up to 3 months worth of medicine sponsored by Fullerton Healthcare. These medication provided to the CHAS/PG cards members had decreased a substantial amount of medical fees for many of the lower income residents. Pupil Breakfast Scheme School Based Programmes Sterling support to our mission Loving Heart Multi-Service Centre (Jurong) started Pupil Breakfast Scheme (PBS) in 2006, to provide free breakfast for Primary and Secondary School students within Jurong GRC. The current number of schools was 22 schools, with 745 student beneficiaries enrolled in the programme. The total amount disbursed to the schools for PBS 2015 was $197,000. PBS complements the assistance provided by the Ministry of Education, Financial Assistance Scheme (MOE-FAS) and the Straits Times Pocket Money. Started in 2006 with only 6 schools, the programme had since expanded to cover all Primary and Secondary Schools in Jurong GRC and Yuhua SMC, including 2 Special Needs Schools. NTUC FairPrice Foundation Limited donated $100,000 in support for PBS, while South West Community Development Council (SWCDC) supported with a 1:1 matching grant. Free Tuition School Based Programmes Sterling support to our mission Loving Heart Multi-Service Centre (Jurong) Free Tuition was established in 2005. This programme was established to aid the less-privileged children who reside within Jurong GRC, with their studies. Children from Primary 1 to Secondary 2 gather at Loving Heart Activity Centre at Blk 312 Jurong East Street 32 every Saturday for tuition. The tutors are volunteers, ranging from students, working adults and retirees who take time off every Saturday morning to tutor the students. Heritage Tour During school holidays, a series of enrichment classes are organised for the students. Schools and corporations engage activities from time to time. Many of the activities are sponsored by the corporations. On 29 November 2014, Loving Heart brought the students for a heritage tour to the fire station as well as the Stamp museum. At the fire station, they understand and visualize the daily routine of the fire man. S.T.A.M.P & S.T.R.I.V.E School Based Programmes Sterling support to our mission Loving Heart s Sports, Tuition and Mentoring Programme (S.T.A.M.P) has been serving children and youth since 13 February 2006. S.T.A.M.P was established to reach out to needy students identified in their respective Primary and Secondary school environments. It aims to foster healthy and positive holistic development in the areas of physical and socio-emotional well-being from exposure to sports, tuition and group mentoring activities. S.T.R.I.V.E was eventually initiated in March 2011 to serve as a 3-year continued enhancement to S.T.A.M.P. It embodies the following objectives: The extended period promotes sustainable support and growth to students. It has also enabled Loving Heart to monitor the progress of the student on effectiveness of the programme on a long-term basis. Both S.T.A.M.P & S.T.R.I.V.E comprise of a 3-component package of sports, tuition and group mentoring sessions. These sessions are held weekly within school premises throughout the year, which amounts to about 20 sessions for each component. The sessions expose students to new skills or interests through healthy and engaging activities. We currently offer cycling, photography, soccer and in-line skating in the various schools we serve. This has been made possible with services provided by Shimano (Singapore), Kaveman, Kid s Soccer and Jeremy Wong In-Line. In-Line Skating Train the trainers @ Shimano Tuition Tuition sessions for S.T.A.M.P consist of weekly math tuition conducted by professional and experienced tutors who provide additional academic support to students and reinforce learning that has been taught in school. S.T.R.I.V.E tuition incorporates an Enhanced Learning Programme formulated by Boys Town which promotes skill-based learning to help students study more effectively. School Based Programmes Sterling support to our mission Mentoring Group mentoring sessions aim to impart positive morals and values through fun games and activities. It promotes healthy socio-emotional development for students. Volunteer mentors undergo a training course certified by the Workforce Development Authority (WDA) which is conducted by professionals from Boys Town. Mentor training equips volunteers with the necessary skills and understanding in working with youths in order to conduct sessions effectively. Loving Heart served a total of eight schools. Rulang Primary, Yuhua Primary, Fuhua Secondary and Hong Kah Secondary are under S.T.A.M.P while Shuqun Primary, Shuqun Secondary, Jurong Secondary and Jurongville Secondary are under S.T.R.I.V.E. It aims to encourage our S.T.A.M.P./S.T.R.I.V.E students to improve and remain motivated.the teacher-in-charge of each school nominates a maximum of 3 students who went through a year of the programme. Awardees are selected based on their academic results and behavioural conduct. Cash prizes, vouchers and a Certificate of Achievement award winners. Thereafter, winning and merit entries from each category would be selected based on technical and creative aspects of the photograph. Awardees receive cash prizes and a Certificate of Achievement. Tentative Plans Loving Heart is looking at expanding its S.T.A.M.P Programme to cater to the Jurong community. The Community S.T.A.M.P initiative was proposed during our recent Task Force Committee meeting, which was set up to review our existing programmes and determine the future direction of Loving Heart. Case Management Community Based Programmes Lighting up Hearts! Food Gifts Loving Heart provides assistance to needy residents residing within Jurong GRC. Our assistance encompasses a broad range of services catered to help our clients cope with unfortunate circumstances or difficult situations. Assistance include the provision of food gifts, enrolment in our free tuition programme, house cleaning services or referrals to other welfare organisations. We work closely with organisations such as the Social Service Offices, Lakeside Family Services, Southwest CDC and ComCare to assist our clients with financial assistance, counselling or any relevant therapy that will improve their well-being. On average, we manage about 7 to 8 new cases every month. These cases may be referred by external organisations mentioned above as well as Jurong Polyclinic and Jurong Central Meetthe People sessions. We also cater to walk-in clients seeking assistance. Loving Heart supplies about 40 food gift packs to regular clients every month. Our food gift assistance has been increasing over the years and the projected number of food gifts for 2015 could very well exceed the average of 480. Loving Heart receives regular donations of food items from various organisations such as NTUC, Bo Tien Welfare Services Society etc. Individuals and other welfare and community organisations also bestow their love and food gifts to Loving Heart. The Staff Union of NTUC-ARU donated about 70 packs worth of food items to Loving Heart on 19 March 2015 as part of their community service efforts. Adopt-A-Home Community Based Programmes Lighting up Hearts! Lunch Outing for the Elderly at Jurongville Secondary School 21 October 2014 Student Performance; Singing and Dancing Jurongville Secondary School students made difference from their hearts in every possible way, when the 25 elderly residents from Bukit Batok Home for the Aged were invited to their school. Programmes were lined up. The residents clapped and sang along with the students who presented songs and dances. The highlight of their performance was the Piano concert. Residents revealed heartfelt contentment and rejoiced throughout. Students then ushered the residents to the canteen for lunch and had a lively conversation with them. Annual Commemoration and Appreciation Civic Appreciation Dinner 27 June 2014 Every year, to appreciate and thanked the contribution and effort by the Civic Groups, Loving Heart will organised and host the Annual Civic Appreciation Dinner. All the Civic Groups Organiser had been invited to express our heartfelt appreciation for support towards Loving Heart. This year, the dinner was organised at Jurong Country Club, Fu Lin Men Restaurant. The event was graced by our Guest-of-Honour Mr Desmond Lee Ti-Seng; Minster of State for National Development and Mr Ang Wei Neng; Members of Parliament for Jurong GRC. 84 guest turnout on the day. It was to appreciate the efforts and commitment given by the principals of various schools and the unwavering support and contribution given by the volunteers respectively to all our programmes and activities. We had invited our Advisers, Mr Desmond Lee Ti- Seng; Minster of State for National Development, MP for Jurong GRC and Mr Ang Wei Neng; MP Jurong for GRC to grace the event. Group Dialogue session A chit-chat session was held between the Advisers and various Schools Principals and Volunteers for more feedbacks so that Loving Heart could make improvement for the activities organised. These feedbacks had aided in activities/events during the planning process. Connect, Coordinate and Engage! Makino outing for Elderly - Visit to Sentosa Sea Aquarium 11 October 2014 Makino organised an outing for 29 residents from Blue Cross Thong Kheng Home with Loving Heart as the coordinator, accompanied by 9 caregivers and 42 Makino staff and volunteers. A sweater and a pair of socks were given to each of the resident during the outing at the Aquarium. For many of them, it was a new and memorable experience visiting the largest Aquarium in the world. After the visit, the beneficiaries were treated with a sumptuous vegetarian lunch at Grace Court Cuisine Pte. Ltd. Warming up before cycling Cycling for Health, Happiness and Harmony by Shimano 25 October 2014 Shimano, who is our partner in the S.T.A.M.P/S.T.R.I.V.E cycling programme, teaches students on bicycle maintenance and cycling etiquette. Loving Heart Gala Charity Dinner 2015 14 March 2015 Loving Heart s Annual Fundraising Event was held at Jurong SAFRA Joy Garden Ballroom with Mr Tharman Shanmugaratnam, Deputy Prime Minister, and Minister for Finance and Special Adviser to Loving Heart MSC (Jurong) as Guest-of-Honour for the night. Pledge Card 2015 Fund Raising Events To give a brighter tomorrow! A total of 10566 cards were given out. The event has successfully raised $17, 3271.30. Half of the funds raised were channeled back to the school that has participated. Sixteen schools participated in our 2015 annual Pledge Card Fund Raising project. Pledge cards were distributed to students of different educational levels. NPS International School was gracious to donate all the money collected amounting to $22,905 to Loving Heart. Programmes with our Partner Schools Sharpening the vision Singapore Polytechnic Eye check-ups for elderly Jurong Central Group Elderly learning simple exercise Loving Heart with Jurong Central Active Ageing Committee, Zone D and Jurong Spring Active Ageing Committee brought 78 elderly residents for eye check-ups on 3 separate sessions, conducted on Thursdays, 24 July, 31 July and 14 August 2014. The eye check-ups were conducted by final year students from Optometry course in Singapore Polytechnic. In addition, students from Nutrition, Health & Wellness Faculty conducted light stretching exercises and dietary talks. Free prescription spectacles were given for those who required them.
