The Institute for Urban Indigenous Health (IUIH) was established to provide a coordinated and integrated approach to the planning, development and delivery of primary health care services to Aboriginal and Torres Strait Islander populations within the South East Qld Region. In doing this, you will: Provide health screening for clients and families including but not limited to: o Routine clinic observations, comprehensive health screening as the first stage of an annual Preventive Health Checks and scheduled screening. Provide education for clients and families on the roles of various providers involved in their care, reasons for referral and importance of follow up care Work collaboratively and holistically within a multidisciplinary team environment to engage clients and families in the management of their wellbeing from a social, cultural, physical, and emotional perspective To be successful in this Aboriginal Health Worker role, you will have: Minimum Cert III in Aboriginal and Torres Strait Islander Primary Health Care, and/or Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Practice), or willingness to work towards completion of qualifications Previous experience working with an electronic Patient Management System e.g. MMEX (electronic medical appointment system) Previous experience within an Aboriginal Community Controlled Health Service preferred. IUIH established the Moreton Aboriginal and Torres Strait Islander Community Health Service (MATSICHS) in 2011 as a strategic response to the significant growth and geographic dispersion of the Aboriginal and Torres Strait Islander peoples in the Moreton Bay area. The clinics provide culturally appropriate, comprehensive and holistic primary health care services to clients. In addition, in line with our whole of life philosophy and comprehensive holistic services, clients can access to allied health and health specialists services; family wellbeing services (including counselling and family support) and aged care services (including in home support, respite and transport). Role Purpose The primary function of this position is to provide high quality services including assessment, examination, treatment and case management, along with contributing to the development of models of holistic health incorporating continuity of care. The Aboriginal Health Workers work within a clinical team environment with the aim of improving the health status of Aboriginal and Torres Strait Islander people through the provision of comprehensive and culturally valid primary health care services.

Role Responsibilities Provision of screening, assessment and follow up services Provide health screening for clients and families including but not limited to: routine clinic observations; comprehensive health screening as the first stage of an annual Preventive Health Check (item 715) and scheduled screening prompted by the MMEx to do list, within scope of practice Receive and act on 81300/10950 EPC Referrals to provide specified follow-up services for clients and families who have completed comprehensive preventive health assessments (Item 715) and / or GPM Ps / TCAs Provide hearing screening on referral for children aged 5 years and over; for adults on referral, working closely with the GP and visiting audiologist to engage clients whose results require further assessment and/or treatment. In line with the regionally endorsed Smoking Cessation Protocol, provide comprehensive assessment, brief intervention advice, and intensive follow up and support for clients wanting to quit Provide assessments of lung health on referral, including performance of PEFR and / or spirometry Undertake venesection and point-of-care testing as requested by the GP and in accordance with own competencies and scope of practice Actively maintain knowledge of the application of the MBS relevant to your job role; ensure all EPC follow-up visits are compliant with the referral and that recording and reporting of follow-up services meets the criteria for MBS claiming Where barriers exist for clients and families in attending the clinic, provide services in the home or community in keeping with scope of practice, and in accordance with the IUIH Home Visiting Policy guidelines Education and promotion of health literacy Provide education for clients and families on the roles of various providers involved in their care, reasons for referral and importance of follow up care Support clients and families with information, resources and advice in response to specific requests and / or identified need Working closely with the Community Liaison Officer, identify opportunities for individual, family and community education to build knowledge about wellbeing including healthy life choices, risk factors, chronic conditions, and the breadth and availability of health care and related services Collaborate Work collaboratively and holistically within a multidisciplinary team environment to engage clients and families in the management of their wellbeing from a social, cultural, physical, and emotional perspective Provide support and guidance to non-aboriginal/torres Strait Islander Colleagues on cultural ways/societal factors which might influence or require consideration when working with clients and families Service development and improvement Actively participate in regular clinic meetings, case conferencing continuous quality improvement meetings and activities in the clinic Identify opportunities for contributing to regional service development and quality improvement, through regional Aboriginal and Torres Strait Islander Health Worker forums, Lead Clinician Group meetings, and other forums Drawing on direct input and feedback from clients, families and community, along with information drawn from the electronic Patient Information Management System, contribute to the understanding of family and community needs from a social, cultural, physical and emotional perspective

Demonstrate leadership in IUIH s Work Health Safety goals and objectives remaining complaint with WHS Legislation and IUIH s policies and procedures. Carry out your duties safely in accordance with your ability and competence. Legal Obligation: All relevant health professionals who, in the course of their duties, formulate a reasonable suspicion that a child or young person has been abused or neglected in their home/community environment, have a legislative and a duty of care obligation to immediately report such concerns to the Department of Child Safety
