Chapter 4 Employee First Aid, Medical and Emergency Procedures Chapter 4 - Employee First Aid, Medical and Emergency Procedures Non-Occupational Illness or Injury Diagnosis and treatment of non-occupational illness or injury are limited to minor first aid, emergencies, and special situations for which treatment is in the best interest of Hayward Electric and the patient. Return to Work Restrictions may be placed on an employee's work pursuant to physical examination, occupational or non-occupational illness, or injury. Pregnancy & Workplace Hazards As soon as an employee learns that she is pregnant, she must inform the Responsible Safety Officer of the expected date of childbirth. Safety Glasses The safety glasses are supplied free of charge to each new employee. Employees are expected to have these safety glasses at all times while on the job. In the event that the glasses are broken or not usable, they will be turned in to the Company and a new pair issued. Gloves The gloves are supplied free of charge to each employee for hand protection. Employees are expected to use gloves when performing tasks where hand injury hands are possible (for example, threading conduit, feeding cable, etc.). New gloves will be issued as necessary. Job Startup Procedures When the Job Supervisor arrives on the job site for the first time, he should check to see that the following exists on the job site: 1. A copy of this Injury and Illness Prevention Program. 2. That a properly stocked first aid kit is on site. 4-1

Hayward Electric Injury & Illness Prevention Program 3. That the appropriate Material Safety and Data Sheets (MSDS) are on site. 4. That emergency notification information to include a medical provider has been identified and posted. 5. That a fire extinguisher is on site. The office should be contacted to obtain any of the following if they are not available. First Aid Procedure Minor First Aid Treatment All job related injuries and illnesses, no matter how minor, must be reported to the employee's immediate supervisor. Where minor first aid treatment is required and adequate first aid supplies are available at the jobsite, office, shop, etc., seeking outside medical facility help is not mandatory. First Aid Kits Approved first aid kits must be present and adequately supplied at every Hayward Electric main office, shop, remote offices, and in most of our company on road type vehicles at all times. They are available at the Hayward Electric main office or by contacting the company Purchasing Agent. First Aid kits should be checked before being put into use and then weekly thereafter by the jobsite supervisor. On all remote job sites where outside emergency medical assistance is further than (20) minutes away (i.e., walk-in clinic, hospital, doctor's office, etc.), the company shall have at least one approved first aid kit, and endeavor to have at least one first aid qualified employee on the site, if possible. All Hayward Electric employees are urged to participate in certified CPR and/or basic first aid training. All employees are eligible for reimbursement of 100% of the cost of such training. Emergency Procedure (Injury) The general procedure to follow in the case of an injury that is serious enough to require professional medical attention is as follows: Immediately attend to the injured's life threatening conditions. Call 911, or send someone to call 911 if a phone is not available for any reason. Give your exact location, and the specific details of the emergency. Have the person read the information back to you before you hang-up. If a phone is not available, use a company radio or any other appropriate means to request assistance. Make sure that you provide your exact location, type of emergency, and what you need. 4-4

Chapter 4 Employee First Aid, Medical and Emergency Procedures Be certain that the Hayward Electric Safety and Health Manager is notified of the emergency at the earliest opportunity. Secure the accident scene until released by the Safety and Health Manager or Hayward Electric Company senior management (President, General Manager or Controller). Medical Attention If there is a need to send a Hayward Electric employee to a medical facility for a job related injury or illness other than an emergency, use the medical care provider identified for the job site. If a medical care provider is not identified, contact the Hayward Electric Human Resources Department for direction. An injured employee must never be allowed to drive or otherwise transport himself or herself to any off-site area or facility. The injured person must be driven by a Hayward Electric employee in good physical condition and with a valid driver's license to the nearest approved medical care provider. Under California law, the employer has the right to choose the authorized medical provider in all cases except emergencies. Every employee intending to use outside medical attention should make every effort to contact the Hayward Electric Human Resources Department for pre-authorization and direction. Toilets A minimum of one separate toilet facility shall be provided for each 20 employees or fraction thereof of each sex. Such facilities may include both toilets and urinals provided that the number of toilets shall not be less than one half of the minimum required number of facilities. EXCEPTION: Where there are less than 5 employees, separate toilet facilities for each sex are not required provided the toilet facilities can be locked from the inside and contain at least one toilet. Under temporary field conditions, not less than one toilet shall be available. 4-5

Hayward Electric Injury & Illness Prevention Program Where the provision of water closets is not feasible due to the absence of a sanitary sewer or the lack of an adequate water supply, non-water carriage disposal facilities shall be provided. Unless prohibited by applicable local regulations, these facilities may include privies (where their use will not contaminate either surface or underground waters), chemical toilets, recirculating toilets, or combustion toilets. Toilet facilities shall be kept clean, maintained in good working order, designed and maintained in a manner which will assure privacy and provided with an adequate supply of toilet paper. The requirements of this section shall not apply to mobile crews having readily available transportation to nearby toilet facilities. Washing Facilities Washing facilities shall be provided as follows: A minimum of one washing station shall be provided for each twenty employees or fraction thereof. Washing stations provided to comply with this requirement shall at all times: A. Be maintained in a clean and sanitary condition; B. Have an adequate supply of water for effective washing; C. Have a readily available supply of soap or other suitable cleansing agent; D. Have a readily available supply of single-use towels or a warm-air blower; E. Be located and arranged so that any time a toilet us used, the user can readily wash; and F. When provided in association with a non-water carriage toilet facility a. Provide a sign or equivalent method of notice indicating that the water is intended for washing ; and b. Be located outside of the toilet facility and not attached to it. 4-6

Chapter 4 Employee First Aid, Medical and Emergency Procedures Exception to F: Where there are less than 5 employees, and only one toilet facility is provided, the required washing facility may be located inside of the toilet facility. Exception to Washing Facilities: Washing Facilities need not be provided to mobile crews having readily available transportation to a nearby toilet and washing facility. Purpose Bloodborne Pathogens This company is committed to providing a Safe and Healthful work environment. In pursuit of this endeavor, the following Exposure Control Plan (ECP) is provided to eliminate or minimize occupational exposures to bloodborne pathogens. Scope The base of this Plan is to meet the re-accruement of the OSHA Bloodborne Pathogens Standard, Title 29 Code of Federal Regulations 1910.1030. It will provide protection for employees through the use of "Universal Precautions." Universal Precautions assume that all blood and body fluids are infectious for bloodborne pathogens, and must be treated accordingly. The Safety Coordinator will also be responsible for reviewing and updating the ECP annually or sooner if necessary. Its purpose is to reflect any new or modified tasks and procedures which affect occupational exposure, and to reflect new or revised employee posters with occupational exposure. Employee Exposure Determination Occupational exposure to blood and body fluids is limited to our designated first aid responders. The exposure determination shall be made without regards to the use of personal protective equipment. Our facility has decided to offer hepatitis B pre-exposure vaccination to the first aid provider. In the event of a first aid incident of blood or other potentially infectious materials are present, the first aid responder(s) is (are) instructed to report to the Safety Coordinator before the end of his (their) work shift(s). 4-7

Hayward Electric Injury & Illness Prevention Program The Safety Coordinator will maintain a report, which describes name of the first aider, as well as the date, time and description of the incident. He/she will ensure that any first aider that desires the vaccine series after a all incident will receive it as soon as possible, but not later than twenty-four hours after the incident. The Safety Coordinator will train first aid providers on the specifics of the reporting procedures and to all others training associated with Bloodborne Pathogen requirements. Engineering Controls and Work Practices Engineering controls and work practice controls will be used to prevent or minimize exposure. Hand washing facilities are available at all jobsites. Employees will wash after administering first aid all equipment will be decontaminated or discarded in appropriate containers. Personal Protective Equipment First aid responders will use Personal Protective Equipment appropriate for administering the first aid required All jobsite first aid kits contain gloves, eye protection, resuscitation bags and mouthpieces. Labeling Biohazard warning labels will be placed on all containers for wastes, which may be contaminated with blood or body fluids, and red bags will be used as required. Housekeeping If a first aid incident occurs, the first aid responders will take precautions to decontaminate work surfaces, tools and equipment. Personal protective equipment will be used during cleanup. Mechanical means such as tongs, forceps or a brush and a dust pan will be used to pick up contaminated broken glassware. The waste will be treated as regulated waste and disposed of in closable and labeled or color coded containers. When storing, handling, transporting or shipping, place other regulated waste in containers that are constructed to prevent leakage. The waste will be discarded according to federal, state, and local regulations. In the event of a first aid incident in which the first aid responder s clothes become contaminated, the following actions will be taken: 4-8

Chapter 4 Employee First Aid, Medical and Emergency Procedures Contaminated laundry will be handled as little as possible and with a minimum of agitation. Appropriate Personal protective equipment will be worn when handling contaminated laundry. Contaminated laundry will be placed in color coded bags at its location of use, and taken by a commercial launderer. The launderer will be given the appropriate warnings. Training All designated first aid responders will receive training conducted by the Safety Coordinator. The training program will cover, at a minimum, the following elements: A copy and explanation of the standard. Epidemiology and symptoms of bloodborne pathogens. Modes of transmission. The Exposure Control Plan and a way to obtain a copy. Methods to recognize exposure tasks and other activities that may involve exposure to blood. Use and limitations of Engineering Controls, Safe Work Practices, and PPE. PPE types, use, location, removal, handling, decontamination, disposal and basis for selection. Hepatitis B Vaccine offered free of charge. Training will be given prior to vaccination on its safety, effectiveness, benefits, and method of administration. Emergency procedures for blood and other potentially infectious materials. Exposure incident procedures post exposure evaluation and follow-up signs and labels. Post Exposure Evaluation and Follow-up If an exposure incident occurs, contact the Safety Coordinator immediately. A confidential medical evaluation and follow-up will be conducted by a Medical Review Officer (MRO). The following will be performed: 4-9

Hayward Electric Injury & Illness Prevention Program Document the routes of exposure and how exposure occurred. Identify and document source individual, unless infeasible or prohibited by State or local law. Obtain consent and test source individual's blood, document the source's blood test results. If the source individual a known to be infected, testing need not be repeated. Provide the exposed employee with the source individual's test results, and if information about applicable disclosure laws and regulations concerning the source identity and infectious status. After obtaining consent, collect exposed employee's blood as soon as feasible after the exposure incident and test blood for HBV and HIV serological status. If the employee does not give consent for HIV serological testing during the collection of blood for baseline testing, preserve the baseline blood sample for at least 90 days. Post Exposure Evaluation The circumstances of exposure incidents will be reviewed to determine if procedures, protocols and/or training need to be revised. Health Care Professionals Health care professionals responsible for employee's HB vaccination, post exposure evaluation and follow-up will be given a copy of the OSHA Bloodborne Standard. The health care professional evaluating an employee after an exposure incident will also receive the following: A description of the employee's job duties relevant to the exposure incident. Route(s) of exposure. Circumstances of exposure. 4-10

Chapter 4 Employee First Aid, Medical and Emergency Procedures If possible, results of the individual's blood test. Relevant employee medical records, including vaccination status. Health Care Professional's Written Opinion The designated Health Care Professional will provide the employee with a copy of the evaluating health care professional's written opinion within 15 days after completion of the evaluation. The written opinion for post exposure evaluation and follow-up will be limited to whether or not the employee has been informed of the results of the medical evaluation and any medical conditions which may require further evaluation and treatment For H B vaccinations. The opinion will be limited to whether the employee required or received the vaccine. All other diagnoses must remain confidential and not be included in the written report. Recordkeeping Medical Records: Medical records are maintained for each employee with exposure in accordance with 29FCR-1910,20. In addition to the requirements of 29 CFR 1910. 20, the medical record will include: The name and social security number of employee A copy of the employee's Hepatitis B vaccinations and any medical records relative to the employee's ability to receive vaccination. A copy of all results of examinations, medical testing, and follow-up procedures as required. A copy of all Health Care Professional written opinion(s) as required by the standard. Employee medical records will be kept confidential and will not be disclosed or reported without the employee's express written consent except as required by the standard or by law. Employee medical records shall be maintained for at least the duration of employment plus 30 years. Employee medical records shall be provided (within 15 working days) upon written request of the employee or to anyone having written consent of the employee. 4-11

Hayward Electric Injury & Illness Prevention Program Training Records: Bloodborne pathogen training records will be maintained by the Safety Coordinator at a designated location Transfer of Records: If our company ceases to do business and them is not a successive employer, the employer shall notify the Director of the National Institute for Occupational Safety and Health (IN IOSH) at least 3 months prior to scheduled records disposal, and prepare to transmit them to the Director.
