Radiation Safety Audit Checklist Date., Contact and No 1. Management and supervision Outline the management structure for radiation safety in your school/section Guidance 1 It is recommended that a lever arch file be used for the day to day management of radiation safety in schools. This file would contain a number of sections covering different aspects of radiation processes, for example, wipe testing, protocols, records of receipts, waste disposal, monitor calibration sheets. It would also contain relevant working rules and process instructions. There should be a clear picture of the management structure and it should look something like the diagram below, please fill in the names in the places shown. School Manager.. Where is the radiation records file kept? Are your radiation registration records up to date? Are your license records up to date? Are they consistent with registration details? Are the relevant research group leaders and other personnel licensed? Are your risk assessments (Protocol) details up to date? Are they consistent with license details? 2. Supervision, training, instruction and working rules Do you have systems that ensure occupants of the laboratory are suitably supervised, trained, instructed and that radiation working rules are on display? Is there a system for induction of new personnel for radiation safety? Are there adequate systems for radiation safety training? Are appropriate radiation safety working rules available? Are records for radiation safety inductions and training up to date? Where are they located? Guidance 2 The induction process provides a beginning point for a personal record. It must include records of the points below. It provides a permanent record that the manager has managed their personnel, with regard to safety in an appropriate manner. It records Supervision, Instruction, Training and that Working procedures have been read and understood. An example of some points to be included in the radiation induction checklist is attached. Personnel must be aware of their line management. Supervisors must induct personnel into the laboratory, team and University. Instruction on radiation safety and any attendant hazards with materials, equipment or processes must be provided. Radiation working rules must be on display. For specific tasks, with significant risk, specific rules must be drawn up and be readily available. Records of training are required as part of your risk assessment programme. Details of radiation training course attendance are kept by the Safety and Health Office if you need to confirm details. Radioactive Materials 1. Records of radioactive materials holding - receipt and disposal Is there a written work instruction available for the recording system? Are records of receipts and disposal up to date - what was the last entry date? Are materials booked in when received? Are materials booked out as they are used? Are radionuclides used consistent with Protocol, license and registration details? Are there unknown or unmarked packages in the fridge or waste storage area? 2. Laboratory design and services Do the radiation areas conform to the appropriate Acts, Regulations, Standards and Codes? Designated radiation areas and labelling Are specific areas for radiation procedures designated and suitably identified? Is there labelling on relevant entry doors? Is there labelling on refrigerators and storage location? Is there labelling on relevant equipment? Is there labelling on containers? 4. Radiation monitoring equipment Is appropriate monitoring equipment available? Is there a written work instruction available for monitor calibration? Are the records up to date? Do all the monitors have a current calibration? The following information will be available on the calibration certificate: Manufacturer, Model, S/N, Type, Location, Calibration due, Radionuclides that the monitor is suitable for. 5. Radiation surveys Are radiation surveys carried out (monthly for partially enclosed XRA units, 3 monthly for fully enclosed XRA units)? Are radiation survey records up to date? 6. Personal monitoring procedures Are personal monitoring procedures in place and adhered to? Is there a written work instruction available for the personal monitoring system? Where are the records kept? Is the record system up to date - what was the last entry date? 7. Wipe testing Is there a written work instruction available for the wipe testing? Are wipe testing records system up to date - what was the last entry date? What is the frequency of wiping (recommended: 1 month for high level labs (iodination), 2 months for general radiation labs, 3 months for counting labs)? Diagram showing wipe locations? Radionuclides checked? Who does the wipe testing? 8. Equipment and shielding Do you have systems to ensure adequate and sufficient shielding and equipment is provided? Lasers 1. Records of laser holdings and designated laser areas Are the Class 3B and 4 lasers registered? Are the laser locations in accord with the registration? 2. Laboratory design Do the lasers conform to the appropriate Acts, Regulations, Standards and Codes? Shutter interlocks? Warning light interlocks? Windows covered? Warning lights (power supply, entrance, area)? Beam stops? Other equipment? 5. Designated radiation areas and labelling Are specific areas for transilluminators designated and suitably identified? Is there labelling on relevant entry doors? Is the warning sign/warning rules displayed nearby? Is there labelling on relevant equipment? 4. Shielding, equipment, interlocks Is there a UV shield on the equipment and is it in good order? Is there a face shield? Goggles? Interlocks? Other equipment? 5. Emergency procedures Are there adequate working rules in the area for emergencies? Do personnel understand their role in an emergency?
