2 Overview Today will briefly cover: The purpose of the screen How is the screen used Screen requirements Screen completion CCCW screeners The Role of the Screener Hint: They are not determining the rates The Role of the Provider

3 Purpose of the Screen Determines Functional Eligibility for Long Term Care Programs including Family Care Determines benefit package available to member Nursing Home* or Non-Nursing Home Determines target group(s) *No it doesn t mean they have to live in a nursing home

4 Other uses of the screen Determines CCCW s capitation payment (how much we get paid from the state) Uses acuity based review of membership to determine amount Changes every year Data collection CCCW specific determine provider rates Acuity based

5 Screen Requirements Screen completed at least once every 365 days in accordance with screen instructions The Department of Health and Family Services (DHS) oversees completion of the LTCFS for Family Care Maintain and interpret screen instructions Instructions are black and white but many people are not Must look at each individual person s needs Hard to generalize across all people due to individual differences Instructions have a lot of information but won t find everything spelled out in them Instructions change periodically

6 Screen completion Once information is gathered via face to face screen interview/assessment, screener enters information in state website which automatically determines eligibility Screen is protected by HIPAA Need a ROI specifically for the LTCFS We even need a ROI when a member enrolls from another program Printed screen is not part of contract exchange of information since it is from state website and not our record.

7 CCCW Screeners Screen must be completed by a Certified Long Term Care Screener Contract with DHS has specific qualifications in order to be certified as a screener Pass an online course and additional on the job training Continuing Skills Test every 2 years to maintain certification CCCW has dedicated screeners to complete screens. Screen Specialists are located in: Centuria, Rice Lake, Spooner, Ashland, Superior, and coming soon Ladysmith and Park Falls in the Northwest (Emily Connor supervises Northwest) Wausau, Marshfield, Wisconsin Rapids, and Stevens Point in Central area

8 Role of the Screener IS NOT TO DETERMINE RATES PAID TO PROVIDERS They will NOT do NOT know about your pay They will NOT do NOT know if it will stay They will NOT do NOT know anymore today They will NOT do NOT know about the rates okay

9 Role of Screener when meeting with Provider Obtain needed information on member s abilities in order to complete the screen in accordance with state requirements to determine functional eligibility for the Family Care Program Answer screen related questions about the person specifically being screened May not be able to be too specific since may need to consult with instructions or screen lead depending on situation

10 Role of Provider Provide an accurate report of member s abilities Have someone present at the screen who assists member with day to day care needs that can answer the screener s questions Can still have others at screen but most important to have someone who knows member s abilities It is considered Medicaid fraud to falsify information on the LTCFS

11 Provider Communication Tell the screener if he needs help to feed Tell the screener if he makes himself bleed Tell the screener if he has any need Tell the screener if there are warnings to heed Tell the screener if he needs you to lead Tell the screener if on the floor he has peed **Important to provide as much detail as possible at time of screen for an accurate screen.**

12 Change in Member Needs Always talk to a member s IDT staff when there are any changes in a member s condition or needs IDT will review screen (hopefully with the provider) and submit a screen discrepancy report if there are any discrepancies noted/questioned Rescreen is not automatically completed just because a provider requests it. There needs to be things not captured on the current screen or that have changed and warrant a rescreen.

13 Role of the screener and Determining rates NOTHING.see earlier slide They complete the screen and the Community Resources department applies the residential rate tool.

14 Final note about Screener Role Please at the time of a screen being completed do NOT ask a screener questions about the rate. Again this is not their area of knowledge

15 They do not know if you might get a new rate They do not know your rates fate They do not know if a new rate is on slate They do not know how long you may have to wait So please with them do not get irate.
