COVENANT HEALTH STRUCTURE & LEADERSHIP Governance Covenant Health brings together the rich history of sponsorship of eight communities of Catholic Sisters who founded the organizations and sites we currently operate. The Sisters transferred these operations to the sponsorship of the Catholic Bishops of Alberta over a span of 25 years beginning in 1976. The Bishops established Covenant with the consolidation of 12 different entities through a private bill of the Alberta legislature. Currently a Board of Directors is appointed by, and accountable to, the Catholic Bishops of Alberta. The Board of Directors has full governance authority for the entire organization and has appointed Community Boards in each community. These Community Boards are an important component in the governance of Covenant Health by assisting the Board of Directors in the areas of developing the vision, input in strategic planning, community engagement, site monitoring and physician liaison. The Bishops, the Board and the Community Boards are governed by Article of Association, Bylaws and Terms of Reference. These documents define the relationships, accountabilities and responsibilities of each group. Administration President and CEO Patrick Dumelie leads the Senior Leadership Team responsible for advancing the organization's strategic directions and working within the integrated provincial health system to lead operations, address operational issues, and respond to provincial priorities and opportunities. Operations are divided into three portfolios. 1. Edmonton Acute Care includes programs and services at our two large urban hospitals. 2. Operations in Seniors' Health include large continuing care programs in Edmonton, Lethbridge and St. Albert. 3. Our Rural Health portfolio brings together facilities in our smaller urban centres and rural areas. The portfolios have been designed to meet and address local community needs, look for new areas of growth and respond to Alberta Health Services priorities across the continuum of care in both urban and rural care settings. Facilities Map Covenant Health is part of a larger Covenant family. As part of the Seniors strategy, Covenant Health recognized a growing need for senior s services in Alberta and the Government s need for sustainability and responsive models for serving seniors in their communities. Covenant Care was established as a sister organization in 2013 with a nimble and flexible service model able to provide innovative models of care that met Alberta s need. Covenant Care provides continuing care at 6 facilities in 5 communities, providing a home for 538 seniors with another 310 suites under construction. Together, Covenant Health and Covenant Care have over 15,000 staff, physicians and volunteers working in 24 hospitals and seniors care centres in 15 communities across Alberta with a combined budget over $895 million. The third member of the family, Covenant Living, advances opportunities for seniors housing and other initiatives that are mission-aligned but not part of government funded programs. MEDICAL AFFAIRS AT COVENANT HEALTH Location & Function Medical Affairs is the administrative arm that supports the Portfolio of the Vice-President, Medicine and Chief Medical Officer of Covenant Health. Dr.Owen Heisler, Vice-President Medicine & Chief Medical Officer of Medical Affairs, and Joanne Cowie, Corporate Director, Medical Affairs & IPC are responsible for the strategic direction of the administrative objectives of the portfolio, including budget, human resources, and staff leadership and development. Medical Affairs provides a number of services to support the relationship of Covenant Health to the privileged professional medical staff (physicians, dentists chiropracters and podiatrist) and manage all aspects of healthcare administration. Our Medical Affairs Department provides support services for all of our credentialed professional staff, and provides day-to-day support to our physician leadership team. The Vice-President and Chief Medical Officer is responsible for the Medical Staff role within Covenant Health, and similarly members of the Covenant Health Medical Leadership Structure are responsible for components of this. The rules outline in detail the terms and conditions on which Covenant Health may grant practitioners clinical privileges; the responsibility of the medical staff to Covenant Health for the quality and safety of all professional services provided by practitioners to patients and to Covenant Health; the responsibilities of the medical staff and Covenant Health to each other for the organization and conduct of the medical staff, and in particular, the processes relating to medical staff appointments and delineation of clinical privileges; and the administrative structures for the governance of practitioners working in Covenant Health facilities. In keeping with Covenant Health s focus on creating a respectful workplace, all practitioners are expected to demonstrate respect, accountability, transparency and engagement in their interactions with Covenant Health staff, colleagues and patients. Support is provided by using a variety of acute care coverage providers such as licensed physicians working as clinical associates, international medical graduates working as clinical or surgical assistants, and residents providing service outside of their academic responsibilities working as physician extenders. Code of Conduct Our Covenant Health Medical Staff Code of Conduct and Commitment to Ethical Integrity outlines the values, principles and standards of conduct that guide our actions and interactions. Our Covenant Health s Commitment to Ethical Integrity is our code of conduct it sets the standard and reinforces that as a member of the Covenant Health team, we are each accountable to uphold standards of behaviour and create the ethical culture by which our organization will be widely known. CPSA Code of Conduct The CPSA Code of Conduct clarifies the college s expectations for Alberta s physicians in all stages of their careers, in all facets of medicine, and in all methods of care delivery. Concerns - Professional & Patient Healthcare Professional Concerns Most concerns are effectively dealt with at the first point of contact. If resolution is not achieved please escalate to your medical leader. If the issue still cannot be resolved you can bring your concern to your leader s superior. If the concern continues to persist, the next step would be to submit a written formal concern to your facility chief, site medical director or to the Chief Medical Officer, or designate. Concern From Patient Involving Patient Care All patient concerns regarding care by a physician are routed through Covenant Health Patient Relations and forwarded to Medical Affairs for review. A Triggered Initial Assessment [TIA] Part A as per MSB Section 6.3 (an investigation and initial assessment of a concern) may be initiated. (All information will be kept strictly confidential and will not be disclosed to anyone outside of the process unless agreed to, in writing, by all parties or required by law to ensure public and/or patient safety). A letter and a copy of the concern is sent to the affected practitioner for information. If further investigation is decided, TIA-Part B (Triggered Review) will be started and a letter and a copy of the concern is sent to the affected practitioner to provide written response to reviewer and a copy to the Associate Vice President (AVP) or designate, who then reviews responses and provides written assessment and any further actions needed. For detailed information regarding concerns see Staff Bylaws Concern Process Concerns Guide Curtailment of Privileges for Incomplete Charts Practitioners and Covenant Health share the responsibility to create and maintain an accurate health record of the care provided to every patient in Covenant Health facilities. In accordance with the AHS and Covenant Health Medical Staff Rules (Section 4.21.4), a shared AHS/COV process was initiated in 2012 for the Curtailment of Clinical Privileges for Incomplete Health Records. provided to the Practitioner for completion, or any single Patient health record has remained incomplete for more than 90 days after it has been provided to the Practitioner for completion, the Practitioner is eligible for curtailment of clinical privileges. Physician Wellness Health & Wellness Physicians are important role models for their patients and peers, which makes their own personal health an influential factor in health promotion and disease prevention. Physicians are characterized, in all of their interactions with patients, colleagues and support staff, by values of stewardship, integrity, collaboration, respect, compassion and social justice. The medical profession is demanding. Often physicians work long hours, with many patients and staff depending on them. The best interests of patients are served when physicians take time to meet their own needs and are continually aware of their own wellness. This means recognizing limits imposed by fatigue, stress or illness and taking care to ensure a healthy work-life balance. This is not always easy. Physicians set high expectations for themselves and may not immediately recognize either transient or longer term periods of incapacity. Recognition of transient incapacity is particularly difficult. Covenant Health recognizes the impact a medical career can have on a clinician s lifestyle and family. A Physician Wellness Committee is being created to address the physical, emotional and social needs of these very busy professionals. The purpose of Physician Wellness Committee will be to promote clinician well-being, prevent impairment from any cause and to assist clinicians who are impaired, or at risk of impairment, to return to the full, active, productive and satisfying practice of medicine, with no risk of patient harm. The Covenant Health Physician Wellness Committee will be available to offer confidential assistance when a clinician needs emotional or behavioural evaluation, support, guidance and referral. Physician Absence Notification Process Notifying Medical Affairs As per section 4.16 of the Covenant Health Medical Staff Rules, Absence from a Covenant Health Site(s) of Clinical Activity, a Practitioner shall ensure requirements are met for coverage of all his/her responsibilities, and shall identify an alternative Practitioner to assume those responsibilities and to serve as Most Responsible Practitioner for his/her admitted Patients. The Practitioner providing coverage must have the appropriate Clinical Privileges. The Practitioner will document the identity of the covering Practitioner in the Patient s health record and shall inform the Patient, and/or his/her family and/or the Patient s Legal Representative. Leave of Absence Process Flowmap Key Messages The MS Rules are intended for medical staff to notify stakeholders if they are away from Clinical practice between 96 hrs and 45 days. Ifr you are working at multiple sites the 96 hr - 45 day rule does NOT apply when doing clinical work at another site. As per the MSR if you are taking vacation or on medical leave for greater than 96 hrs - 45 days you would complete the LOA form. 96 Hrs Coverage for patients during an absence of less than or up to 96 hours is provided through the on-call schedule of the service or group in which the practitioner participates, or by alternative prior arrangements. ESSENTIAL EDUCATION FOR MEDICAL STAFF Hand Hygiene Hand hygiene is the act of removing and/or killing microorganisms (germs) on the hands. When performed correctly, hand hygiene is the single most effective way to prevent the spread of communicable diseases and infections. In healthcare settings, hand hygiene is used to remove microorganisms that have been picked up through contact with patients, contaminated equipment or the environment. HEALTH INFORMATION MANAGEMENT Transcription Dictation Services Medical Transcription services provides timely and accurate transcription/editing of clinical reports and distribution, with the main focus being on acute care dictation. The Dictation, Speech Recognition, Transcription (DST) platform is province wide at both Covenant Health and Alberta Health Services sites. The DST system allows for standardized tools and processes for both providers and medical transcriptionists, which provides better turn -around times and improved patient care. In 2014/15 all Covenant sites transitioned to the Dragon Medical 360 escription (Nuance) medical transcription platform, a speech recognition solution designed to fully integrate with the EHR and streamline clinical documentation. INFORMATION & PRIVACY Covenant Health Confidentiality Policy Confidentiality of information is governed by Covenant Health policy and provincial law. You may have access to personal, health or other confidential information as part of your duties and responsibilities and you are obliged to handle that information in a secure and confidential manner in compliance with Covenant Health policies and the laws. In addition, there are rules and expectations related to accessing Alberta Health Services (AHS) information systems network and AHS/Covenant Health information systems. You may be required to comply with additional terms and conditions before accessing specific information systems. The Misericordia Medical Staff Association (MSA) represents physicians at the Misericordia to the Covenant Health Board of Alberta and administration. The president serves as an ex-officio member of the Board of Directors of Covenant Health of Alberta. The association is also represented at: Facility Medical Advisory Committee (FMAC) meetings Provincial Medial Executive Committee (PMEC) Edmonton Zone Medical Staff Council Association (EZMSA) I am honored to be the President of the Misericordia Medical Staff Association and I take this position of advocacy seriously. This Medical Staff Association connect you with your colleagues and medical leaders in Covenant and AHS. Please utilize this Association to influence matters that affect you and your patients. We are lucky to have leaders such as Bob Black and Owen Heisler and I plan to communicate closely with both of them. It is important that you bring issues to me or to our Quarterly Medical Staff Meetings. Our upcoming Quarterly Staff Meeting will be held on December 13 at 5-6:30pm. expenditures as required. If the Misericordia is your prime site, you are requested to pay these dues as part of your membership in the MCH Medical Staff Association. You are not expected to pay dues for more than one site. Therefore, if the Misericordia is not your principal site, and you are paying dues for the site association at another site within the Edmonton Zone, you are not expected to pay dues at the Misericordia. Only processing of annual reciprocal parking is done through this office. Processing of all other forms of parking will be through the MCH parking office, 8th Floor Cabrini Building, Room 817. ID Badge, Mailbox, Lab Coats, Lockers ID Badge Once privileges have been approved, an ID/access badge is provided by Parking Services Office (780.735.2503) Mailbox Site mailboxes in the physician lounge are granted once dues are paid. Physicians Lounge A lounge for the use of the medical staff is provided on the main floor in Room 1N-108. Basic amenities are provided along with three computers and a printer which are available for your use. Misericordia Medical Staff Association Events Calendar Medical Staff Association Events Edmonton Zone Medical Staff Association (EZMSA) The EZMSA will offer an opportunity for meaningful engagement for practitioners and a strong unified voice. The EZMSA is the formal voice of advocacy for practitioners in the Edmonton Zone. The members include practitioners, medical staff of hospitals, continuing care facilities, PARA, PCN and Non-PCN based community physicians, community-based specialists, dentists and oral surgeons, clinical scientists and podiatrists amongst others. It is paramount that issues concerning practitioners and the patients we serve, particularly those related to access and quality, are heard and receive appropriate attention. It is our goal to have all medical staff in the zone join and participate in the activities of this Medical Staff Association.
