Our current initiatives are highlighted in this edition of Memorial Health Focus. We are thankful to be guided by our mission each and every day. We are blessed with the heritage and leadership that the Sisters of the Little Company of Mary provide. We are honored to participate in the ministry of health care right here in our greater community. Our highest priority every day is the safety of our patients and our staff, and we work every day to prevent and control infections. Our efforts are reflected in this edition s article about infection control that describes some of the many ways we create and protect a safe patient care environment. Our current initiative that is most visible from the street is the construction at 13th and Bartley Streets in Jasper. The new facility will house our Family Medicine Every three years, Memorial Hospital and Health Care Center conducts a Community Health Needs Assessment, or CHNA, to better understand the health and wellness of those we serve. This assessment has been beneficial in addressing and prioritizing health needs in multiple areas of our four-county primary service area. The CHNA may be viewed on the Memorial Hospital and Health Care website www.mhhcc. org or you may contact the Marketing and Public Relations Department at 812-996-2323 to request a copy. 2 Memorial Health Focus Residency Program that we are bringing to our community with the assistance and sponsorship of Indiana University School of Medicine. We know this will provide access to primary care physicians and services for years to come. We have also been working on an initiative to enhance mammography services in our Women s Center by investing in 3D mammography imaging technology. These services are in place, and we have significantly reduced the need for patients to leave the area to benefit from this advanced imaging. Finally, we have installed a new cardiac catheterization lab that demonstrates our commitment to providing excellent, time-sensitive cardiac care for the patients and communities we serve. We know these initiatives are important for today and tomorrow. More importantly, we are empowered by the example of Christ s healing ministry of compassion, and we want to reflect His compassion to each person that enters one of our facilities. Ted earned his bachelor of science degree in economics/ finance with a secondary concentration in accounting from Bentley College in Waltham, MA and his master of business administration from Nichols College in Dudley, MA. He is a Fellow of the American College of Healthcare Executives (ACHE) and the Healthcare Financial Management Association (HFMA). Ted is also a member of the Indiana Hospital Association Council on Finance and Chief Financial Officer Council for Healthcare Leaders Magazine. Ted has over 30 years of financial experience within healthcare systems, with the most recent being Vice President and Chief Financial Officer at Baptist Health Floyd in New Albany, IN. Ted says he is drawn to Catholic health care organizations because In fiscal year 2016-2017, Memorial Hospital and Health Care Center provided many benefit activities to local communities free of charge. Much of this involved education and job shadow opportunities to college and high school students. It also included providing sports physicals at high schools, attending health fairs, providing blood pressure checks at events, and giving educational presentations and classes. And we re sure you ve seen our ambulance and staff at an event or two. As a not-for-profit, Catholic organization, being for others is our mission. it s about service serving each other, serving the hospital, and serving the community. New! Mammograms Now In 3D By Lokender Bommisetty, M.D., Medical Director of the Women s Center Memorial Hospital and Health Care Center is excited to announce that we now offer 3D mammography to all of our patients at the Women s Center! This new technology promises to significantly increase the detection of breast cancer while at the same time reducing the number of unnecessary callbacks. So the question is What is 3D mammography? Tomosynthesis is another term for this technology used interchangeably with 3D mammography. This technology was introduced to the United States in 2009. Prior to that, it was in wide use in Europe. Since its introduction eight years ago, there have been significant improvements in image quality, computer software, and patient comfort. The use of 3D mammography has brought about improvements in breast cancer detection by about 40% according to some studies. Another big improvement that this technology brings to our screening program is a reduction in the number of women who are called back for workups that turn out to be negative. Conventional mammography relies on imaging the breast, which is a three-dimensional structure, by using two-dimensional pictures. This obviously has several limitations. One of the major limitations of 2D mammography is that normal tissue in the breast can sometimes overlap on a 2D picture, and appear like a mass, necessitating the patient having to be called back for additional imaging. A more serious limitation of 2D mammography is that patients who have dense breasts (meaning that there is an increased amount of fibroglandular tissue) can have small cancers masked or hidden by this dense, overlapping tissue. How does 3D imaging work? One way of thinking about this is to imagine a CAT scan of the breast. A CAT scan relies on taking pictures of the body slice by slice so that the radiologist can look at structures in detail without having to worry about overlapping. In a similar fashion, 3D mammography technology takes images of multiple slices of breast tissue. This way, problems caused by overlapping breast tissue can be eliminated. True cancers can be detected often times in their own individual slice without being obscured by overlapping breast tissue. Overlapping breast tissue that creates the illusion of being a mass on conventional 2D mammography can be recognized more easily on 3D mammography as the radiologist sifts through images, and consequently the number of unnecessary callbacks is reduced. What is the patient experience? Well, just like in conventional mammography, the technologist will position the patient s breast in the machine with some degree of compression. The 3D machine has a component that makes a short sweep over the breast lasting for approximately four seconds. In this short amount of time, multiple images of the breast are obtained and sophisticated computer algorithms then process these images into a format suitable for the radiologist to interpret. The new machines that Memorial Hospital has acquired represent the latest in technology, and one of the features of these machines is a better comfort level for the patient, especially those who have had a hard time tolerating compression on the conventional 2D technology. Is there an increased radiation dose with 3D? The analogy with CAT scans raises this question in some patient s minds. The good news is that the answer is no. The radiation dose is comparable to that of conventional 2D mammography, and well below the level set by the FDA as an acceptable radiation dose for a mammogram. MHHCC has obtained a sophisticated computer enhancement to the 3D software that reconstructs the data into images that look like 2D views in order to aid the radiologist in comparing the current study with older mammograms. This reconstruction is done without any additional radiation dose to the patient. The staff and radiologists at the Women s Center are excited to be offering this new patient care technology to our patients. 3D mammography is sweeping across the nation, and we are proud to be acquiring the latest machines with this technology. There s a New CATH LAB in Town The new year welcomed an innovative new development in the cardiac care of patients at Memorial Hospital and Health Care Center. Beginning in January, a new GE cardiac catheterization lab was unveiled, signaling continued dedication to provide excellent By Josh Leonard, M.D. cardiovascular care in Southwestern Indiana. In record time, the GE Innova IGS 530 was installed in a state-of-the-art room in the catheterization suite on Level 6 of Memorial Hospital. Our skilled team of nurses, radiation technologists, and cardiologists from Heartland Cardiology are extremely excited about this new upgrade! The previous GE room was opened in 2004. This type of equipment is integral in the cardiac, peripheral vascular and electrophysiologic procedures routinely performed at Memorial Hospital. The new GE room has significant improvements from the previous models. The new equipment greatly reduces the radiation dose delivered to both the patients and staff by over 50%! Improved visualization during the procedure is obtained with software upgrades such as Stent Viz and Stent Vessel Viz. New automated functions allow for reduced intravenous contrast usage and radiation exposure. This allows our team to not only continue to offer extraordinary cardiac care, but also offer peripheral vascular services rarely found in surrounding centers. Today, the vast majority of our cardiac procedures are performed via access through the wrist, also known as the radial artery approach, which maximizes patient safety and often allows for a quicker return to normal life after the procedure. Our team has been the first in a multi-state region to adopt this approach in a widespread fashion and remains an area leader. The combination of compassionate staff and providers and top-of-the-line technology makes Memorial Hospital and Health Care Center elite in cardiovascular care. As heart disease remains the number-one cause of death in the U.S. - accounting for almost 1 in every 4 deaths - our dedication to combating this disease remains steadfast. New Cath Lab 6 Memorial Health Focus Meet Our NEW PROVIDERS Industrial Medicinist John Dohanich, DO received his doctor of osteopathy degree at Kansas City University of Medicine and Biosciences in Kansas City, Missouri. He completed a family practice internship at Tri-City Hospital in Dallas, Texas, and a family residency at Texas College of Osteopathic Medicine, University of North Texas, in Fort Worth. He is a diplomat of the National Board of Osteopathic Medical Examiners, and board certified by the American Osteopathic Board of Physicians. He is a certified Medical Review Officer and Medical Examiner. Dr. Dohanich is located at Memorial Hospital s IMED clinic. Podiatrist Travis Hubbuch, DPM received his doctor of podiatric medicine degree at Dr. William M. Scholl College of Podiatric Medicine in North Chicago, Illinois. He completed the first year of his podiatry residency at the Cleveland Clinic Foundation in Cleveland, Ohio, and the remaining two years at the Surgical Hospital of Oklahoma in Oklahoma City. Our Infection Prevention Program Hospitals take care of sick and hurting people...that s part of its purpose. Along with illnesses and injuries come germs and bacteria. Keeping hospitals safe for patients, visitors and staff is a huge and very important task. At Memorial Hospital, our Infection Prevention and Control Program strives to do just this. Our Infection Control Practitioners work with The Joint Commission, a group that upholds patient safety and care standards for health care organizations, to identify and reduce the risks of developing infections for our patients, visitors, and health care workers. They research everything from employee health requirements to the best way to clean, sterilize and disinfect. They study how diseases and illnesses spread from one person to another, and how to best treat them. Our Infection Control Practitioners also give input on construction issues to ensure infection prevention steps are maintained throughout the organization as it grows and changes. A few years ago, new information became available on best practices for orthopaedic surgery. Best practices are steps or procedures that are accepted as the correct or most effective way of doing something. With this new information, Memorial Hospital decided to focus on infection prevention for patients undergoing a total joint replacement. The result was a new program - Memorial Orthopaedic Center Beyond the Hand Sanitizer By Sue Roberts-Willis, RN, MSN, CIC, Certified Infection Preventionist/Employee Health Coordinator of Excellence. This program focuses on preventing infections long before the surgery takes place. A preassessment by an internal medicine provider and an orthopaedic nurse navigator reviews the patient s history and lab results to focus on preparing the patient for surgery. Post-procedure exercises are reviewed as well as ways to prepare the home in advance of surgery. Sanitizing wipes are provided to the patient to use before surgery to reduce skin bacteria. The nose is swabbed to determine the presence of organisms, and treatment is recommended prior to surgery. This swabbing also helps in the choice of the appropriate pre-operative antibiotic. During surgery, special dressings and wound devices are utilized to reduce the risk of infections. While the orthopaedic program continues to make changes and evolve as best practices and evidence dictate, the improvements so far have resulted in national medical excellence and patient safety awards. Another area of focus for our Infection Control Practitioners was vascular access (direct or rapid method of accessing the bloodstream). When people become very sick, the quickest route for medicines to get into the body s system is often through the bloodstream. Keeping these devices that access the bloodstream free from infection can often be challenging, especially when these devices need to be left in place for a longer period of time. The Centers for Disease Control recommends that central vascular access devices, devices inserted into veins, be inserted and maintained by a specialized team. Seeing the positive results other organizations have had by creating these specialized teams, Memorial Hospital created a Vascular Access Services (VAS) department in July 2011. VAS is charged with improving all things related to inserting needles into veins to ultimately improve patient safety. The specialists in this department serve as patient advocates by identifying ways to reduce the number of needle sticks a patient may have to endure. They provide education for new nurses, bi-monthly informational updates, and train the hospitalist staff to place midline catheters when VAS are not available. Their expertise in insertion techniques, knowledge, and innovative ideas have contributed greatly to the prevention of infections and have increased patients comfort. Memorial Hospital has not had a central line infection in over a year. Other infections are also monitored, such as Ventilator Associated Events (VAE), as part of infection prevention surveillance. It was discovered that attention to oral care is known to help prevent VAEs for ventilated patients. A group recently revisited our oral care regimen for our ventilated patients to ensure we were using the best combination of products to continue to keep the number of VAEs as low as possible. Infections associated with the use of a Foley catheter are also followed. Infection reductions have occurred by simply critically evaluating the use of a Foley catheter for each patient s needs. Infection Control Practitioners also monitor new technology that comes on the market related to infection prevention. Memorial Hospital recently purchased a mobile Ultraviolet Light (UV) unit to compliment manual cleaning. The UV light utilizes a preprogrammed dose of short-wavelength ultraviolet light to hospital surfaces to help further eliminate pathogens that could live through standard cleaning methods. The light is ran in select rooms after manual cleaning is complete. By supplementing environmental cleaning protocols with the UV light, Memorial Hospital adds an extra layer of cleaning protection for those we serve. Infection reduction activities are revisited frequently to meet regulatory guidelines and evaluate successes, but more importantly, to help reduce health care-acquired infection risks and ensure the patient experience at Memorial Hospital reflects our healing mission of Christ by promoting safety, as we continue to Be for Others. Excitement Brewing for New Physician Training Program By Stanley Tretter, M.D., Vice President of Medical Affairs & Chief Medical Officer ongoing primary care needs. The American Medical Association estimates that up to 70% of physicians ultimately practice within 100 miles of where they did their residency training. This will be a great opportunity to bring new physicians to Southwestern Indiana. Currently, there is only one residency program within Indiana that is located south of the Indianapolis metro area. Memorial Hospital and Health Care Center is excited to partner with Indiana University School of Medicine and our hospital partners to bring graduate medical education to our area, and believes this will certainly enhance the scope and quality of medical services we provide to our communities. Just one more way in which Christ s healing mission of compassion empowers us to be for others through quality and excellence! As construction crews continue to work on a new facility at the corner of 13th and Bartley Streets, it is clear that something big is happening at Memorial Hospital. However, the bricks and mortar building is only the visual representation of a new program in development at Memorial Hospital. The Indiana University School of Medicine-Memorial Hospital Family Medicine Residency Program has been in development for several years, and is starting to take shape before our eyes! The Family Medicine Residency Program is a training program for new physicians who will become boardcertified family physicians. Sponsored by IU School of Medicine and located at Memorial Hospital, this program is working toward accreditation by the Accreditation Council for Graduate Medical Education to accept its first class of resident physicians in July 2019. A family medicine resident physician is a new physician who has graduated from medical school, has chosen to specialize in the practice of family medicine, and is licensed by the state of Indiana to care for patients under the supervision of board-certified physicians. These resident physicians will see their own patients for whom they will provide primary care services in their offices at the newly constructed facility, with supervising physicians overseeing their care. In addition, these resident physicians will broaden their clinical experiences throughout various areas at Memorial Hospital, as well as our medical community, working with a variety of specialists and other primary care physicians in all aspects of medicine. Dr. Rex Stroud, Family Medicine Residency Program Director, will lead the program with assistance from Program Coordinator Tara Schipp, a group of family medicine faculty physicians and specialists from Memorial Hospital, and our hospital partners within Southwestern Indiana. Five resident physicians will be selected to participate in the program each year, which lasts for three years, and are anticipated to begin in July 2019. The new facility is expected to be completed later in 2018. Access to primary care services will be enhanced for patients within our communities, as the resident physicians will be available to offer primary care services as the program begins in July 2019.
