All documents/devices listed above must be reviewed and approved by the monito before the facility can begin transporting children. An application indicating that transportation is being added and the change of license fee for $63.96 must be submitted to Child Care Licensing and Enforcement, 2995 N. Dixie Highway, Ft. Lauderdale, FL 33334.
