Interested in donating an item? Use the following form to fill in the specific fields with details about your donation. Complete a Silent Auction Donation Form for each donation Auction Item Title - Provide a short item title for your auction donation. Item Description - Provide a brief description of your donation item. If you are donating multiples of the same or similar items, please include a separate form for each individual auction item. Name of Individual or Business - Provide the name of the individual and/or business that should be recognized as the donor. Estimated Value - Provide the estimated value of the item. Donation Category - Identify the category in which the donation item should be included. Gifts - Products, services, or experiences. Items may include, but are not limited to, gift baskets, wine or beer, event tickets, books, gift certificates for products or services.
