All applicants seeking to install a wireless communication facility ( WCF ) or a modified WCF must complete this application and submit all documentation requested thereunder. WCFs apply to all facilities for the provision of wireless services including antennas, poles, towers, cables, and wires. b. These requirements are in addition to the Excavation Permit requirements specified in COSH Municipal Code 12.08, which also apply, and must be included with this submittal. c. For all WCF applications, you must submit three (3) copies of this application, the application filing fee and any other documents as may be required by the Signal Hill Municipal Code. d. A Pre-submittal conference with COSH Public Works is strongly encouraged prior to application submittal. The purpose is to receive informal feedback on the proposed scope, location and application. Public works department staff will endeavor to provide applicants with an appointment within approximately five business days after receipt of a written request. e. All applications must be submitted to the city at a pre-scheduled appointment. Applicants may submit one application per appointment but may schedule successive appointments for multiple applications whenever feasible as determined by the city. City staff will endeavor to provide applicants with an appointment within five business days after receipt of a written request. f. Application submittal appointments shall be in person at the following location: i. 2175 Cherry Avenue, Signal Hill, CA 90755 ii. Please call (562) 989-7355 for an appointment. INSTRUCTIONS g. Complete the following application in its entirety. An incomplete application will result in any of the following: (1) denial; (2) delay; (3) a request for supplemental information.

h. All written responses to the questions below must be typed in 12 point font. Several questions require you to attach as exhibits supplemental documentation and commentary to support your answers below. All your exhibits must be numbered to correspond as directed in the application. All supporting documentation must be clear and legible. All exhibits must be stapled or bound to the application. i. Excavation Permit requirements specified in COSH Municipal Code 12.08 must be met in addition to the WCF application requirements of this application and COSH Municipal Code 12.21. i. The Excavation Permit application requests certain information, specifies the related fees for a WCF application and must be submitted with this application along with the required fees. ii. COSH MC 12.21.050.E authorizes the City to retain an independent, qualified consultant to review any application for a permit for a wireless telecommunications facility. The cost of this review shall be paid by the applicant through a deposit. PUBLIC NOTICE REQUIREMENTS j. Following initial acceptance of the application and if determined applicable, prepare public hearing information pursuant to the public noticing requirements of COSH Municipal Code section 20.64.050. This section of the municipal code references Planning Commission noticing. The hearing body for WCF applications is the City Council of Signal Hill, however, the noticing requirements of 20.64.050 apply. k. Applicant will be required to provide proof that all applicable public notices articulated in the City s Municipal Code and the City s noticing policies have been met. l. Provide sample of notification to the property owner(s) near the proposed installation. The public noticing requirements of the City s municipal code listed in paragraph g above shall apply to all wireless installations, that is, all applicants shall give property owners, within 300 feet of the proposed installation, notice of the same. Applicant shall provide the notice via door hanger, flyer or other manner approved by the City. i. Provide the City twenty (20) days advance notice of an upcoming shot clock expiration date to provide the City with a final opportunity to approve or deny the application before it is deemed approved. 01002.0005/279975.1 Page 2 of 8

IV. Description of the project and project purpose Provide a narrative description of the project. Your response shall include, but not be limited to, a description of the proposed facility or modification, the anticipated construction activities involved, the maintenance requirements and schedule for the new or modified facility, and the number of antennas to be installed. Provide any supporting documentation regarding the purpose of the project. Attach responses and documentation marked as Exhibit C. 1. The location and dimensions of the existing facility and maximum height above ground of the facility ii. The benchmarks and data used for elevations iii. The location of existing access ways and the location and design for all proposed access ways iv. The design of the facility, including the specific type of support structure, type, location, size, height, and configuration of applicant s existing and proposed facilities v. If applicable, the method by which an antenna will be attached to the mounting structure should be depicted in the site plans b. Three (3) copies of the Master Plan of all existing and proposed facilities that will be installed in the next two years in the City. c. Provide three (3) sets of photo simulations of the before and after images of the project and project site. d. Provide three (3) copies of any applicable landscape plans. e. Provide all other documentation required by the City s Municipal Code, including mock-up design, installation and notification plan in accordance with COSH MC 12.21.050.A.22. 2. Attach a narrative description and supporting documentation marked as Exhibit E2 relating to the following: a. Identify whether the proposed project is consistent with the General Plan and any specific plans. b. Identify whether the proposed use is conditionally permitted within the zoned area. c. Identify applicable zoning and development codes and requirements and demonstrate whether the proposed project complies with all applicable provisions of the City s zoning and development code. d.
