October 25, 2005 Subject: CALL FOR AWARD NOMINATIONS The National DHIA annual meeting will be held in Phoenix, Arizona on March 9, 2006. The following awards will be presented. National DHIA Outstanding Service Award presented to a person who has for many years been dedicated to the improvement of DHIA and has provided notable leadership to the advancement of DHIA National DHIA H. Victor Joachim Distinguished Leadership Award presented to a DHIA dairy producer and/or spouse who has held a leadership position in DHIA or the dairy industry and rendered outstanding and unselfish service for many years, along with a notable contribution to the advancement of DHIA in the United States Martin A. Wilson Memorial Award presented to a worthy person who has been employed in a DHIA management position for at least five years and has been dedicated to the improvement of DHIA and provided leadership to the advancement of DHIA Everyone is eligible to submit nominees for any award. A set of the materials required to make nominations is enclosed. Please reproduce these materials (or find them at www.dhia.org) and forward to colleagues who may also be interested in submitting nominations. A list of past recipients of these awards is enclosed. Please note that nominations are not automatically carried over from year-to-year. However, if you submitted a nomination in the past and wish to reactivate it for 2005, you can do so by calling the National DHIA office (614-890-3630). This will alleviate duplication of paperwork from past nominations. Please note: All nominations and supporting material (if a new submission) must be in written form and must be received in the National DHIA office by close of business on Friday, December 16, 2005. Fax material is acceptable, but nominations will not be accepted via telephone. Thank you for your prompt response to this request.

NATIONAL DHIA OUTSTANDING SERVICE AWARD Nomination Form The National DHIA Outstanding Service Award may be presented to a person who, in the opinion of the National DHIA Board of Directors, has over many years been dedicated to the improvement of DHIA and has provided notable leadership to the advancement of DHIA. The National DHIA Outstanding Service Award consists of an inscribed plaque and the expenses paid for the recipient and spouse to travel to the National DHIA annual meeting. Victor Joachim, may be made annually to a DHIA dairy producer and/or spouse who has held a leadership position in DHIA or the dairy industry and rendered outstanding and unselfish service for many years, and who the Board of Directors feels has thereby made a notable contribution to the advancement of DHIA in the United States. Please note that past National DHIA directors are not eligible for this award until they have been retired from the board at least two years. The H. Victor Joachim Distinguished Leadership Award consists of an inscribed plaque and the expenses paid for the recipient and spouse to travel to the National DHIA annual meeting. The Martin A. Wilson Memorial Award consists of an inscribed plaque and the expenses paid for the recipient and spouse to travel to the National DHIA annual meeting.
