Our last survey was completed in December 2013 and a three -year accreditation was awarded. CARF accreditation is a process that demonstrates a provider has met standards for the quality of its services. CARF established these standards to guide providers in offering their services. HISTORY Lincoln Self Reliance, Inc. has been in operation as a Section 501 (c) (3) corporation since July 2000. LSR, Inc. maintains physical facilities in Diamondville, Kemmerer, Afton, Thayne, and Evanston Wyoming. The Administration Office is located in Afton Wyoming. The expansion to Uinta County (Evanston) has been a huge success. Several new clients have entered services as a result of Lincoln Self Reliance, Inc. being in the Evanston community. Management staff have been working fast and furious to get all support staff trained on LSR policies and procedures. The aging vehicle fleet that was acquired from the Bridges Habilitation purchase is slowly being replaced. Internet services were added to all office and residential locations to allow for Therap Services to be used at all LSR service locations. A small remodel project was completed on the Evanston office to allow for privacy/hipaa for a newly added Assistant Program Manager. A wheelchair ramp was also installed at the Champs residential facility. The Centers for Medicaid Services and Wyoming Behavioral Health Division have made many changes to the Medicaid Waiver over the past year. As a result persons served are being offered more choices in their daily services. LSR had to discontinue some of the paid work offered because it was not considered community integrated and did not pay minimum wage of $7.25 per hour. LSR has discontinued using the Federal Less Than Minimum Wage Certification as a result of the Waiver changes. LSR s thrift store located at 1038 South Washington is continuing to grow. The local communities have been very supportive of the new thrift store and donations for the store have been very generous. Revenue from the thrift store for the fiscal year 2014/2015 was $62,696. LOVELAND LIVING PLANET AQUARIUM This was an increase of $21,253 from the prior fiscal year 2013/2014. LSR discontinued skilled nursing services during the fiscal year as a result of waiver changes made by the Wyoming Behavioral Health Division. All skilled nursing services are now being provided through a third party Medicaid biller (Home Health Agency). Afton clients at the Craig Thomas Discovery Center in Moose, WY LEADERSHIP LSR, Inc. maintains a Governance Board of five to ten members from various backgrounds and communities. LSR, Inc. employs an Executive Director, Darrel Skinner, MSW, to manage the day-to-day operations of the organization. HUMAN RESOURCE On June 30, 2015, LSR employed ninety-nine (99) staff. LSR also employed eleven (11) persons served on-site. 1 Executive Director 1 Deputy Director 14 Lead Direct Support Professionals 57 Full-Time Direct Support Professionals 2 Persons Served Shredding/Recycling 6 Full time Administrative Assistants 5 Program Managers 10 Part-Time Direct Support Professionals 2 Thrift Store Clerks 2 Vocational Supervisors 9 Persons Served Custodial Duties 1 Full-Time HR Trainer PROGRAMS AND SERVICES On June 30, 2015, LSR was providing services to 61 individuals. Twenty-seven individuals received services through LSR. Forty nine individuals received Day Programming Services through LSR. LSR also provided Supported Living, Personal Care, Supported Employment, Respite, Companion Services, Adaptive Equipment, Support Brokerage and Intervention Services during the fiscal year. LSR OUTCOMES: In a Satisfaction Survey conducted in December 2014, respondents indicated 93% overall satisfaction with Day Hab Services; 100% satisfaction with Residential Services. LSR did not have any formal grievances filed by clients or stakeholders during the fiscal year. Of all individuals served in Adult Services 39% participated in paid or volunteer work. In an employee Satisfaction Survey conducted in December 2014, respondents indicated 88% employment satisfaction working for LSR. Evanston Corn Maze Adventure Evanston Fire Department Tour 3

Quarterly Safety Committee Meetings Community Events Fossil Fest, Mountain Man Days, Lincoln County Fair, Oyster Ridge Festival Persons receiving services and staff network with: WYNOT, Wyoming Independent Living, Parent Information Center, Behavioral Health Division, CARF, Protection and Advocacy, Wyoming Guardianship, Law Enforcement, DVR, Public Health, DFS, Employment Services, OSHA, WIND, ANCOR, Wyoming Community Service Providers Association, Turning Point, etc. Staff participated in: Star Valley Rotary Club, Kemmerer Rotary Club, Evanston Rotary Club, Star Valley Chamber of Commerce, Evanston Chamber of Commerce, Diamondville/Kemmerer Chamber of Commerce, Human Resource Councils, Wyoming Community Service Providers Association, BHD Work Group Committees, CARF Peer Surveying, etc. CELEBRATION OF ACHIEVEMENTS An exciting evening was enjoyed by all who attended the annual awards banquet held at the Thayne Community Center, Kemmerer Events Center, and Evanston Machine Shop. All persons receiving services with LSR received recognition for accomplishments throughout the past year. FINANCIAL A financial audit of LSR records for the period July 1, 2014 to June 30, 2015 was completed by DeCoria & Company, Certified Public Accounts. The audit found no significant financial irregularities for the fiscal year.
