PBC Program Request Procedures 90 Days Before Program Procedure 90 days before program Step One 1. Request date availability from Sister Wilson. (Is the date you want open on the church calendar) 2. Submit program request to Sister Betty Polin or Sister Loraine Thompson. (Fill out this form and give it to the two selected program committee members) 3. Program OVERVIEW/Outline (Attach an outline of what you are planning to do during the program.) 4. Speaker/Guest Request - Select a minimum of two choices. Sister Wilson will send out an invitation after Pastor has approved the guest. No communication may be sent by any other person or auxiliary. First: Second: Name Church Affiliation 5. List committee member s names and include their individual responsibilities 3. PBC Program Request Procedures 30 Days Before Program Procedure 30 days before program Step Three 1. Program Updates (Identify changes or additions you have had to make during the past few months) 2. Submit FINAL Program to Sister Banks 3. Submit selected paper, type setting and any other details for published program. 4. Fill out the appropriate form for the event, if applicable: Check Request if you need to reserve an outside venue, to request payment of the events speaker/guest, etc. Ministry Card Request - if items need to be purchased for the program/event.
